Overview
This Presentation provides the information you will need to apply for a Medical Reserve Corps (MRC) Capacity Building Award (CBA) for 2012-2013.
This presentation will cover: •Who is eligible to apply for each tier•Application requirements for each tier•General background information requirements•How to apply•Other important information

General Information• Capacity Building Award: 2 Tiers & Estimated Award Ranges
Tier 1: Non-Competitive Award- $2,500 - $5,000Tier 2: Competitive Award- $5,000 - $10,000
**The number of awards and award amounts will be determined by the number of applicants and available funding.**
• Applications must be completed online between Monday, August 27, 2012, and Friday, September 28, 2012, at 5:00 PM EDT
• Paper applications and supporting documents WILL NOT be accepted.

Who can apply for a CBA?
In order to be eligible for either tier, a unit must:• Be registered with the DCVMRC as of August 27, 2012 (prospective units in
the process of registration must have applied by August 27, 2012 in order to be eligible for a CBA);
• FULLY update your unit profile on www.medicalreservecorps.gov between July 1 and September 28, 2012;
• Have scheduled and/or participated in a Technical Assistance (TA) Assessment, if requested by the MRC Regional Coordinator (For units in existence for more than six months); and
• Be eligible to receive federal funds through the Sponsoring/Housing Agency or 501(c)(3) non-profit.

How to Apply for a CBA•To apply for the CBA, you must log into your NACCHO account on the NACCHO Website: https://eweb.naccho.org/eweb/DynamicPage.aspx?WebCode=LoginRequired&expires=yes&Site=NACCHO.
If you already have a NACCHO account, please log into the system and skip to Slide 12.
•If you are a New User, please create an account.

If you don’t recall your password, click “Forgot Password” then enter your e-mail address again. You will receive an e-mail with a link to reset your password. Refer to slide 11 and 12 for information regarding resetting your password.

Resetting Your Password• Follow the directions on the screen to reset your password.
• If you have requested to have your password reset, watch for an e-mail from [email protected]. This may take up to 24 hours to arrive in your inbox.
• If you do not receive the e-mail within 12 hours, check your spam filter and junk email. (For MS Outlook Users - Once you locate the email, right click on your mouse and scroll down to “junk email” then scroll over to “mark not as junk”. The system should then move the email to your “Inbox”).
• If you do not receive an email 24 hours and after you have checked your junk mail, contact [email protected] with your full name, state, and MRC unit and a temporary password will be issued to you.

Resetting Your Password (cont.)• Once you have received your email, click the link in the e-mail to reset your
password. Enter your new password and click “Change Password”.
• After you create your new password, you will be taken back to the login page.
• Log in with your new password.

Locating the ApplicationsOnce you have logged into NACCHO’s website:• Click “NACCHO Applications” located on the left side of the screen in the left-
hand menu.

Corrective ActionIf you do not see “NACCHO Applications” in the left-hand menu, you must contact [email protected] with your unit name, city, and state and indicate that you are unable to locate the application.

Next Step
Click the “Apply” button next to the MRC Capacity Building Award application you wish to work on. Remember, you can come back to work on the second application.

• Narrative responses are limited to 350 words. A word counter appears below the text box. You may type more than 350 words, however only the first 350 words will be submitted with your application.
• DO NOT use these symbols: &, ^, <, > anywhere in your application. Write out the words (and, greater than, less than, etc.). The database will not record any responses if these symbols are used.

Sponsoring/Housing Agency InformationQ: How would I know the legal name of our of sponsoring/housing agency? A: Your housing organization is the entity that manages your MRC unit’s finances.
Check with your Finance Director or office. If you are awarded a CBA, NACCHO will send a check to this agency or 501(c)(3).
Q: What is the sponsoring/housing agency employer identification number (Tax ID)?
A: This is the 9-digit number used by your agency to submit their tax forms
(e.g., 12-3456789).
Q: Who is considered the authorized signer for sponsoring/housing agency?A: This is the individual who is authorized by your agency/organization and obligate
your agency/organization to a contract. If you are awarded a CBA, this person’s name will appear on your contract as the representative of the sponsoring/housing agency.
Note: NACCHO cannot mail contracts or checks to a PO Box.

Preliminary Budget Form• NACCHO has provided a required preliminary budget form for you to complete in
Microsoft Excel.• The NACCHO Preliminary Budget Form, Preliminary Budget Form Sample and
guidance document can be found here, under “Supporting Documents” http://www.naccho.org/topics/emergency/MRC/CBA.cfm.
• If applying for the Non-Competitive CBA, preliminary budgets should be submitted for an amount between $2,500 and $5,000.
• If applying for the Competitive CBA, preliminary budgets should be submitted for an amount between $5,000 and $10,000.
• Please note, CBA Funding may not be used to purchase promotional items or food and beverages.
• If awarded, upon receipt of Notice of Award, awardees must submit a final budget along with two signed copies of the contract and Certification of Non-Debarment or Suspension.

Upload DirectionsThe preliminary budget template, along with guidance documents, a line item budget sample and detailed upload instructions can be found on the NACCHO CBA website under the section listed Supporting Documents at http://www.naccho.org/topics/emergency/MRC/CBA.cfm.
1. After completing your preliminary line item budget, save your Excel file on your computer where it will be easy for you to retrieve.
2. You must name your document where indicated on the application before you begin the upload. The system will not allow you to upload any files without entering the Document Name.
3. To enter the Document Name, click your mouse in the text field located underneath the field that reads “Document Name”. Name the file using the following format: 2012 CBA Preliminary Budget_MRC Unit Name_Unit Number
4. After entering your document name in the required format, click on the “Browse” button.5. After clicking the browse button, your computer should display your file library. Locate the
Excel file you wish to upload to the application. Click on the file and click “Open”.6. After clicking open, the File Name on your preliminary budget upload should correspond
with the file you have chosen to upload. 7. When all fields have been entered and you have verified the file name corresponds with the
file you wish to submit, click “Upload”. A green box will appear with the document name indicating your upload was successful.

• Once you submit the application, you will not be able to access the application to make any corrections at this point. Please be sure that your application is the final draft before submitting. Applications will not be re-opened for any reason!
• You should receive a confirmation email after you submit your application via the email address you entered on the last page. If you do not receive this email, first please check your spam filter/junk mail.
• If you do not receive the email within 24, hours please login to see if your application has successfully been submitted.
• If the application was not properly submitted, an “In Progress” status will be displayed next to the application.

Application Evaluation and Review
• The purpose of this award is to develop unit’s capacity and community resiliency. You should clearly describe how you are addressing the purpose in your responses. It is not about how much you write. Your responses should be clear and succinct.
• Applications for the Non-Competitive Award will not be scored per se; however, applicants must meet basic eligibility requirements in order to be considered for an award.
• In addition to basic eligibility requirements, applicants for the Competitive Award will be based on answers to questions in three broad areas:
I. Applicant Background (15 points possible)II. Project Information (70 points possible)III. Preliminary Budget (15 points possible)
• Award Decisions will be made in December 2012.

Reminders• Please start and complete your applications early. Waiting until the last days,
hours and minutes may prevent you from successfully completing the upload before the deadline.
• All inquiries regarding CBAs should be sent via e-mail to [email protected]. When e-mailing, please be sure to include the unit leader's name, phone number, e-mail address, state and MRC unit number. NACCHO will respond as quickly as possible. Please do not call! You will be instructed to submit an email so we can respond to all inquiries in a timely fashion.
• All applications and preliminary budgets must be submitted online. No paper applications will be accepted.
