HKCFP Research Fellowship 2012
Introduction
The HKCFP Research Fellowship was established by the Hong Kong College of Family
Physicians to promote research in family medicine. The grant is up to the value of HK$100,000.
It provides the successful candidate with protected time to develop research skills. Applicants
are expected to have regular contact with a nominated supervisor with research doctorate degree or equivalent.
Eligibility
Applicants for the HKCFP Research Fellowship must be active Fellows, active Full Members or
Associate Members of the HKCFP. New and emerging researchers are particularly encouraged
to apply.
Selection criteria
Applications will be judged on*:
• training potential of applicants
• relevance to family medicine and community health
• quality
• value for money
• completeness (incomplete or late applications will not be assessed further).
* Please note that new researchers and those at an early stage
of their research careers are encouraged to apply.

The HKCFP Award for the Best Research of 2011
The Research Committee of the Hong Kong College of Family Physicians has an Award for the
Best Research of the Year 2011. All members and fellows of the College are invited to participate
and submit their research papers to the Research Committee for selection. The Award will be
presented at the Conferment Ceremony in 2012.
Entry and assessment criteria are listed below:
Entry Criteria:
1. The principal investigator has to be a Member or Fellow of the Hong Kong College of
Family Physicians.
2. The research must be original work of the investigator(s).
3. The research should be done in Hong Kong.
4. The research must have been completed.
5. The paper should be presented under the standard headings of Abstract, Introduction,
Methodology, Results, Discussion and Conclusion. References should be listed in full
at the end in Harvard or Vancouver format.
Assessment Criteria:
1. How relevant are the topic and findings to Family Practice?
2. How original is the research?
3. How well-designed is the methodology?
4. How well are the results analysed and presented?
5. How appropriate are the discussion and conclusion(s) drawn?
6. How useful are the results for patient care in Family Practice?
7. How much effort is required to complete the research study?
Each research project submitted will be assessed according to the seven criteria listed above by
a selection panel. Each criterion may attract a different weighting to be decided by the selection

The following depicts the tortous details of a 5 day cycling tour in Tuscany at the height of summer 2011.
CYCLING IN TUSCANY

principles in each of our issues that i s to on l y pub l i sh h igh l y entertaining and leisurely articles which have no hints of hard work at all. But alas, I may be the first and last of our team to publish such a brow sweating ordeal of which I have described in detail below.
The beginning started with a brochure displaying pretty idyllic vineyards in the mist of a rising Tuscan sun. What better way to explore the very bowels of Tuscany than cycling along the picturesque village lanes amongst a meadow laden with flowers? So the trio of us set o with much enthusiasm on our 5 day bicycle tour of Tuscany in
Italy. Little did we notice the small print of warning at the very back of the brochure stating that cyclist had to be quite fit and take care not to exert themselves too despite the factthat this was not Tour de la France.
We were quite cocky after the first hour of cycling and congratulating ourselves that we weren’t too far behind from our fellow travelers. By the end of the day, we could hardly find the strength to eat or find the bit of buttock that cou ld s i t comfortably on a hard c h a i r . S o I h a v e detailed our chronicles:
Monday
Setting offfrom Florence. We stop at a beautiful vineyard for lunch and wine tasting in its century old cellar. Then it’s all uphill and downhill all the way to Greve.
Tuesday
GreveWe set off for a loop tour in Greve where we visit the 11th century abbey of Badia di Passignano and enjoy dinner at the Michelin awarded Osteria di Passignano.
Wednesday
PanzanoWe stop at this small town where we meet Dario Cecchini, one of the most famous
Tuscan butchers (shown in photo below). Dario has appeared in Jamie Oliver’s cookbooks!
Thursday
RaddaWe meet many other friendly fellow cyclist on our way who shout - “the girls again!” They have repeated the route twice whereas we’re still finishing one lap.....
Friday
SienaAt last we arrive at the beautiful medieval city of Siena. Most importantly- we survive the trip!

Genogram Drawing Made Ridiculously Simple!
Drawing a genogram is a basic and importanttaskforeveryfamilydoctor,especiallyforFMexitexamcandidates. Thereareproblemswith thetraditionalhand-drawngenogram:
1) Needtoupdate theageofvariousmembersinthegenogrameveryyear.Althoughyoucanchoosetowritedowntheyearofbirth insteadoftheagewhiledrawingthe initialgenogram,itisactuallyquitetroublesometofigureouttheactualageofeachpersonwhenyouarereadingthegenogramduringaconsultation.
2) Whenyouaddnewmembers toanexistinggenogram,suchasanewbaby,oranewmaritalrelationship,youmayhavenospace todrawthem,andyoumayneedtoredrawthewholegenogram.
3) Difficult toadda lotof information toeachmemberonthegenogram,anddifficulttokeepthegenogramtidywhenyouneedto rewritetheinformation.
4) Nowadaysfamilymedicineclinicsarebecomingmorecomputerized.Youmaywanttostorethegenograminanelectronicformat.

Toshowthelegendforthesymbolsusedineachparticulargenogram,simplypointthemousetoanemptyplaceonthechartandpress“Y”,andyouwillgetthefollowinglegends.

Youcanusepatient’s IDnumberorcasenumberasthefilenames.WiththepowerfulsearchfunctionofWindows7,youcansearchandopenaspecificgenogram f i le within f ive seconds! ( Warmreminder: for GOPC doctors who are storingthese genograms in their personal computersfor thepurposeofexitexam,neverkeepany IDnumberor identifiablepersonal information inyourcomputers.Ithinkusingcasenumbersasfilenamesshouldbeacceptable).
Are thereany imperfectionswith thisseeminglyperfectgenogramdrawingprogram?
1) Itcostsmoney,althoughitisquiteaffordable.
2) When tak ing a family h is tor y f rom yourpatients, theywillusually tell you theageofthe individuals in their families, insteadof thedateofbirthofeachperson. Therefore,youwillneedtotranslatetheageintoyearofbirthif youwant toshow theage. E.g.anage32personshouldhavebornin1978,thenyouneedtoenter1978tomakethesymbolshowsage32within it. FDA Recommends New Restrictions, Contraindications, and Dose Limitations for Simvastatin to Reduce The Risk of Muscle Injury
On6thAugust2011,theU.S.FoodandDrugAdministration(FDA)recommendedlimitingtheuseofthehighestapproveddoseofthecholesterol-loweringmedication,simvastatin(80mg)becauseofitsincreasedriskofmyopathy.Simvastatin80mgshouldbeusedonlyinpatientswhohavebeentakingthisdosefor12monthsormorewithoutevidenceofmyopathy.Simvastatin80mgshouldnotbestartedinnewpatients,includingpatientsalreadytakinglowerdosesofthedrug.FDAisalsorequiringchangestothesimvastatinlabeltoaddnewcontraindications(shouldnotbeusedwithcertainmedications)anddoselimitationsforusingsimvastatinwithcertainmedicines.
Myopathyisaknownsideeffectofallmedicationsbelongingtothestatingroup.Patientswithmyopathymaypresentwithmusclepain, tendernessorweakness,andelevationofcreatinekinase intheblood.Thehigherthedoseofstatinused,thegreatertheriskofdevelopingmyopathy. Rhabdomyolysis,themostseriousformofmyopathy,mayresult inrenalfailure.Patientswithrhabdomyolysismayhavedarkorredurineandfatigue,inadditiontotheirmusclesymptoms.
Data summary:TherecommendationsarebasedonFDA’sreviewoftheStudyoftheEffectivenessofAdditionalReductionsinCholesterolandHomocysteine(SEARCH)trial.Itwasaseven-year,randomized,double-blindedclinicaltrialcomparingtheefficacyandsafetyofsimvastatin80mgtosimvastatin20mg,withorwithoutvitaminB12andfolate,insurvivorsofmyocardialinfarction.
52patients(0.9%)inthe80-mggroupversus1patient(0.02%)inthe20-mggroupdevelopedmyopathy(definedasunexplainedmuscleweaknessorpainwithaserumCK>10timestheupperlimitofnormal[ULN]).22patients(0.4%)inthe80-mggroupversusnopatientinthe20-mggroupdevelopedrhabdomyolysis(definedasunexplainedmuscleweaknessorpainwithserumCK>40timesULN).Therewerenofatalitiesrelatedtorhabdomyolysis.
Therisksformyopathyandrhabdomyolysiswithsimvastatin80mgwerehighest inthefirst12monthsoftreatment,5per1000person-yearsand2per1000person-years,respectively,anddecreasedto1per1000person-yearsand0.4per1000person-yearsafterthat.
Olderageandfemalesexareassociatedwithincreasedriskofmyopathy. InSEARCH,theriskofmyopathywasapproximatelydoubledinpatientstakingacalciumchannelblocker, inparticulardiltiazem.Approximately60%ofthecasesofmyopathywereassociatedwithageneticvariantwhichaffectsthecodingofthetransporterresponsibleforsimvastatinuptakeintotheliver.Thisvariantincreasestheplasmaconcentrationofsimvastatin,thusincreasingtheriskofmyopathy.
ThefindingsfromtheSEARCHtrialaresupportedbyanalysesoftheFDA’sAdverseEventReportingSystem(AERS)database,whichshowthatthelevelofreportingoffatalrhabdomyolysisassociatedwiththe80-mgdoseofsimvastatinhasbeenhigherincomparisonwithlowerdosesofsimvastatinor lowerdosesofmostotherstatins. Inaddition,clinicaltrialdatafromotherlong-termstatintrialsshowedhigheroverallratesofmyopathyandrhabdomyolysis inpatientstreatedwithsimvastatin80mgversuslowerdosesofsimvastatin.
Recommendations to healthcare professions:1. Maintainpatientsonsimvastatin80mgonly if theyhavebeentakingthisdosefor12ormoremonthswithoutevidenceof

Dr. Shih Tai Cho is a dermatologist in privatepractice. During themeeting,Dr.Shih sharedwithushispersonalexperiencewhenfacedwithcommonmisdiagnoses in skindiseasesandhisapproachtomanagement.
Learning points:In making a diagnosis of eczema, always notethe multi-morphologies like redness, dryness,desquamation, itchinessetc.atdifferentsitesofinvolvement. Thisdiagnostic feature is inclearcontrast toskindiseasewithsinglemorphology,e.g. chickenpox. Dr.Shih tends toavoidusingtopicalsteroidonface,andpreferstousetopicalcalcineurin inhibitorsexcept for theveryyoung.Howeverhe findstopicalcalcineurin inhibitors inotherbodyareasnotaseffective.
Scabiesisoneofthemosteasilymisseddiagnosisas it can also present with dif ferent types ofmorphologies (including urticaria), as the skinlesionsaremostlyduetosensitivity reactions tothescabiesantigen. Always look forareas likegroinsandaxillaeas theparasitespreferwarm,moist locations.Acarefulhistorytakingofrecentcontactandfamilyhistoryisuseful,thoughitisnotuncommonthatthesehistoriescannotbeelicited.Indoubtfulcases,DrShihrecommendsusinganti-scabiestreatmentonabasisoftherapeutictrial.
Ifone findsa tinea infectionofhandsand feetdifficult to treat,always thinkof thealternativediagnosisofpompholyxwhichcanbeaggravatedbyhotweather,anxietyandexcessivesweatingofthehandsandfeet.
Rosaceacanco-existwith facialeczema. Itcanalsobeinducedbyuseoftopicalsteroidintreatingfacialeczema–SteroidRosacea. Look for thepresenceofseborrhea,telangiectasiaandrelativelack of itchiness. Ask for aggravating factorslike hot environment, inadequate rest, anxietyetc.Thephysiciancanadviseagainstuseofoilymoisturizers,trytostopusingtopicalsteroidsbutwarnaboutreboundphenomenon,andprescribetopicalcalcineurin inhibitors inadditiontousualanti-rosaceamedications.
In seborrhoeiceczema, look for involvement inearcanalandscalpmargin. Avoidusingtopical
steroid. Use topical ketoconazole shampooonface, scalpandear canal. Sometimesonecanprescribeshortcourseoforalitraconazoleaswell.
In treating atopic eczema, empathic attitudetowardsboth thepatient and thecarer is veryimportanttogaintheircooperation. Tryto instillthe concept of avoiding excessive cleaniness,avoidhot tub-bath, restrictbathing to less thanfiveminutes,andusesimpleemollientsasmuchaspossiblepreferablyat leastthreetofourtimesper day. Adding a short course of oral anti-staphylococcalantibioticsduringacute flare ishelpfultoreduceuseoftopicalsteroids.
Pityriasisalbacanbedifferentiated fromvitiligoeasilybyusingUVlightinadarkroom:vitiligowillshowdepigmentationwhereas inpityriasisalbatheinvolvedareaswillhavesimilarappearanceassurroundingskin.
Inmelasma,bordersareusually indistinctwhereasin solar lentigines, thebordersareusuallywell-defined.
Syringomas usually only involve lower eyelidswhereasplanewartsusually involveotherpartsoffaceaswellandthesurfaceofwartsareusuallymorerough.
