Mosaic was formed on July 1, 2003

when Bethphage and Martin Luther Homes consolidated their ministries into one organization that serves thousands of people nationally and internationally.

Case Management: Coordination of services and supports for people who might receive other services or supports from Mosaic or another service provided.

Children's Services:Services provided to people under the age of 21.

Supports may include residential services,supported/intermittent, host homes, day services(transition),special education, daycare, or early intervention.

Day Services: Any vocational services, supported employment,

job coaching, community participation or habilitation training.

Residential: Supports are provided 24hours/day and 7 days/week.

We provide affordable and accessible housing. BothICF/MR and waiver residential settings are included.Generally these services are provided in an agencyleased or owned-facility.

Supported Living: Supports are provided fewer than 24 hours per day and can be provided in a home owned or leased by the client of the agency.

Supports can vary from a few hours a week to several hours a day. These services are provided through a waiver facility and usually are based upon the needs of each person.

Host Homes: Services provided by anindependent contractor while individual lives in that person's home.– Individuals served in host home settings have a range

of care needs, from total care (24 hours a day, seven days a week) to limited care (those who are semi-independent and employed in the community).

Affordable and Accessible Housing: Because most people with developmental

disabilities are not able to find competitive employment, poverty is a serious issue.

Mosaic partners with state and federal agencies to create affordable housing that is accessible for all people. Residents must meet federal income guidelines to qualify.

● We are the first national provider of disabilities services to earn a Network Certification in Basic Assurances(SM) from CQL.

● This means that we have common expectations for high quality services and that they are monitored and measured for effectiveness at all locations.
