    Application formAny omission of information required in the application forms or supporting documents will render the application voidand should be avoided. Please indicate N.A. or leave the column blank if any part of the form does not apply to you or

    no information is available. Please complete the application form in English.

    Supporting documents

    All supporting documents submitted in a language other than English must be accompanied by an officially translatedand certified copy in the English language. Documents submitted should be A4-sized and all envelopes are to beremoved (except the original full transcript to remain in official SEALED envelope). Please do NOT file or bind yourapplication documents. Original copies of supporting documents submitted by candidates will not be returned.

    paper. Once we begin processing your application, the card will be returned to acknowledge receipt of your application.We will also indicate on the card outstanding documents (if any).

    Transcript Request FormAn ORIGINAL official transcript of academic records, or its certified true copy, is required from each universityattended. Candidates with qualifications other than those from NUS are to request the transcript(s) from your relevantuniversity. Please use the transcript request form for this purpose. If you have attended more than one university, youshould make photocopies of the transcript request form as required.

    Application FeeEach programme applied for should be accompanied by the payment of an application fee ofS$40 (or S$20 if you areapplying online), inclusive of 7% GST. Do complete the Graduate Application Fee form and submit it together withyour cheque/bank draft. Cheques and bank drafts must be made out to National University of Singapore Please referto the application fee form for details on payment.

    TOEFL/IELTS ScoresheetsTOEFL/IELTS scores are required for applicants who graduated from universities where English is not the mainmedium of instruction. [Minimum TOEFL score 600 (paper-based test) or 250 (computer-based test) or92(internet-based test) or IELTS result of 6.5].Candidates who do not submit a satisfactory official TOEFL/IELTS

    score by the application closing date will not be considered.

    NUS Graduate Scholarship for ASEAN Nationals (NUS GSA)We advise applicants applying for this to be familiar with the eligibility, rules & conditions described in our applicationbooklet and/or website before applying. TOEFL or IELTS scoresheet is compulsory for application to the NUS GSA.Applicants who fail to meet the eligibility requirements need not apply.
