Where children under 18 years of age intend to travel to Australia, their parents or legal guardians (guardian meaning anyone else who can lawfully determine where the child/children is/are is to live) are required to sign a consent form to give permission for the child/children to be granted an Australian visa for the purpose of travel to Australia. If one parent has lawfully been made solely responsible for making decisions as to where the child/children is/are to live, then only that parent needs to sign the consent form. A certified copy of the relevant evidence (to confirm the family relationship or guardianship and the sole responsibility for the child/children) must be provided. Note: If this consent form is requested and not lodged, the application will normally be refused. How to complete this form This form must be completed in English. The completed and signed form (pages 3 and 4 only) and any additional documentation must be sent to the office nominated in your application. You must attach to this form: a certified copy of the personal details (or biodata) page of the child’s/children’s passport; a certified copy of the personal details page of the non-accompanying parent’s or legal guardian’s passport, driver’s licence, National Identity Card (ID card) or birth certificate; a certified copy of the child’s/children’s birth certificate(s); a certified copy of any relevant evidence making you solely responsible for making decisions about where the child/children is/are to live (if applicable). If you wish to attach this consent form and the required documentation to your electronic visa application, please use the electronic attachment facility available on the eVisa website. You can access this facility via the eVisa Online Services page at www.immi.gov.au/e_visa To attach documents electronically you will need to identify your application by entering: your Transaction Reference Number; your date of birth; and the password that you supplied when your application was lodged and saved. Tourist visa applicants Parts A, B and C of this consent form must be completed by the parent(s) or legal guardian(s) of the under 18 years of age Tourist visa applicant(s). For children (under 18 years of age) who intend to travel to Australia, and: whose stay in Australia will not be in the company of either or both of his or her parents or guardians, and will not be staying with a relative, and who is not a member of an organised tour with adequate maintenance and support arrangements for the stay in Australia, then their parent(s) or legal guardian(s) are asked to nominate a person (21 years or older) to give an undertaking to provide accommodation for and be responsible for the support and general welfare of the child during the child’s stay in Australia. The nominated person should be physically present in Australia for the period of the child/children’s intended stay. Depending on the child/children’s personal circumstances, the nominated person may be asked to provide an undertaking in writing. Student visa applicants Parts A and C of this consent form must be completed by the parent(s) or person(s) who has legal custody of a student visa applicant who is under 18 years of age. If the applicant is: applying electronically, and if the student is not submitting a signed statement from their education provider confirming that appropriate arrangements have been made for the student’s accommodation, support and general welfare, then they must also complete the Student Welfare Declaration Form. Who can witness this consent form or certify copies of original documents? This consent form requires the signatures of the parent(s) or legal guardian(s) of the child/children to be witnessed. Outside Australia – the witness or person certifying documents must be a person who would normally be authorised to witness the signing of legal documents under the law of the country that the applicant is in. then their parent(s) or legal guardian(s) are asked to nominatea person (21 years or older) to give an undertaking to provideaccommodation for and be responsible for the support andgeneral welfare of the child during the child’s stay in Australia.

The nominated person should be physically present in Australiafor the period of the child/children’s intended stay. Dependingon the child/children’s personal circumstances, the nominatedperson may be asked to provide an undertaking in writing.

Student visa applicants

Parts A and C of this consent form must be completed by theparent(s) or person(s) who has legal custody of a student visaapplicant who is under 18 years of age.

If the applicant is:

• applying electronically, and

• if the student is not submitting a signed statement from theireducation provider confirming that appropriate arrangementshave been made for the student’s accommodation, supportand general welfare, then they must also complete theStudent Welfare Declaration Form.
