✓ Certificate of Worker's Compensation Insurance- Minimum $1,000,000 per occurrence and combined single limit coverage for bodily injury or disease;
✓ Certificate of Automobile Insurance- Minimum $1,000,000 per occurrence and combined single limit coverage for bodily injury and property damage; and,
✓ Certificate of General Liability Insurance- Minimum $1,000,000 per occurrence and combined single limit coverage for bodily injury, personal injury and property damage.
Completed M1W Liquid Waste Truck/Fleet Registration Form, Signed and Certified
Valid Monterey County Department of Environmental Health Non-Hazardous Liquid Waste Permit Inspection Form and/or Permit

ATTENTION PERMITTED GREASE HAULERS
Inedible Kitchen Grease Manifest Protocol
The State of California Department of Food and Agriculture (CDFA) passed legislation regarding Inedible
Kitchen Grease Manifest requirements (Article 42, Subchapter 2, Chapter 4, Division 2 of Title 3 of the
California Code of Relations,
Section 1180.24) which became
effective April 1, 2013. This
legislation requires commercial
transporters of Inedible Kitchen
Grease (IKG) in California to
create and maintain a manifest
for each collection, transport, and
delivery of IKG. IKG Manifests
may be maintained on forms
provided by the Department of
Food and Agriculture (see
Example 1.1), or on forms of your
own creation which document the
same information.
Example 1.1 CDFA Inedible Kitchen Grease Manifest
As of April 1, 2013, all grease discharges brought to the M1W Regional Treatment Plant must have the
accompanying IKG documentation attached to the M1W Liquid Waste Hauler Manifest. All trucks used to
haul grease must be permitted with the CDFA and the authorized sticker displayed in a visible location on the
truck.
