CORRIGENDUM TO DIVISION MEMO NUMBER 162, S. 2017 ENTITLED "CITY SCHOOLS DIVISION MONITORING AND EVALUATION OF CURRICULUM IMPLEMENTATION AND SCHOOL OPERATIONS"

TO: OIC-Asst. Schools Division Superintendent CID Focal Person SGOD Focal Person Education Program Supervisors School Heads ( Public Elementary, Junior and Senior High Schools) All Other Concerned

1. This is to inform all concerned about the Monitoring Tool for InstructionalSupervision for School Heads and Department Heads to be used on theScheduled Monitoring and Evaluation by the CID Personnel particularly theEPSs in all schools within Imus City Division.

2. All other provisions stated in the Memorandum still remain in effect.

3. Please see attached Monitoring Tool.

4. Immediate dissemination of and strict compliance with this Memorandum isearnestly desired.
