Qualified Person’s Statements and CertificationsAs the qualified person, I certify that:
A phase one environmental site assessment of the RSC property, which includes the evaluation of the information gathered from a records review, site reconnaissance, interviews, a report and any updates required, has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation.
A phase two environmental site assessment of the RSC property, which includes the evaluation of the information gathered from planning and conducting a site investigation, a report, and any updates required, has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation.
The information represents the site conditions at the sampling points at the time of sampling only and the conditions between and beyond the sampling points may vary.
As of 2013/11/25, in my opinion, based on the phase one environmental site assessment and the phase two environmental site assessment, and any confirmatory sampling, there is no evidence of any contaminants in the soil, ground water or sediment on, in or under the RSC property that would interfere with the type of property use to which the RSC property will be put, as specified in the RSC.
Ground water sampling has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation.
As of 2013/11/25, in my opinion, based on the phase one and phase two environmental site assessments and any confirmatory sampling, the RSC property meets the applicable full depth generic site condition standards prescribed by section 36 of the regulation for all contaminants prescribed by the regulation in relation to the type of property use for which this RSC is filed, except for those contaminants (if any) specified in this RSC at Table 2, Maximum Contaminant Concentrations Compared to Standards Specified in a Risk Assessment.
As of 2013/11/25, the maximum known concentration of each contaminant in soil, sediment and ground water at the RSC property for which sampling and analysis has been performed is specified in this RSC at Table 1, Maximum Contaminant Concentrations Compared to Applicable Site Condition Standards.
I am a qualified person and have the qualifications required by section 5 of the regulation.
I have in place an insurance policy that satisfies the requirements of section 7 of the regulation.
I acknowledge that the RSC will be submitted for filing in the Environmental Site Registry, that records of site condition that are filed in the Registry are available for examination by the public and that the Registry contains a notice advising users of the Registry who have dealings with any property to consider conducting their own due diligence with respect to the environmental condition of the property, in addition to reviewing information in the Registry.
The opinions expressed in this RSC are engineering or scientific opinions made in accordance with generally accepted principles and practices as recognized by members of the environmental engineering or science profession or discipline practising at the same time and in the same or similar location.

Phase Two Conceptual Site Model
81 Nelson Street, Hensall, Ontario
Summary
R.J. Burnside & Associates Limited (Burnside) was retained by the Trustee Board of the
Presbyterian Church in Canada to complete a Phase One Environmental Site Assessment
(ESA) and a Phase Two ESA to examine the property located at 81 Nelson Street in Hensall,
Ontario (hereinafter referred to as the “Site”). Figure 1 provides a site location map.
The purpose of the ESAs is to support the filing of a Record of Site Condition (RSC) for a
change in land use from community to residential, as there are plans to convert a church on the
Site to a single-detached residential dwelling. The church was constructed prior to 1896 and
has been used for more than 100 years by various church congregations for religious
gatherings, but has been vacant since 2010. There is no previous RSC filed in the Registry for
the Site.
It is Burnside’s understanding that there have not been any spills at the Site and there have not
been any remedial actions at the Site. The Phase One ESA did not identify any actual
contamination at the Site. The Phase One ESA identified only one potentially contaminating
activity (PCA) on the Site (an above-ground storage tank inside the church). An area of
potential environmental concern (APEC) was identified in the vicinity of the tank.
A Phase Two ESA was completed, examining the soil and groundwater at the Site for potential
petroleum hydrocarbon contamination (PHC F1 to F4, including BTEX) in the vicinity of the
above-ground storage tank. The analytical results for soil and groundwater samples collected at
the Site confirm that concentrations of contaminants of potential concern were below the
analytical detection limits and below the Table 2 Residential Site Condition Standards for PHC
F1 to F4 and for BTEX.
The Phase Two Conceptual Site Model, which represents the current condition of the Phase
Two property, has been prepared in accordance with Schedule E, Part V, Table 1, S6(x) of
Ontario Regulation 153/04 (as amended). No contaminants are present at concentrations
above the Table 2 Residential Site Condition Standards. Based on the work completed to date,
submission of a Record of Site Condition for Residential land use is considered appropriate.

1.0 Property Information
The Site is a rectangular shaped property on the east side of Nelson Street with a road frontage
of 14.6 m, a depth of 28 m and a total area of 408.8 m2 (0.041 hectares). Figure 4 identifies the
Site boundary, the location of the church, location of the tank inside the church, location of PCA
and APEC associated with the tank, location of the monitoring wells, neighbouring land use,
roadways and the inferred direction of shallow groundwater flow. The Site is owned by the
Trustee Board of the Presbyterian Church in Canada (the sole beneficial owner of the property
in fee simple) and the church has been vacant since 2010.
The municipal address of the Site is 81 Nelson Street, Hensall, in the municipality of Bluewater,
Huron County, Ontario. Land registry information states the legal description of the Site is Part
Lots 46 and 47, Plan 262, Hensall as in HE2541; S/T and T/W HE2541 partially released by
R119646; Municipality of Bluewater. The property identification number (PIN) is 41225-0162.
The Assessment Roll Number is 4020110001095000000. A letter from the client’s lawyer
confirming the above legal description is attached to the Record of Site Condition.
1.1 Historical Land Use
A plan of the village of Hensall dated 1875, referred to as “Petty’s First Survey” shows the Site
was owned by George and James Petty, founders of the community of Hensall. The Site is
occupied by the Carmel Presbyterian Church, which was constructed prior to 1896. The church
has been used for more than 100 years by various church congregations for religious
gatherings, but has been vacant since 2010. Nelson Street borders the west boundary of the
Site. Properties to the north, west and east are residential. Immediately south of the Site is a
right-of-way (a laneway), which provides access to a residential garage on the residential
property east of the Site.
1.2 Planning Conversion to Residential Dwelling
As the Site was no longer being used by the church organization, the property was put up for
sale. A number of buyers have shown an interest in the Site, with plans to convert the church
into a single-detached residential dwelling. The conversion plans are simple modifications to
make the interior of the building suitable for residential use, while maintaining the character and
structure of the building.

2.0 Regional Setting
2.1 Regional Topography / Water Bodies
The Site is within the Black Creek watershed. There are no water bodies in the study area.
Lake Huron is located about 15 km west of the Site. Elevation of the study area ranges from
about 270 to 275 m above sea level (asl) with regional topography gently sloping to the west
(Figure 2). The topography of the Site is flat. Regional groundwater flow is interpreted to flow
westward, towards Lake Huron.
There are no areas of natural significance in the study area.
There are no drinking water wells at the Site or in the study area. The Site and study area are
serviced by a municipal water supply. Treated water is delivered to the Hensall Distribution
System by a trunk main, from the Lake Huron Primary Water Supply System.
2.2 Regional Geology
Geological information from the MOE Well Record database indicates the uppermost layer of
the overburden in the study area consists of sand and silt (1 to 2 m), overlying a thick clay layer
(about 20 m thick). Bedrock, at a depth of about 22 m below ground surface (bgs), is described
as limestone, dolostone and shale of the Dundee Formation. Figure 3 provides a cross section
of the interpreted regional geology.
3.0 Potentially Contaminating Activities
The Phase One ESA identified an above-ground storage tank, located inside the church, as a
potentially contaminating activity (PCA), listed in Schedule D, Part VI, Table 2, under Item #28
(Gasoline and Associated Products Storage in Fixed Tanks). There were no other PCAs
identified on the Site. There were no indications that soil or fill material had been brought from
another property and deposited on the Site.
The tank is situated above grade, inside the west wall of the washroom (Figure 5). The tank
was formerly used to store furnace oil (decommissioned in 1988). There was no odour, staining
or evidence of contamination observed near the tank, on the floor, or in the crawl space beneath
the floor.

the Phase One ESA. Due to their off-site locations, distance from the Site and the direction of
groundwater flow, it is the opinion of the QPESA that these off-site PCA locations are not areas of
environmental concern in relation to the Site.
4.0 Areas of Potential Environmental Concern
An area of potential environmental concern (APEC) was identified by the Phase One ESA in the
vicinity of the above ground tank located inside the church (tank is identified as a PCA). There
were no other areas of potential environmental concern identified at the Site. The contaminants
of potential concern associated with the tank are petroleum hydrocarbons (PHC F1 to F4),
which includes benzene, toluene, ethylbenzene and xylenes (BTEX).
Soil and groundwater in the vicinity of the above ground tank were identified as the media that
may potentially be impacted by the contaminants of potential concern.
The Site is serviced with overhead electrical wires. Other utilities (natural gas, telephone) enter
the south side of the church and would not likely affect potential contaminant transport.
5.0 Phase Two ESA Sampling Program
The church is a one-storey building with a sunken lower level (basement). The ground surface
beneath the church is about 0.4 m below grade. During the Phase Two ESA, three boreholes
were advanced in the vicinity of the fuel tank (PCA), to collect soil and groundwater samples in
the area of potential environmental concern (APEC). One borehole was advanced outside, into
the ground on the north side of the church. Two boreholes were advanced from inside the
church, into the ground surface beneath the church. There was no evidence of non-aqueous
phase liquids (NAPL), free product, unusual odours, staining or sheen in the soil or
groundwater. There were no indications of any contamination.
Soil and groundwater samples were analyzed for contaminants of potential concern associated
with the tank, specifically, petroleum hydrocarbons (PHC F1 to F4), which includes benzene,
toluene, ethylbenzene and xylenes (BTEX). The analytical results confirm that concentrations
of contaminants of potential concern were below the analytical detection limits and below the

5.1 Physical Setting
The Site stratigraphy is similar to the local stratigraphy reported in historical borehole logs of the
Hensall area from the MOE Well Record database. The Phase Two ESA drilling program
determined that the uppermost layer of the stratigraphy of the Site consists of native sand and
silt, extending to a depth of 1.4 to 2.0 m below ground surface, overlying native clay till.
The clay till layer extends from a depth of about 1.4 m below ground surface (bgs) to beyond the
ends of the boreholes (greater than 4.5 m bgs). Grain size analyses characterize the clay till as
fine textured soil, containing 82% to 85% by mass, particles smaller than 75 µm.
Based on local well records, the clay till layer is about 20 m thick, overlying bedrock
(encountered at a depth of about 22 m bgs) consisting of the Dundee Formation, described as
limestone, dolostone and shale.
Static water level measurements indicate the groundwater level is 3.5 m bgs and is present in
the clay till, with a perched water table at about 1 m bgs in the overlying sand and silt layer. The
horizontal hydraulic gradient in the perched water table is calculated to be 0.08 to the west and
the hydraulic gradient in the clay till unit is estimated to be about 0.08. Shallow groundwater
flow direction is interpreted to flow westward. Cross sections illustrating the interpreted Site
stratigraphy are shown in Figure 6 and Figure 7.
5.2 Applicable Site Condition Standards
5.2.1 Soil SCS
For characterization purposes, the results of the soil analyses were compared to the Ministry of
the Environment’s Soil, Ground Water and Sediment Standards for Use under Part XV.1 of the
Environmental Protection Act, Table 2: Full Depth Generic Site Condition Standards in a
Potable Ground Water Condition.
The following were considered in determining the applicable Site Condition Standards:
The residential standards were applied as there are potential plans to convert the church to
a single-detached residential dwelling.
Soil samples were collected from the sand and silt layer, therefore, the coarse textured soil
criteria were used for comparison to analytical results.

Soil samples were collected from each borehole and submitted for pH analysis, ensuring
that a shallow soil sample, less than 1.5 m below ground, and a soil sample collected from a
depth below 1.5 m was analyzed for pH. Values range from 7.61 to 7.97, confirming that
soil pH is within the acceptable range and does not classify the Site as sensitive.
The Site is not a shallow soil property and is not within 30 m of a water body.
There are no areas of natural significance in the study area and the Site is not in an
environmentally sensitive area.
5.2.2 Ground Water SCS
The results of the groundwater analyses were compared to the Ministry of the Environment’s
Soil, Ground Water and Sediment Standards for Use under Part XV.1 of the Environmental
Protection Act, Table 2: Full Depth Generic Site Condition Standards in a Potable Ground
Water Condition for All Types of Property Use.
5.3 Analytical Results Based on the Phase Two ESA, the concentrations of contaminants of potential concern (PHCs,
including BTEX) in the soil samples collected at the Site are below the analytical detection limits
and below the Table 2 Residential Site Condition Standards for coarse textured soils.
Based on the Phase Two ESA, the concentrations of contaminants of potential concern in the
groundwater samples (PHCs, including BTEX) are below the analytical detection limits and
below the Table 2 Potable Ground Water Site Condition Standards.
A summary of Site conditions, including a table displaying analytical results for the three
sampling locations (MW-1, MW-2 and MW-3), is presented as Figure 8, showing that no
contaminants are present at concentrations above the applicable site condition standards.
6.0 Record of Site Condition
Based on the work completed to date, submission of a Record of Site Condition for Residential
land use is considered appropriate.
