ASSOCIATION FOR SYSTEMS MANA GEMENT AWARD  The Association for Systems Management Award is available to full time students in the Computer Programmer Analyst, Computer Systems Technician, and Computer Systems Technology within the School of Information Technology.  One award will be presented annually.  By August 31, 2011, the recipient of this award must have completed Levels 1 and 2 of their program of study with a minimum 3.0 GPA and no failures or withdrawals.  The recipient must return to Level 3 of his or her program, or be on co-op placement in September 2011.  Fi nal selection of a recipient will be based on academic achievement and involvement in their program of study and/or community.   Application forms must be returned to the Awards and Scholarships office, London Campus, Room K1003, by Friday July 29, 2011 .  The recipient will be recognized at the annual Awards Ceremony held in November.  Name: Student Number: Address: S.I.N.: City: Postal Code: Phone: Date of Birth: Program of Study: FOL Email Address:     PLEASE PROVIDE RESPONSES TO THE FOLLOWING ON A SEPARATE TYPEWRITTEN SHEET:  1.
