will be sent out before 20 June 2013 • Program Duration: 15 July – 26 July 2013 

 QUALIFICATIONS

• Non‐Indonesian citizens 

• 18 years or above with valid raveling documents 

• Able to communicate in English. For students from non‐English speaking countries, please provide evidence of English proficiency such as TOEFL score of 500 min, or similar to this  

• Completed application forms with supporting documents received by 30 May 2013 (please send in application forms and make queries if needed ahead of time) 

• Completed full payment received by 13 June 2013 

APPLICATION SUBMISSION & REQUIREMENTS • Deadline : 30 May 2013 • Applicants should submit following materials for the admission (applications submitted without all 

the required documents will not be considered): 1. Application Form (completed) 2. A certified copy of English or other Language Proficiency Test Result (only for applicants whose 

country of origin (country issuing passport) does not use English as the native language; test result obtained in the last two years) 

3. A recent photograph (3 x 4 cm) (attached in the Application Form) 4. A copy of passport (scanned document) 5. A copy of bank transfer payment slip (scanned document)
