Enrolling in Benefits:

Newly Hired Benefits Eligible Associates

Click on Benefits Change Task in Workday Inbox

To add a Dependent, click the 3 bars at the right in the highlighted cell for the plan in which you want to enroll.

Answer the below questions and click OK.

Complete all of the highlighted boxes and click OK. A Social Security Number must be added under “National IDs”.

To add additional dependents, click in the dependent cell again and on 3 bars at right side of cell.

Scroll to the Surcharge section at the bottom of the page, click in the cell and choose the appropriate reason. A $50 surcharge for covering a spouse will be charged unless a Spouse Surcharge Waiver Form is submitted to benefits@choc.org.

Can enroll in the Health Savings Account only if elected the Cigna HDHP. Click “Continue” if did not.

Elect Flexible Spending Accounts (FSA) on this page.

Elect Life and Disability insurance on this page.

List Beneficiaries of Life Insurance on this page. Completing this page is optional. Click “Continue”.

Enroll in Group Legal Plan on this page. Can also elect to use your badge when purchasing items in the CHOC Hospital cafeteria.

Review your elections, click “I Agree” and click “Submit”.

A $50 Surcharge for covering a spouse will be charged unless a Spouse Surcharge Waiver Form is submitted to benefits@choc.org. The Waiver Form can be found under the Resources tab at http://benefits.choc.org.

If you enrolled dependents, Workday will send a Task to your Inbox to upload proof of dependent documents. Documents for each covered dependent must be uploaded into Workday before benefits coverage will start.

Only 1 document may be uploaded, so all documents (e.g., proof of marriage, divorce, adoption, birth, loss of coverage) must be provided in 1 document.
