Partnered with Jay Haapala, AARP, to present an overview of the Age-Friendly process to Area Agencies on Aging Eldercare Development staff throughout the state.
Regional Age-Friendly/Dementia Friendly Virtual Events were offered quarterly that included a presentation, discussion, and networking. A leadership team was developed that include regional providers to determine the on-going mission and vision.
Work continues with ACT on Alzheimer’s and new Dementia Friendly communities. When the pandemic forced cancellation of in-person events we connected current and new Dementia Friends Champion trained leaders to offer virtual information sessions.
Silos to Circles Communities
Two Silos to Circles communities continued. Cuyuna Area Connection offered virtual educational events. Chisago Age Well offered virtual Dine & Discover events and completed the process to become a 501c3 Nonprofit.
Workforce
Presented at the virtual Minnesota Board of Soil and Water Regions Academy. Those attending were statewide members that service homeowners including rural, agricultural, and lake area properties. Presentation
Through all of the challenges of 2020, the CMCOA Community Development Team continues efforts to maintain services,
networking, and advocacy.

Virtual Meetings/Events
As a result of the COVID19 pandemic and social distancing guidelines, hosted virtual Aging Network Meetings for community providers.
Led one of four projects funded by Blandin Foundation collaborating with Chisago County HRA-EDA and Chamber of Commerce to create EmPOWERED by Innovation. The project included the development of technology focused education for adults 50+ with on going technical support to be implemented in 2021.
Age-Friendly & Dementia
Friendly Communities
Worked with Princeton and St. Cloud leadership teams to begin the process to become Age-Friendly Communities.
focused on mental health and disability limitations for older adults and their caregivers, and ways to assist and communicate when meeting with individuals in their home.
With the increase of isolation for older adults due to the pandemic, mental health continues to rise as a top
priority.
Mental Health
Participation in regional Adult Mental Health Initiative (AMHI) meetings. Assisted East Central Innovations team to develop Quick Reference and Resource Guides.
As part of the St. Cloud Suicide Prevention Coalition participated in billboard campaign beginning fall of 2020 through the end of the year.
Partnered with Nystrom & Associates to offer general mental health training for older adults to CMCOA staff, Live Well at Home, and Title III Grantees.

Partnered with CentraCare Suicide Prevention Coordinator to offer a Suicide Prevention/Awareness
training focusing on older adults.
Promoted Wellness in the Woods offered WRAP (Wellness Recovery Action Plan) for older adults in Todd
and Crow Wing counties.
Housing with
Homelessness Focus
Continued to participate on local and regional housing focused groups including; Continuum of Care (CoC) and Family Homeless
Prevention & Assistance (FHPAP)
Trained CMCOA staff on housing and homeless needs for older
adults.
Partnered with Minnesota Engagement on Shelter & Housing (MESH) and Living at Home nurses program to provide older adult training to homeless providers in the region. The goal was to increase knowledge of in-home services and why they are critical to keep an
older adult stable in housing.
Collaboration with Alzheimer’s Association to develop training for housing providers for early 2021. The presentation will provide information on cognitive loss and how homeless providers can identify possible signs of cognitive loss and how they can provide support to Older Adults experiencing
homelessness.
Transportation
Continued participation in the Regional Transportation Coordinating Council (RTCC). Each of the regional councils have multiple priority areas including; Transportation Management
Coordination Center, Volunteer Driver Programs, Policy Action, Transportation Exploration, and Geographic Information Systems
(GIS) Mapping Project.
Participated in development of the Minnesota Council on Transportation (MCOTA) Volunteer Driver training series, which began in fall 2020 and
continues through spring 2021
Participated in Benton and Stearns counties Toward Zero Deaths (TZD). The focus for 2020 was the Highway 15 corridor to provide education, enforcement, engineering, and emergency medical and trauma services to improve roadways with the intention of reducing accidents and eliminating
fatalities.
Continuing to work to build strong communities in effort to expand and improve technical assistance, increase knowledge of grant opportunities and
sustainability activities.
Sustainability
Developed a presentation, Sustainability Funding and Beyond and presented to several nonprofit groups and Board of Directors. Plan to offer
more presentations in 2021.
Healthy Aging Coordination through
Evidenced-Based Classes
In March 2020 due to COVID19, Evidence-Based Health Promotion (EBHP) classes were cancelled. Allowing an
opportunity for the CMCOA Healthy Aging Coordinator (HAC) to work with state and regional evidence-based leaders to re-group and look for other opportunities to continue the
services.
Some of the EBHP classes were transitioned to virtual options. The task was to determine leaders that would continue to teach classes and locate equipment resources needed to
implement the various classes.
Developed a Social Connect class with three components: gentle chair movement (either yoga or Tai Ji), mindful stress management, and sharing. Some of the participants continued to meet on their own virtually after the classes were
over.
Social & Communication
Engagement
Worked with potential partners to offer a series of classes including; Mindful Presence, Social Connect, Aging Mastery Program, and Guided
Autobiography.
Facilitated a virtual Renewal and Refresh meeting for EBHP leaders across the region to provide, curriculum updates, review techniques, and provide updates especially as it related
to virtual options.

Community Living Specialists (CLS) assist nursing home residents who have complicated needs to return home and individuals who are living in their home to remain there as long as possible. CLS assisted 359 consumers to return to a community setting or remain in
the community in 2020.
54,695 Points of Contact
2020
Preadmission screening (PAS) is a process individuals go through to ensure it is appropriate for them to be placed into a long-
term care facility.
5,983 screenings were received in 2020. PAS Specialists received
4,936 (83%) referrals with an estimated stay of less than 30 days.
1,130 follow-up calls to individuals discharged from long-term care.
Central MN Council on Aging provides Senior LinkAge Line® services in the 14 county
central planning and service area, in partnership with the Minnesota Board on Aging.
During 2020, we assisted 34,941 individuals through 54,695 points of contact with older adults and caregivers providing information and resources on Medicare, housing,
transportation, financial services, and other community supports.
The call center Senior LinkAge Line Specialists locate services or research options to determine what kind of service might be helpful. Senior LinkAge Line Advocates do an in-depth assessment and
help navigate individuals to the appropriate community services.
