QOF Dementia 2015/16

DEM 1 The contractor establishes and maintains a register of patients diagnosed with dementia

DEM 2 The percentage of patients diagnosed with dementia whose care plan has been reviewed in a face-to-face review in the preceding 12 months

DEM 3 The percentage of patients with a new diagnosis of dementia recorded in the preceding 1 April to 31 March with a record of FBC, calcium, glucose, renal and liver function, thyroid function tests, serum vitamin B12, and folate levels recorded between 12 months before or 6 months after entering on to the register

• We now have to produce a care plan which

has been reviewed in a face-to-face review in the preceding 12 months

• More money as points more than double (from 15 to 39!) • Bloods can now be done 12 months before

or 6 months after entering on to the register

• Most but not all patients will probably benefit by going

on the AUA ES (these patients are reviewed every 3/12 so you can build up the information in the care plan over time)

• A lot of the codes will pull across into both templates

• The Dementia Care Plan would then be printed off and form part of the AUA ES Care Plan
