This study assessed Nurse Practitioner members of NP-PittNet, an AHRQ registered Practice

Based Research Network, awareness of PA cAARds!, a Pennsylvania state intervention that

targets the dual diagnosis of Diabetes and Tobacco Use using the Ask, Refer, Refer method .

The purpose of this study was to 1) assess NP-PittNet NPs’ awareness, knowledge, and

utilization of PAcAARds! , and 2) explore interest and feasibility of PAcAARds! as a mobile

health application for utilization in the referral process and follow-up continuity of care strategies

members; an AHRQ registered Practice Based Research Network, and utilizes an Expert-Model

Framework to provide evidence based Diabetes Prevention program guidelines. Strategies are

presented to change the behavior of the health care providers when counseling patients at risk

for Type 2 Diabetes.
