Why person-centred collaborative care is important and what it involves

Our vision for the future

There is an increasing recognition of the great potential for new and better ways for people’s expertise to come together to ensure improved outcomes for people with long-term conditions.

But also frustration that the promising practice that can be seen in patches around the country is not becoming mainstream practice quickly enough.

We want to change this and work with those determined to move person-centred collaborative care from the margins, to the centre of practice and experience.

How do we plan to do this?

We want to create the right conditions for this to flourish, through:• Workforce development• Providing powerful evidence• Identifying and pulling system levers to

drive positive change in practice

It is about supporting and nurturing a powerful movement for change amongst practitioners, people living with long-term conditions, their family carers and the wider community.

Ensuring this ‘movement’ has the tools, connections and support it needs.

And that people with long-term conditions and their family carers are at the heart of this.

How do we plan to do this?

• We will play an important part in key developments that will shape the future of health such as the Integrated Personal Commissioning Programme

• We will look for all opportunities to get significant support targeted to those local systems showing clear commitment to transform practice. This includes support to the people with long-term conditions so they are able to contribute to this movement

• We will support our members to become better informed, to link together and develop their ability to have positive local impact and we will consult with members on how best to do this

How do we plan to do this?

Creating sustainable impact

There is an opportunity to make a real difference to the lives of people with long-term conditions.

But for deep, large-scale, sustainable change to happen we need to creative a collective, powerful movement.

We already have a strong group of national partner organisations and from today we’ll be actively encouraging additional

partners and members to join.
