–  VaccinaPon  –  Exclusive  breas[eeding  –  Presence  and  state  of  Latrines  –  UPlizaPon  of  LLITNS  –  Source  and  preservaPon  of  water  –  Hand  washing  

ImplementaPon/DeclaraPon  of  Household  Visits  

•  Who  Conducts  Home  visits?  – Health  Facility  staff  –  Community  relay  Agents  (members  of  health  commi4ee)  

•  Frequency:  – Once  in  3  months  for  each  household    

•  Conduct:  – Household  register  for  each  community  – Monthly  declaraPon  to  Performance  Purchasing  Agency  

– Unit  cost  for  each  home  visit:  $2  (XAF1000)    

VerificaPon/ValidaPon  and  Payment  

•  Carried  out  by  PPA  staff  

•  DeclaraPon  concordance  with  Register  

•  Sample  drawn  in  register  for  community  verificaPon  (veracity)  –  5  households  per  community  per  month  – Acceptable  non-­‐concordance  rate  10%
