plan is current, please indicate as much; we will mark the web page as “Last Reviewed [current date]”. No further information is needed.

The site should contain an up-to-date assessment plan with planning going out a minimum of three years beyond the current year. Please review the

plan displayed for your department at the above site. The plan should include a list of courses from which data will be gathered and the schedule, as

well as an overview of the assessment strategy the department is using (for example, portfolios, or a combination of Chi assessment data and student

8 Report due 11/15/2017

F. Report of assessment results for the most previous academic year:

There are a variety of ways in which departments can choose to show evidence of learning. This is one example. The critical pieces to include are 1) what learning outcome is being assessed, 2) what method of measurement was used, 3) what the threshold for ‘acceptable performance’ is for that measurement, 4) what the actual results of the assessment were, 5) how those findings are interpreted, and 6) what is the course of action to be taken based upon the interpretation.

A. Evidence of Learning: Courses within the Major The following document is Standard 6 of Systematic Plan of Evaluation for the School of Nursing’s undergraduate program. Appendixes can be provided if needed.

Program evaluation demonstrates that students have achieved each end-of-program student learning outcome and each program outcome.

There are School of Nursing program outcomes and student learning outcomes:

SON Program Outcomes o Outcome #1: 80% of School of Nursing students will graduate from their programs within 150% of the stated length of the program. o Outcome #2: 80% of School of Nursing graduates seeking employment will be involved in role related professional practice within 6-12

months of graduation. o Outcome #3: 80% of those students who take certification/NCLEX licensure exams will pass on the first attempt.

SON Program Outcomes will be measured annually through the following methods: o Program Completion Rates o Employment rates 6 months after graduation o NCLEX Scores o Certification rates (MSN/FNP)

School of Nursing Student Learning Outcomes:

o Patient-centered Care: Recognize the patient or designee as the source of control and full partner in providing compassionate and

coordinated care based on respect for patients’ preferences, values, and needs.

o Teamwork and Collaboration: Function effectively within nursing and inter-professional teams, fostering communication, mutual

respect, and shared decision-making to achieve quality patient care.

o Evidence-based Practice: Integrate best current evidence with clinical expertise and patient/family preferences and values for delivery of

optimal health care.

o Quality Improvement: Use data to monitor the outcomes of care processes and use improvement methods to design and test changes to

continuously improve the quality and safety of health care systems, including participating in healthcare policy.

o Patient Safety: Minimize risk of harm to patients and providers through both system effectiveness and individual performance.

o Informatics: Use information and technology to communicate, manage knowledge, mitigate error, and support decision-making.

9 Report due 11/15/2017

The SON End of Program Student Learning Outcomes (EPSLO) will be measured annually through the following methods (Refer to Course

Maps (Standard 4) for alignment of Course outcomes/units and assignments to SON/End of Program Student Learning Outcomes):

o Student will report obtainment of Program Level Student Learning Outcomes at or above a 3.0/5.0 on the End of Program (EOP)

Survey

o 80% of ADN students will achieve a 50% or higher score on Kaplan Exam.

o 90% of students will receive a minimum on a B- or above for aggregate coursework (Course Grades)

Systematic plan for program (SPE) evaluation tool reports aggregated data, analysis of data, and program responses to the evaluation data with

reference to ongoing program development, maintenance, or revision will be completed maintained and annually by October of each year.

Please respond to the following questions.

1) Based on your program’s assessment findings, what subsequent action will your program take?

Findings are currently solid. Changes are made to each course annually based on evaluations by students and by faculty course committees.

2) We are interested in better understanding how departments/programs assess their graduating seniors or graduate students. Please provide a short narrative describing the practices/curriculum in place for your department/program. Please include both direct and indirect measures employed. Finally, what were your findings from this past year’s graduates? The School of Nursing utilizes end of course and end of program evaluations to assess how students perceived achieving program outcomes as well as satisfaction with the courses and materials with an expected level of achievement of 3. RN-BSN core course pass rates are evaluated with an expected level of achievement of 80%.
