Are you planning to travel seasonally?

Are you planning an extended trip away from

Phoenix, AZ?

If so, you may need some coordination of your medical care with another Department of Veterans Affairs (VA) facility if you do NOT already have an assigned physician in that location.

To ensure your care, including medication refills, continues uninterrupted during your extended travel, please inform your Primary Care Provider or Patient Aligned Care Team of your plans before you depart. This enables our staff can help ensure you can easily receive any needed or continuing VA medical services during your travels.

Also, be sure to let us know your temporary address and phone number so your medication refills can be sent to you at that location and so we can better assist with your care.
