“Transition Services”The term transition services is defined

as a coordinated set of activities for a child with a disability that:

+ Is designed to be within a results-oriented

process, that is focused on improving the academic and functional achievement of the child with a disability to facilitate the child’s movement from school to post-school activities, including

post-secondary education vocational education integrated employment (including supported

employment) continuing and adult education adult services independent or community participation

+ Is based on the individual child’s needs,

taking into account the child’s strengths, preferences, and interests

Each student must have included in the IEP appropriate measurable post-secondary goals based on age appropriate assessment related to: Training Education Employment, and where appropriate Independent living skills The transition services (including courses of study)

needed to assist the child in reaching those goals

+What does a Transition Coordinator do? Works with Local District Coordinators and

Special Education teachers to assist students who will be transitioning from school to adult life

How??? By providing information and referral services By attending IEP meetings By assisting in planning and placement decisions By coordinating and assisting with Transition Fairs By organizing summer work opportunities for high

school students

Instruction and related services, community experiences, development of employment and vocational evaluation All of these activities are coordinated based on

the student’s individual needs

A student is ENTITLED to services from school for as long as he/she is in school

A student must be found ELIGIBLE for services after high school

+Strategiesfor Successful Transition Start early! Encourage students to apply for County

Board Services as soon as possible in order to be placed on the waiting list.

The student should be driving the goals, wants, needs, and actively participate in the entire process

Help the student and the teams have an awareness and understanding of strengths and limitations

Help parents buy into the outcome of the plan and the student becoming employed

Attend special events (transition fairs, job fairs, summer employment, etc.)

Good collaboration with all persons and community organizations who are involved with the student

+Where to begin?

You may refer a student for transition support from Personnel Plus by filling out a referral form

We will then contact the student and begin working with him/her in collaboration with the school

Referral forms can be obtained from the Personnel Plus office or from Chris Stewart, via email or fax

Parents, teachers, guidance counselors, and other school personnel may refer a student who is age 14 or older and has an IEP

Students over 18 years of age may refer themselves

+Summer YOUTH

A summer employment program provided by PersonnelPlus.

•During the months of June and July

•For anyone ages 16-21

•Must have an IEP (Individual Education Plan)

•Workers earn $7.30/hour

•Work 4 days a week (Mon—Thu), 8:00am—12:00pm

•Mondays are training days at Personnel Plus

•Tue, Wed, and Thu are work days at the job site

•Program is 6 weeks long
