Bipolar,MonopolarwithERS(impedancematchingsystem),andSuperficialRFRejuvenationTheallnewINTRAcelwaslaunchedatIMCASinSingaporeafterthesuccessfulcompletionofclinicalstudies.PresentationsbyDr.MichaelGoldandDr.YanTrokelfromtheUSA,Dr.ChingILuofTaiwanonINTRAceldemonstratedvariousadvantagescomparedtootheraestheticlaserandRFdevices.TheexpertgroupsparticipatinginthesestudiesinvestigatedanewmonopolarmodewithanimpedancematchingfunctionforsuperficialRFrejuvenation.ThenewfeaturesoftheallnewINTRAcelresultsinimprovedefficacyandreducedpainduringtreatment.RFMicroneedleandMono-polarFigure1.illustratesthatbipolarmodemakesanarrowercoagulationzonethanmonopolarmodeusingthesameenergy.Bipolarmodeshowsanarrowerzonewithstrongercoagulationduringtreatment,whereasmonopolarmodedevelopsawiderweakerdenaturationzone,whichexplainswhymonopolarhasreducedpainaswell.Infact,40%lesspainwasmeasuredinmonopolarmodethanbipolarmode.Thestudyalsoconfirmed30%shorterdowntimewithmonopolarmodethanwithbipolarmode.

Importanceofimpedancechecking

Impedancevariesbypatient,skintypeandtargettissue.Thus,predictingapreciseresultwithRFtreatmentisnotpracticalwithoutmonitoringimpedance.Inaddition,impedancevalueschangewhenRFenergypenetratesintohumantissue.Figure2.showsthedifferencebetweennormalmonopolarandmonopolarwithERS(EnergyRewardingSystem)byimpedancematching.Inthisexperiment,impedancematchingreducesbodyresistancetoallowsufficientenergyintothetargettissue.

FRM(FractionalRFMicroneedling)Monopolarforbetterresults.Manyaestheticpatientswhovisitedtheclinicdesiredimmediateresults.Ifitisdifficulttofindamoderatemethodtostimulatethesuperficialdermiswithnon-invasivemethods,orforpatientswhodonotwanttowaitforresultsoftreatmentsfor2months.Forthosepatientsmonopolartreatmentwouldbeanalternatesolution.Accordingtothestudy,theidealdepthtostimulateusingmonopolarmodeisapproximately0.8mm,thetypicaldistancebetweensuperficialdermisandmid-dermis.

LongPulseBipolarAstudyofenergydiffusionshowedthatLongPulseBipolar(BipolarLongOn-time)isnotabletoinfluenceaslargeanareaasmonopolar.Energyemissiontimedoesnotmakeasignificantdifferencebecausebipolarhasthecharacteristicoffocusingandemittingenergyatthetargetpoint.Inotherwords,nomatterhowlongtheenergyisemittedinbipolarmode,expandingthedenaturedareawouldbeunavoidablewithoutincreasedtimeandpain.Therefore,monopolarisapreferredsolutionforheatingandwarminglargepartsofskinbyexpandingthethermaldenaturationzonebetweenneedleswithlessenergyandshorterpulses.

• A24needletipforfullfacetreatmentisavailableforbothbipolarandmonopolar.• Anewnon-invasivesuperficialtipisavailableforFractionalRFResurfacing,whichcan

providepatientswithapainless,safeandmoreeffectiveresultonsuperficialareas.ThenewINTRAcelfromJeisysisthebestinclassproductforfractionalRFmicroneedlingfortheforeseeablefuture.HowThreeLayerTherapyWorks?
