A. LOCAL EXCHANGE SERVICE (GENERAL)

1. DESCRIPTION

Local Exchange Service provides access to the exchange system of the Company andconsists of a termination of the subscriber’s premises connected to a line to the centraloffice serving the area in which the subscriber is located. Connection at the central officeto switching equipment permits communication with other telephone users.

Tariffs located in Part 4, Section 2 of this Guidebook set forth the availability of, and ratesfor, the types and grades of local exchange service.

2. REGULATIONS

The Reno Exchange consists of the following wire centers: RENONV02, RENONV12,RENONV13, RENONV14, RENONV15, SNVYNV11, SPRKNV11, SPRKNV12,STEDNV11, LCWDNV11, WASONV11 and VERDNV11.

The Company’s Parts 1, 2, 3, 4 and 8 are incorporated by reference as applicable tothe provisioning of the services herein.

Federal Communications Commission (FCC) regulations, rates, and charges areapplicable as specified in Tariff FCC No. 1 to the services herein.

B. PROMOTIONS AND LIMITED OFFERINGS

Term and Volume Discount Plan (TVD)

a. Description

An optional discount pricing plan providing for term pricing on 1 or more measured or flat-ratebusiness access lines, with additional discounts available for volume commitments. TVDprovides for discount on 12, 24 and 36-month term commitments and volume commitmentsstarting with a minimum of 1 business access line.

b. Regulations

1. Discounts are applied to currently applicable rates.

2. Customer must commit to a minimum volume of business access lines. The total numberof business access lines within the Company’s territory will apply toward the volumecommitment. At any time during the term, if the total number of business access lines inthe Company’s territory is less than the minimum, an adjustment charge will be billed tothe customer.

3. Customer must sign a written service agreement establishing their commitment to the termperiod, volume, discount percentage to be applied to the monthly rate.

4. When a TVD customer moves service from one service location to another, the durationand discount associated with the service agreement is not affected. If the applicable localexchange access monthly rate changes as a result of the move, the monthly rate at thenew service location will be based on the tariff rate in effect at the time of the move for thenew location.

5. Customers requesting the termination of their service agreement prior to the completion ofthe term excluding moves, upgrades and renewals, will incur an early termination charge.

6. Early termination charges will not be billed if the customer upgrades to another Companyservice under a service agreement which term period is equal to or greater than the termperiod remaining on the service agreement being terminated.

7. Early termination charges do not apply if the customer agrees to continue the serviceagreement at the new service location and retain an equal or greater number of businessaccess lines than were connected at the old service location.

8. Early termination charges will not apply if the customer terminates an existing TVD serviceagreement prior to the end of the term if customer has signed a new TVD serviceagreement with a term that is equal to or greater than the term period remaining on theterminated TVD service agreement, and the volume of business access lines is equal to orgreater than the volume of business access lines on the terminated TVD serviceagreement.

9. The TVD cannot be combined with other monthly recurring discount plans or promotions onbusiness access lines.

10. The TVD cannot be combined with any plans or promotions that waive non-recurringcharges on business access lines.

11. Upon completion of the term commitment, if a new service agreement has not been signed,the service will automatically be billed at the current monthly rates for the business accesslines set forth in this tariff.

12. At the time a customer subscribes to TVD, applicable nonrecurring charges (NRCs) will bewaived for existing customers migrating existing lines to the TVD plan, existing customersthat add lines and for customers coming to the Company from other carriers. StandardNRCs will apply to lines added subsequent to the initial order.

2. Adjustment Charge

The adjustment charge is charged per line, per month for the number of business access lines underthe minimum line volume commitment for the remainder of the term, or until the minimum linevolume commitment is once again met.

$10.00 per line

3. Early Termination Charge

The early termination charge is equal to 50% of the monthly payments remaining for the term periodfor each business access line. Current tariffed rates in effect will be used to determine monthlypayments remaining for the term period.

4. Renewable Option

The 12-month term has a 12-month renewable option. If the customer selects the 12-monthrenewable option, the plan will renew for 12-month intervals. A maximum of two 12-monthrenewals are available after the first 12-month term. The customer will receive written notificationof their selection outlining the details of the agreements, plus subsequent notifications regardingtheir renewal options prior to the expiration of each 12-month term.

1. Exchange measured service consists of measured business service, measured residenceservice and measured trunks.

2. Measured Service usage refers to charges for local usage on the basis of (1) the number ofcompleted calls, (2) duration or length of conversation in minutes, and (3) time of day andday of week.

3. A base rate area is a more closely built up section of the exchange area in which the baserates apply.

4. A special rate area is an outlying section of an exchange within which special rate treatmentmay be applicable.

B. REGULATIONS

1. These rates apply within the base rate area or special rate area of a listed exchange. Therate for individual access line business and residence service does not extend beyond thepoint of demarcation.

2. Exchange measured business service, standard measured residence service and low usemeasured residence service are offered statewide in all exchanges as facilities of the servingcentral office and operation conditions permit.

3. Low Use Measured Residence Service.

a. This service is restricted to no more than one such service for each premises.

b. This service will not be furnished on a foreign exchange basis.

c. Other residence service will not be furnished concurrently on the same premises.

d. The residence premises at which the service is requested is the applicant’s principal place ofresidence.

4. Measured business line service will be provided in accordance with the following:

a. At rates and charges equivalent to other business service furnished to the same customer onthe same premises.

b. The service is furnished:

- only for operator assisted outgoing toll calls placed from stations connected to the line(USOC TXX), or

- to allow both incoming and outgoing local and toll calls, with time and charge informationreported on all operator assisted set-paid call (USOC TWX).

5. Operator Assisted Service

Dial Station Message Toll Charges for the lowest step (0-10 miles) plus the appropriateOperator Service Charge, as set forth in Part 9, Section 1, apply to local calls placed with theassistance of a Company operator.

6. A combination of flat rate and measured rate business or residence access line service willnot be furnished concurrently on the same premises when service is furnished from the sameexchange.

A. DESCRIPTION

1. Teenline is a package offering consisting of Individual Residence Flat Rate Serviceincluding Touch-Tone, Three-Way Calling and Call Restriction. A directory listing isincluded.

1. The customer must have an established primary Residence Flat Rate Service on thesame premises to participate in this offering.

2. TeenLine will be offered where facilities and operating conditions permit.

A. DESCRIPTION

Farmer line service is exchange service furnished in a suburban area of an exchange by useof lines provi-ded, owned and maintained in part by the customer. Service offerings consistof business (USOC LSE) and residence (USOC LSF).

B. REGULATIONS

1. Service is furnished outside the base rate area or special rate area and normally within theexchange area. A farmer line station shall not be located within the base rate area orspecial rate area. If subsequent to a base rate area or special rate expansion, an existingfarmer line service is located within the expanded area, this service may be retained by thesubscriber or successors at the same location until discontinues at the request of thesubscriber.

2. Mileage rates are based on air-line distance measured between the customer’s premisesand the nearest point on the boundary of the base rate area or special rate area.

A. DESCRIPTION

Private Branch Exchange (PBX) trunk lines provide a telecommunications channel betweenthe customer's premises and the Company’s central office (CO). The channel connects PBXstations to the telecommunications network.

B. REGULATIONS

1. PBX service will be provided outside the base rate area or special rate area and within theexchange area at the rates listed in this tariff.

2. Service under this Guidebook will be provided outside the exchange area according to ratesand regulations listed in Part 15, Section 2 of this Guidebook, or Part 4, Section 5 of thisGuidebook for Mileage Charges.

3. The toll rates applicable to toll service in connection with foreign exchange service will beaccording to the toll tariff provisions of the foreign exchange.

4. Lines and/or stations are installed on the premises on which the PBX switchboard orsubsidiary PBX is located. Rates as specified in Part 4, Section 5 apply to off-premisesmileage.

4.2.3.2 TERM AND VOLUME ARRANGEMENTS

4.2.3.2.1 TERM AND VOLUME DISCOUNT PLAN (TVD)

A. Description

An optional discount pricing plan providing for term pricing on 1 or more measured or flat-rateprivate branch exchange PBX trunk lines, with additional discounts available for volumecommitments. TVD provides for discount on 12, 24 and 36-month term commitments andvolume commitments starting with a minimum of 1 PBX trunk line.

B. Regulations

1. Discounts are applied to currently applicable tariff rates.

2. Customer must commit to a minimum volume of PBX trunk lines. The total number of PBXtrunk lines within the Company’s territory will apply toward the volume commitment. At anytime during the term, if the total number of PBX trunk lines in the Company’s territory is lessthan the minimum, an adjustment charge will be billed to the customer.

3. Customer must sign a written service agreement establishing their commitment to the termperiod, volume, discount percentage to be applied to the monthly rate.

4. When a TVD customer moves service from one service location to another, the duration anddiscount associated with the service agreement is not affected. If the applicable localexchange access monthly rate changes as a result of the move, the monthly rate at the newservice location will be based on the tariff rate in effect at the time of the move for the newlocation.

5. Customers requesting the termination of their service agreement prior to the completion ofthe term excluding moves, upgrades and renewals, will incur an early termination charge.

6. Early termination charges will not be billed if the customer upgrades to another one of theCompany’s services under a service agreement which term period is equal to or greater thanthe term period remaining on the service agreement being terminated.

7. Early termination charges do not apply if the customer agrees to continue the serviceagreement at the new service location and retain an equal or greater number of PBX trunklines than were connected at the old service location.

8. Early termination charges will not apply if the customer terminates an existing TVD serviceagreement prior to the end of the term if the customer has signed a new TVD serviceagreement with a term that is equal to or greater than the term period remaining on theterminated TVD service agreement, and the volume of PBX trunk lines is equal to or greaterthan the volume of PBX trunk lines on the terminated TVD service agreement.

9. The TVD cannot be combined with other monthly recurring discount plans or promotions onPBX trunk lines.

10. The TVD cannot be combined with any plans or promotions that waive non-recurring chargeson PBX trunk lines.

11. Upon completion of the term commitment, if a new service agreement has not been signed,the service will automatically be billed at the current monthly rates for the PBX trunk lines setforth in this tariff.

12. All other tariffed rules and regulations relating to PBX service apply.

13. At the time a customer subscribed to TVD, applicable non-recurring charges (NRCs) will bewaived for existing customers migrating existing lines to the TVD plan, existing customersthat add lines, and for customer coming to the Company from other carriers. Standard NRCswill apply to lines added subsequent to the initial order.

2. Adjustment charge

The adjustment charge is charged per line, per month for the number of PBX trunk linesunder the minimum line volume commitment for the remainder of the term, or until theminimum line volume commitment is once again met.

3. Early Termination Charge

The early termination charge is equal to 50% of the monthly payments remaining for theterm period for each PBX trunk line. Current rates in effect will be used to determinemonthly payments remaining for term period.

4. Renewable Option

The 12-month term has a 12-month renewable option. If the customer selects the 12-monthrenewable option, the plan will renew for 12-month intervals. A maximum of two 12-monthrenewals are available after the first 12-month term. The customer will receive writtennotification of their selection outlining the details of the agreements, plus subsequentnotifications regarding their renewal options prior to the expiration of each 12-month term.

A. DESCRIPTION

Applies to extension service to business and residence individual service.

Extension station service provides the capability of originating or receiving calls from locationsequipped with instruments in addition to the location of the main station.

B. TERRITORY

Within the base rate area or special rate area of a listed exchange.

C. REGULATIONS

Extension station line services located off-premises will be provided at the rates shown in Part 3,Section 1 and mileage rates as shown in Part 4, Section 5.

4.2.4.2 TOUCH-TONE® CALLING SERVICE

A. DESCRIPTION

1. The service permits the use of push-button telephone sets in lieu of rotary dial telephone sets tooriginate calls.

2. The service will be offered at no charge as part of the basic exchange rate, for all classes ofservice, as facilities become available throughout the territory served and as operating conditionspermit.

B. REGULATIONS

1. The service requires special central office equipment and is provided only from central office withthese facilities.

2. One or more stations associated with a line arranged for Touch-Tone Calling Service may remainequipped with rotary dials.
