• Revenue pressure will most likely resultin cost-cutting initiatives and clinicalresearch expenditure might benegatively impacted.
• Additionally, as the innovativepharmaceutical market consolidationprogresses, small R&D orientedcompanies are acquired by largerplayers. As a result, pressure forefficiency and searching forsynergies may decrease the overallnet R&D spend.

• For academic as well as commercial clinical trials, thenumber of amendments is increasing: increase ofapprox. 7% in the total number of amendments per clinicaltrial for the period 2009-2010.
• Although the commercial trials represent approx. 85%of the total number of clinical trials, they account forapprox. 95% of the total number of amendments.
• The increase in total number of amendments mightbecome difficult to manage.

• The FAGG approval timecorresponds to complete filesubmission until the final approval ofthe dossier. The time for providingadditional information in order tocomplete the dossier and the time torespond to objections ("clock-stopperiod"), has been excluded from thiscalculation.

• Emerging Countries– Emerging countries are attracting a growing number of large-scale
clinical trials as they have access to large patient populations requiredto run these trials.
– The costs of running clinical trials is lower.– The shift to the “rest of the world” has increased markedly in recent
years and the trend looks set to continue, ultimately leading to a drop inclinical research activities in EU and US.

Based on a PwC analysis performed in 2010, Belgium is scoring well compared to the other EUcountries & US, but the cost to perform clinical trials is still 10% more compared toPoland and more than 20% compared to China & Russia.As a consequence, it can be expected that by 2012, the clinical trial market for CRO’s willincrease by 15% for Eastern Europe, compared to only 8,5% for Western Europe.

• Clinical trials imply a number of tangible and intangible benefits to the hosteconomy
• Clinical research market impacts a number of stakeholders and many of thembenefit from clinical trials as an additional source of cash flow
• More than one-fourth of revenue earned on clinical trials by sponsors ends up as adirectly paid tax contribution to the state budget
• Clinical trials can provide early access to innovative treatments

Turning around the market decline and maximizing the potential benefits to theeconomy is possible providing that a flexible framework is provided, transparency isenhanced, a responsive structure is implemented and that cooperation &communication are encouraged.

Conclusions and recommendationsIn the light of these market forces, research organizations /companies will need to focus on shorter drug developmentpaths, process efficiency and higher trial flexibility

Companies need to focus on strategies that:
Shorter and morefocussed drugdevelopment paths(less trial & error)
• The future drug development process is expectedto be more refined, as more emphasis will be puton fundamental research (translationalresearch).
Increased focus onprocess efficiency
• Sponsors seek for increased operational (andcost) efficiency, primarily by outsourcing trials toCROs and considering new locations for researchprojects.
Higher developmentflexibility
• Personalized drugs and shorter developmentpaths as well as focus on efficiency will result inrequirements for higher trial flexibility.
• Adaptive trials and smaller trials with highercomplexity will be managed, but recruitingpatients that correspond to all criteria andtimings will become more challenging.
• Life licensing: step-wise approvals.
