Stay healthy and active year-round.Many Medicare Advantage plans make it easy.

Walking a dog and other outdoor activities are easy to enjoy on a warm summer day. But during Midwest winters many older adults get out of their good exercise habits. Time indoors can also mean poor eating habits.Barbara Meier is a great example. For years, Barbara’s passions included her beloved Chicago Cubs. It also included her high-energy dog Wrigley, and caring for her parents.Six years ago, Barbara’s father passed away. Her mother passed away last year. “Caring for my parents had been my life. I was so used to spending most of my days providing care. When my mom died, I was kind of lost for a while,” said Barbara. “I realized I had not paid attention to my own fitness. A woman from my apartment building told me SilverSneakers was an included benefit of her health plan at no cost to her. I looked into it and found that Weight Watchers was also included. About three weeks after my mom passed away, I enrolled in the iCare Medicare Plan. I’m so glad I did.”

SilverSneakers is a fitness benefit. It includes access to 17,000 fitness locations nationwide. Members can use exercise equipment. They can also attend group exercise classes led by certified instructors. Eligible plan members simply show their personal SilverSneakers ID card at the front desk of any participating location.At-home kits are offered for members who want to start working out at home. They are also a good option for people who can’t get to a fitness location due to injury,

illness or being homebound.The iCare Medicare Plan also includes a Weight Watchers benefit. Members can request meeting voucher packs for local Weight Watchers. These packs will contain thirteen-weeks’ worth of meeting vouchers at a local Weight Watchers location. Transportation to and from Weight Watchers meetings are available at no charge. 100 trips up to 35 miles to/from Weight Watchers meetings are covered. Despite her history of back problems Barbara has had great success. She’s lost 14 pounds and her trainer helps her choose exercises that are safe for her back. “Working out and losing weight has helped me cope with losing my parents. It helps with my back and I feel better about myself. I feel so much younger now.”But she’s not done yet. Once the weather improves, she plans to continue going to Weight Watchers meetings. She also plans on gardening and taking more walks with Wrigley. And continuing with her workout routine of weight machines, treadmill and other cardio exercises.“In August, my nephew is getting married, and I really want to knock their socks off.” Look out wedding guests! Barbara is coming!

Working together for better health care.

Your iCare Community Health Worker/Health Coach and you.There are many factors that influence our health. These are called determinants of health. One kind of determinant of health is what is in your genes and your biology. Another determinant is your individual behavior. This could include whether you smoke, exercise, or eat a balanced diet.Many people assume that their health is a result of their genes, their behaviors, and how often they get sick or go to the doctor. But it’s not just how you live that determines how healthy you are. Social and physical environments have a big impact on your health, too. These are called social determinants of health.All these factors play into your health daily. It is important to understand how these things affect your health, so you can take steps to improve them, and improve your health.iCare has a team of Community Health Workers (CHW)/Health Coaches (HC) that can help you and your family with your social determinants of health.

How does the program work? When you sign up for the program, a CHW/HC is selected for you by your zip code. The CHW/HC knows what services are available for you where

you live. They can connect you and your family with resources such as: » Housing » Doctors or medical care » Education » Alcohol or drug abuse treatment » Food and clothing banks » Social service agencies

The CHW/HC will also provide health education resources and information. He/she will provide support and guidance to you through face-to-face home or facility visits, or by phone. Who can enroll? Members in these plans can enroll in the iCare Community Health Coach program: » iCare Medicare Plan (HMO SNP) » Aurora CompleteCare (HMO SNP) » iCare Medicaid SSI Plan

How do I enroll?Talk to your Care Coordinator. He/she will connect you to a CHW/HC. If you do not know your Care Coordinators name or phone number, call iCare Customer Service at 1-800-777-4376 (TTY: 711) and ask to be directed to your Care Coordinator.

Is there a cost? There is no cost for you to use Health Coaching Services. It is simply part of iCare’s mission of service to our members!

WERE YOU RECENTLY A PATIENT IN THE HOSPITAL? If you were recently in the hospital, it’s important that you schedule a post-hospitalization appointment with your doctor. This appointment should be within 30 days of discharge (leaving the hospital), but try to see your doctor earlier. If you can see your doctor within the first 7-14 days after you return home, that’s even better. Be sure to bring your discharge medicine list to your appointment. It is important for your doctor to review your new medicines and make any necessary changes. This is called a medication reconciliation. It is an important step in your care! Our care management teams can answer any questions you have about post-hospital care. Ask us today by calling 1-800-777-4376 (TTY: 711).

WHY IS SLEEP IMPORTANT?Everything can seem harder when you are tired. Here are some facts about sleep and emotions. » If you aren’t getting enough sleep, you may be irritable, angry, sad or stressed.

» Lack of sleep can hurt your social relationships.

» You may have a negative reaction to what’s happening in

your life. You may not be able to cope with challenges.

» Sleep and mood affect each other. Sleep problems may be a sign of depression. Depression can make sleep problems worse.

When you get a good night’s sleep, you: » Have less stress. Without the rest you need, your body is on high alert. This can increase your blood pressure. You may also produce more stress hormones. This can make it harder to fall asleep and recharge.

» Are more alert and active. You have higher levels of energy. It is easier to do complex physical and mental tasks.

» Can avoid depression. Sleep helps control a hormone that affects mood. Lack of sleep may lead to depression. It can cause anxiety and other behavioral health problems too.

» Can control your weight better. Lack of sleep affects hormones that control appetite. This can lead to being overweight.

» Have a healthier heart. Blood pressure and cholesterol levels are higher when you don’t get enough sleep. These are risk factors for heart disease and stroke.

» Repair and restore your body. Sleep helps repair cells damaged by stress, fatigue and muscle strain.

» Have a better memory. Deep sleep improves memory. It helps us connect feelings and experiences.

Tips for getting better sleep If you’re having trouble with sleep, try these tips. » Keep a regular sleep schedule. This includes weekends.

» Avoid caffeine, alcohol and nicotine four to six hours before bedtime.

» Don’t eat large meals within two hours of bedtime.

» Don’t exercise within two hours of bedtime. Do exercise earlier in the day. This can help you sleep more soundly.

» Don’t use your bedroom to watch TV, pay bills, and check social media or text.

» Avoid napping during the day. » Do the same things each night to tell your body it’s time to wind down. Try a warm bath or reading.

» Avoid looking at the clock. » If you can’t fall asleep within 20 minutes, do a quiet activity somewhere else. Talk to your doctor if you are having problems sleeping.

FOLLOW TO HOME FOR HOMELESSNESSiCare wants to make sure our members that have no home to go to after a stay in the hospital get follow-up health care. So, we created the Follow to Home for Homelessness Program.While an iCare member is still in the hospital, a hospital social worker can contact the iCare Nurse Discharge Team. The Nurse Discharge Team will try to visit the member at the hospital. The Team and the member will create a discharge health care plan. This plan will include finding a safe and clean location to allow for skilled nursing care. Skilled nursing care is a high level of medical care that must be provided by trained people. The Team will also help members obtain housing. They can also help the member access community resources.

CHOOSING A PRIMARY CARE PROVIDER AND REQUESTING A PROVIDER DIRECTORYIt is important to choose a Primary Care Provider (PCP) to manage all your health care. And when you need care it is important to call your PCP first. Your PCP will help you decide if you need to see another doctor or specialist. Your PCP will also give you a referral, if you need one. You must get approval from your PCP before you see another doctor. If you need help selecting a PCP, please call Customer Service or your Care Coordinator.You can choose a PCP from the list of doctors. The list of doctors is called the iCare Medicaid SSI/BadgerCare Plus Provider Directory. Doctors who are accepting new patients will have “accepting new patients” written under their name.The Provider Directory can also help you choose a specialist, hospital, urgent care center, clinic, imaging center, and more. All iCare providers are sensitive to the needs of many cultures. FORWARDHEALTH ID CARDYour ForwardHealth card is how your doctor or pharmacist will know you are an iCare Medicaid SSI or BadgerCare Plus member. You may have problems getting care or medicine if you do not have

your ID card. It is important to always carry your ForwardHealth ID card with you. Show it every time you get care. Also, be sure to tell your doctor or pharmacist that you are a member of iCare.

If you have other insurance in addition to iCare, you must tell your doctor or other health care provider. Your doctor or other health care provider must bill your other insurance before billing iCare. If your iCare doctor or other health care provider does not accept your other insurance, call the HMO Enrollment Specialist at 1-800-291-2002. The HMO Enrollment Specialist can tell you how to use both insurance plans.

DON’T LOSE YOUR HEALTH CARE COVERAGE!It is important to renew your health insurance on time. You need to renew it when you get your renewal letter. If you do not renew your coverage, your health care insurance will end.There are three ways to renew your health insurance:1. Apply on-line at the ACCESS

website: www.access.wisconsin.gov;

2. Fill out the form found on the ACCESS website. Mail it to DHS. The instructions are on the form. Go to: www.dhs.wisconsin.gov/forms/F1/F10182.pdf; or

If you become pregnant, please call the IM agency to report your pregnancy.iCare is not able to change BadgerCare Plus member’s address and phone number in our own records. To find an IM in your area, visit www.dhs.wisconsin.gov/forwardhealth/imagency/index.htm or call the HMO Enrollment Specialist at 1-800-291-2002. The Enrollment Specialist will help you choose a new HMO that serves your new area and/or update your address or phone number.
