Following is a listing of unclaimed funds held in accordance with the provisions of 28 U.S.C. Section 2042 for the Clerk of the U.S. Bankruptcy Court, Eastern District of Missouri, Thomas F. Eagleton U.S. Courthouse, 111 S. Tenth St., 4th Floor, St. Louis, MO 63102. Any claimant entitled to any such funds may on petition to the Court and upon notice to the U.S. Attorney and full proof of the right thereto, obtain an order directing payment to him. In order to obtain payment, the petitioning creditor or claimant must file with the Court the following forms:

1.) Order for Payment of Unclaimed Funds, 2.) Application for Payment of Unclaimed Funds, and 3.) Affidavit of Creditor
