• About Follow-up Reviews

• Follow-up reviews are a critical component of the

safety event resolution process.

• Timely and thorough follow-up reviews can assist

with:

• Identifying the underlying causes of safety events

• Analyzing gaps in processes and policies

• Suggesting improvements

• Effectively resolving an event

• Reviews need to be assigned for any event that

requires further investigation.

• User Privileges

provided appropriate privileges before they can

assign or conduct a follow-up review.

• Custom User

• Department Leader

• Event Manager

• Types of Follow-up Reviews

Two types of follow-up reviews in Safety Event Manager:

• General Investigation

• Used to gather information such as actions taken,

contributory factors and recommendations to prevent

future events

• May be multiple such reviews per event and can be

assigned to multiple users.

• Root Cause Analysis (RCA)

• Usually assigned when the outcome is a sentinel event

such as death.

• Used to identify the root cause of the problem, correct

or eliminate the cause, or prevent the problem from

reoccurring.

• Only one per event and only one user can be assigned

this review.

• Extending and Deleting Reviews

• Follow-up reviews can either be extended within 2 days or less of the due date or after the assignment is past due.

• The follow-up review deletion option allows you to delete an assignment that was made in error.

• In-progress or completed reviews cannot be deleted.

1. Access the Open Events folder in your inbox and locate the event for which you wish to assign a follow-up investigation.2. Select the event row to access the event summary below.3. Click Assign Follow-up in the Actions section of the inline summary.

The Follow-ups page is displayed in a new window. The Follow-up Status section (1) at the top of the Follow-ups page displays the follow-ups, if any, that are currently assigned for the event. To assign a new follow-up, use the options provided on the lower half of the page (2).

To assign a follow-up:1. Select the action you wish to perform: Assign new

follow-up review or Assign/Share completed and past due review.

2. Select the applicable follow-up review form.3. Select the review completion date. 4. Optionally, assign the review to yourself for

completion.5. In the Selection of Reviewer section, you can select

the person(s) to whom you wish to assign the review. From the List of Available Staff, select one or more team members. Move your selection over using the > button.

6. Use the external reviewer function if you are waiting for information from an outside source to complete a follow-up task.

7. Enter any notes and comments you may wish to include.

8. When you’re ready, click Assign to complete the assignment. A notification will be sent to the assignee(s) letting them know a follow-up task has been assigned to them.

The Follow-up Status section displays the new assignment. Along with the name of the review and the ability to see a preview of the follow-up form, the table also provides the following options:1. View the assigned due date for the task.2. If the review is within two days of the due date and the reviewer needs more time, you will be able to extend the due date

using the option provided here.3. View the status of the review (Pending, Incomplete or Complete) 4. If needed, you may delete a review. Note that in-progress or completed reviews cannot be deleted.

The Status column displays a visual indicator to let you know that follow-ups have been assigned for a particular event. The column uses color as well as text to let you know if an assignment is completed, past due or within 2 days of its due date. Click the status to view the Follow-up Tasks page, which lists the status for each of the reviews assigned for the event.
