The award will be made on the basis of: 1. SCHOLARSHIP2. CHARACTER

Eligibil:f'.ty:

1. Applicant must be enrolled in an accredited high school in Wisconsin and mustbe a resident of the State of Wisconsin.

2. They must have completed the first half of the twelfth grade

3. Their transcript of grades must show the results of at least one of theCollege Entrance Examination test scores. (ACT) = (SAT) ~ (PSAT).

Receipient to matriculate in September, 2011 at an approved 'Cbllegeor Universityin the State of Wisconsin and be 'enrolled in a,degree-'programin~:Medical':::Technol~gy/,Clinical Laboratory Science in a school accredited by the North Central Associationof Colleges or by the National Accrediting Agency for Clinical Laboratory Science(NAACLS)~

Application must be submitted or post-marked no later than Midnight

March 1, 2011Announcement of the Award Winner will be made on or before May 31, 2011.The Scholarship will paid to the Bursar, on behalf of the Receipient (upon noticeof matriculation from the College or University officials, Registrar) at the rateof $500.00 per semester, for a total of four consecutive years, renewable as long asthe Receipient remains in the degree program and maintains satisfactory grades.

In their request for an application, students must breifly specify why they wishto pursue a career in Medical Technology/Ciiri.ical'Laboratory,,'Science -and(\aLwhich'College or University they plan to attend.

Interested students must send an e-mail or letterto the WISCLS Scholarship Fund, Inc.secretary (name and address below) requestingan application packet. Please include a briefexplanation of your reasons for pursuing acareer in MT/CLS and list the college oruniversity you plan to enter.
