STAGE 1: MANDATORY REQUIREMENTS

1. A Certified copy of certificate of Incorporation/Registration Attach
2. A certified copy of a valid tax compliance Attach
3. A dully filled, signed and stamped business
questionnaire(pg 4)

4. A signed and stamped declaration not to engage in
corruption practices.(pg 5)

5. A copy of YAGPO certificate(where applicable) Attach.
NB. 1. Interested bidders MUST meet the mandatory all requirements to proceed to the next
stage.
2. Those who have YAGPO certificates will NOT be subjected to Technical Evaluation
stage.
