This is non-compliance with 9 CFR 416.4(a) and 4' 6.4(d)_ "Product. must be protccted from adultenltion during pmcessing, handling, storage, loading, and unloading at and during transportation fiom ollicial establishmcnts."

On 03-24-2009 at 1 I 15, on large killlloor the plant was working with excessive fecal material and viscem on the tloor. FSIS Regulation 9 CFR 416.1 states that each Official Establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated. At 0708 after being notified by the QC Manager Ali Ngallkion of the completion of their pre operational inspection, I observed in the sheep/goat kill room at the inside h.1nging area several pieces offat/meat on the dmin cover screen thaI lie underneath its main cover. and several pieces of fat in the drain as well. Sitting on top of the beam above this area where the hooks lire stored, there was a knife soiled with fal and blood and a sharpening block. The beam dircctly across was soiled with hair and dirt. On 0212712008 at approximately 0720 hOllIS after being notified by the QC Manager f the oompletion of their pre-operation inspection, I observed ill two areas ofthe bovine kill room (Zone 2), the wall at the restrainer cage had a .splat of fecal, dirt and grass on the upper wall; about three feet away from the restrainer cage was also a splat of fecal, dirt and grass on the wall behind the urea where an employee had hung his scabbard witll knives inside. In Unit 2, there was a piece offat on the ring of the large split saw and fat residue on the outside of the saw drive. Tag numl)er 838863612 was applied to the kill room. The QC Manager was notified. Regulation 416.IJ(c) was not met in this area today. At 0705 hours the following was observed in the finished raw beef holding cooler, after management told inspection personal that the pre-operational inspection was completed and every thing was found acceptable .. Three knives that are used to break down carcasses from primal to sub-primal cuts for shipment were found unclean with dried blood and smears of fat on the knives handles and on the knives blades which are direct product contact surface. Establishments SSOP program last signed and dated on 7/1/2008, on page 2, under Implementing, Monitoring, Record Keeping, it states:

This is a non-compliance under 9 CFR 416.13( c) which states:" Each official establishment shall monitor daily the implementation of the procedures in the Sanitation SOP's." Two boning knives and a large splitting knives were all rejected from use with US Rejected tag # 842273903.

At 09:15 hours while I was walking through the plant to monitor the sanitary operation procedures, I observed inside the inspected product holding cooler the following: -Two inedible barrels ful1 with inspected and passed beef heads exposing the product to direct contamination. The barrels were tagged together with the US REJECTED tag # B 38 863706 -Big plastic container with harvested edible testicles contaminated with big sponge they used to dry the rail from any condensation. the product was tagged with the US REJECTED tag # B 38 863701. Mr. mahmoud the plant QC observed the non compliance's and was notified that the Regulatory Requirements of 416.4( d) were not met" product must be protected from adulteration during processing, handling, storage, loading and unloading at and during transportation from official establishments".

PRODUCT - Protocol

At approximately 0840 hours on July 1,2010 while observing truck unloading I observed a goat get out of the establishment control, the gate was leftpartially open. I put US Reject tag on gate # B38863747 I immediately notified plant foreman and QC Ali Nagankion about the noncompliance. Plant infonn me that they had told the truck driver to stop the flow of animals which he did not but increased the flow of animals. The new hire was not prepared for the sudden flow of animals there by allowing the goat to escape. Management immediately stopped production to recapture the animal which was done in a timely fashion. There was no injury to the animal as verified by Dr. SPHV there were no injuries or damage to the public according to plant management. Regulation 9 CFR 313.2(a) driving of livestock from the unloading ramps to the holding pens and from the holding pens to the stunning area shall be done with minimum of excitement and discomfort to the animals. Livestock shall not be force to move faster than a nonnal walking speed. The establishment took immediate corrective action shutting the gate and recapture of the goat. The US reject was removed at 0910. This document serves as a written notification that your failure to comply with regulatory requirement(s) 9 CFR 313.2(a)

Regulatory Action was not taken because protective covering was immediately provided to prevent animals from coming in contact with this object. It was also observed that lIIlimalS entering the slaughter chute had difficult footing. There was sawdust applied but this was insufficient to prevent a cow from baving positive footing. Although the cow did not fall it did exhibit behavior associated with a sliding problem. Although the floor throughout the establishment is concrete, the possibility is there, that this might allow slips BIld falls. This noncompliance WlI3 also brought to the immediate attention of both QC Manager md plant ow~er

This is a noncompliance with 9CFR 313.1(b) which nates that " Flocm of livestock pens, ramps, and driveways shall be constructed and maintaine<l so as to provid~ good footing for livestock. Slip resistant or waffled floor surfaces, cleated nunps and the use of sand, as appropriate, during winter months are examples of acceptable construction and maintellll.Dce". Regulatory action was not taken at this time because saw dust was immediately - spread on the area where the cow was seen slipping.
