Based Research Network, awareness of PA cAARds!, a Pennsylvania state intervention that
targets the dual diagnosis of Diabetes and Tobacco Use using the Ask, Refer, Refer method .
The purpose of this study was to 1) assess NP-PittNet NPs’ awareness, knowledge, and
utilization of PAcAARds! , and 2) explore interest and feasibility of PAcAARds! as a mobile
health application for utilization in the referral process and follow-up continuity of care strategies

Framework to provide evidence based Diabetes Prevention program guidelines. Strategies are
presented to change the behavior of the health care providers when counseling patients at risk
for Type 2 Diabetes.
