Purpose Questionnaire designed to measure wrist pain and disability in ADLs Also helps determine change over time, set treatment goals Developed to measure should pain and disability in an outpatient setting Designed to measure self-rated disability of patients with neck pain A modification of the Oswestry-Low Back Pain Index A questionnaire that evaluates symptoms and physical function of the upper limb and monitor change or function over time Developed by the AAOS

Composition Two Subscales to rate level of wrist pain with 5 items, and functional (disability) subscale with 10 items (6 specific and 4 usual activities) Contains 13 items on two domains: a 5-item subscale that measures pain, and an 8-item subscale for disability Most items could be regarded as specific ADLs 10-item questionnaire 30-item questionnaire with 1 (no difficulty of strongly disagree) to 5 (unable to do or strongly agree) also two-optional four-item modules to either assess workers whose daily living requires high level of physical activity or high performing athletes/performing arts

Scoring Each question is scored 0-10 (unable to do) Can be given either an individual sub score or total score out of 100 To get score out of 100 divide disability score by 2 so that pain and disability scores are equally weighted When item is missing you can replace the item with the mean score of the subscale Each question is scored 0-10 Can get individual sub scores or get total out of 130 and transform to out of 100% If items missing just subtract 10 per question from either sub-score total or grand total out of 130 (e.g. if 1 missing out of 120)

each item is scored out of 5 for a max score of 50 (or could be a percentage out of 100) when 1 item is missing could just subtract 5 from 50, when 2 are missing take the mean, and if 3 are missing the score is invalid Interpretation: 0-4 none, 5-14 mild, 15-24 moderate, 25-34 severe, >34 complete Sum of ((n/n)-1)X25 n being the number of completed response If more than 3 items missing than do not score For the 4-item question sum values/4-1X25

Psychometric Properties Excellent test-retest reliability over the short and long term Highly sensitive to determine if improvement has occurred compared to SF-36 MDC (true change not just chance [SEM]): 12 MCID (noticeable change by the patient): 24 Very high internal consistency More responsive than the DASH in shoulder specific pathologies Good construct validity (correlates well with other shoulder specific questionnaires) MDC: 18 Validity and reliability are good Correlated well to McGill Pain Questionnaire MDC: 5 for uncomplicated neck pain and 10 for cervical radiculopathy MCID inconsistent: anywhere from 5-19MDC: 10MCID: 15
