    The meeting was called to order at 8:03am, Thursday, July 26, 2018.

    The following Board members were present: Quinn, Berrigan, Andersen, Boyer, Harrell and Machesky.

    Quinn presided.

    Quinn welcomed members of the audience, explained the procedure for addressing the Board, and stated the

    mission of the Board.

    Brown was present.

    Quinn moved, Andersen seconded, it was unanimously carried, Applicant failed to submit substantial evidence that

    the basis for the previous surrender has been removed and that the reissuance of Applicant’s license will not be a

    threat to public health or safety. Based upon the findings of fact and statute/rule violations identified in the

    investigative report, information presented at the Board meeting, and a review of the evidence submitted by Applicant,

    deny licensure.

    II.H.1. Sage Mark Dunham RN173675 (Curatola)

    The proceeding began at 8:07am with the following Board members present: Quinn, Berrigan, Andersen, Boyer,

    Harrell and Machesky.

    Dunham was present and addressed the Board.

    Based upon the information presented at this meeting, Board members discussed whether a showing of extraordinary

    circumstances or the possibility of irreparable harm has been made supporting the need for a Summary Suspension.

    Board members discussed the evaluation that was completed. Board staff advised it was the incorrect evaluation.

    Board members asked Respondent point of error and accountability.

    Quinn moved, Berrigan seconded, and it was unanimously carried, On a determination of reasonable cause based on

    the information in the investigative report and material presented at this Board meeting, at no cost to the Board, issue

    an Interim Order for a comprehensive psychological evaluation, to include an ethics evaluation to be completed by a

    Board approved licensed psychologist, and to include any additional testing deemed necessary by the evaluator, to be

    completed with 45 days and then return to the Board. If the Interim Order is not completed, based upon the information

    in the investigative report, issue Notice of Charges and include the factual allegations and alleged violations for failure

    to comply with the Interim Order.

    and welfare imperatively requires emergency action and issue the attached Order summarily suspending the license

    of Tracey Lynn Knotts, RN132659 pending further hearing. The hearing shall be promptly instituted and determined.
