Attorney General Bondi Announces that Mortgage

Servicers Report More Than $3.6 Billion in

Relief to Florida’s Borrowers TALLAHASSEE, Fla.Nov. 19, 2012 —Attorney General Pam Bondi announced today that the nation’s five largest mortgage servicers that entered the federal-state settlement over foreclosure abuses and unacceptable mortgage servicing practices report that they have provided more than $3.6 billion in borrower relief to Floridians as of Sept. 30, 2012, with an additional $1.3 billion in modification relief in the pipeline. The servicers reported that 48,998 Floridians have benefitted from an average of $73,663 in relief per borrower. Flu Season Is Here

Panama City, Fla – Flu season is here and the vaccine for seasonal flu is available. Everyone 6 months of age and older should get a yearly vaccine. Right now, there is very little seasonal flu activity so it is a great time to get your vaccine. It is recommended by the Centers for Disease Control and Prevention that people are vaccinated against influenza as soon as flu vaccine becomes available in their community. Influenza seasons are unpredictable and it takes about two weeks after vaccination for antibodies to develop in the body and provide protection against the flu.

The flu vaccine is available at the Bay County Health Department and many other locations around town. At the health department, appointments are recommended for flu shots; however, walk-ins will be accepted as time permits. Hours of operation for this service are Monday-Friday, 8:00am-11:00am and 1:00pm-3:00pm. Please contact the health department at (850) 872-4455 to schedule an appointment to receive this year’s flu vaccine. The cost for the regular flu vaccine is $29 and $42 for the hi-dose flu vaccine (for individuals over 65 years of age). Cash, Check, Visa, MasterCard and Medicare are accepted. The vaccine is free for children under 18 years of age.

Every flu season is different, and influenza infection can affect people differently. Even healthy people can get very sick from the flu and spread it to others. Flu season usually begins in October and can last through May. Seasonal flu is a contagious respiratory illness caused by influenza viruses. It can cause mild to severe illness, and at times can lead to death. Some people, such as older people, young children, and people with certain health conditions, are at high risk for serious flu complications. Getting a flu vaccine is the first and most important step in protecting you and your family against this serious disease. The Bay County Health Department and CDC also recommends that you take everyday preventive actions to stop the spread of germs:

Arrest Log The Gulf County Sheriff’s Office will be conducting vehicle safety checkpoints and DUI check points during the month of November 2012. The check points will be held throughout the county to include Highway 98 near St. Joe Beach, Highway 98 and Garrison Ave, C-30 Simmons Bayou, Highway 71 North of White City, Highway 22 and Highway 22A, Highway 71 and Westarm Creek, Highway 71 Dalkeith Area and Highway 71 near the Calhoun line.

PCPD 12-14879 12-14879 11/13/2012 1:36:42PM 1707 W 23RD ST; On 11-13-2012 at approximately 01:36 pm, I observed a green vehicle traveling west on W 23rd St. with the driver not wearing a seat belt. The driver was wearing a white tank top and the vehicle had tan interior. I conducted a traffic stop on the 1995 Lexus, with Florida tag - AMWZ44, at 1707 W 23rd St., where I made contact with Darius White, the driver and sole occupant of the vehicle. White identified himself by name, address and date of birth. While speaking with White, I detected a strong odor of marijuana coming from the vehicle. White appeared nervous and he was continuously moving and looking around. I detained White for officer safety and explained that I smelled marijuana coming from the vehicle. White denied having anything illegal on his person. White had a large sum of money waded up in his left front pants pocket. I located a large white trash bag under the passenger seat that contained nine (9) clear bags with marijuana. Due to the amount of marijuana in the bag I contacted Sgt. Morris, who arrived on scene and contacted investigations. I also located a smaller clear plastic wrapper that contained marijuana under the passenger seat. I transported

White to the Panama City Police Department for investigations. I collected the evidence and turned it over to investigations. I transported White to the Bay County Jail, where he was charged with possession of marijuana with intent to distribute and possession of drug paraphernalia. I issued White a civil traffic citation for not wearing his seat belt. The vehicle was towed from the scene by Garrett's Towing, a rotation wrecker. PCPD Ofc. K. Doheny #610

NAVARRE The cold fronts have pretty much driven most

inshore species into their fall pattern as well as driven most pelagic species further off shore or even further south. This means sleeping in and letting it worm up some when fishing inshore and layering up for those long cold runs off shore.

Inshore white trout are stacking up at the 3-

mile Bridge, bull redfish are in the pass, and specks are laying low in the deeper channels in the bay as well as heading into area rivers. Redfish, speckled trout, and stripers are starting to show up in greater numbers in local rives. Lastly flounder, trout, redfish, and sheepshead can be found around the power lines in East Bay. Cut bait is the top bait for the white trout right now. The reds are hitting almost any live baitfish presented to them during the outgoing tide in area passes. If you are luck enough to happen upon a school along the beach almost any artificial bait presented to them will do the job. Working ½ to 1oz bucktail jigs, 4 to 6 inch crank baits, gulp shrimp and jerk baits and even flies are producing well when you can find the school along the beach. The flounder bite has begun to slow down somewhat in the bay, but I’m still getting reports from folks fishing in East Bay and in and around the Pensacola pass. Finger mullet, bull minnows and tiger minnows are the go to baits for flounder. Lastly sheepshead landings are on the rise with most being caught around area bridges.

Whiting, redfish, pompano, bluefish, bonita

and sharks are available for those fishing on the pier or in the surf. Bonita have been blasting through almost every day with the top baits being the 1.5oz Got-Cha jig fish jigs, YoZuri 3D crystal minnows and white bucktail jigs. The whiting redfish and pompano are all hitting on live or frozen shrmp. The redfish will also hit pinfish and cut mullet. I’m also getting reports of sheepshead and flounder around the pier pilings. Small live shrimp and fiddler crabs will land the sheepshead and live finger mullet and bull minnows for the flounder.

Offshore deep droping is producing the most action for bottom fishermen. Barrelfish, snowy grouper, long tail seabass, tilefish and yellowedge grouper are being landed 400 plus feet of water. Wahoo, black fin, and yellow fin are available from as close in as the edge to on out around the rigs. Closer in around the edge good numbers of amberjacks, red grouper, and scamps can be found. Live pinfish is the bait of choice although if you can catch a few dozen sand perch they will work just as well. Amberjacks are also available on many of the near shore wrecks, but most of the legal fish are in 100 plus feet of water. Jigging for them with Williamson, Bomber or Shimano Butterfly jig is extremely productive right now, but live baits such as hardtails, pinfish, and red porgies will also get the bite. Near shore wrecks are beginning to load up with flounder as the cold fronts push them into the gulf for the winter. Live shrimp, pinfish, finger mullet, bull minnows and tiger minnows are the top baits.

PORT ST. JOE/MEXICO BEACH The intracoastal canal system in Gulf County, extending from

the Tapper Bridge north and beyond is a perennial favorite for anglers. Shorter days and cooler water temperatures have signaled the speckled sea trout to begin their migration to the warmer water of the canal and other creeks, tributaries and bayous to winter. Reports from local guides tell of a steady increase in trout activity along with redfish, sheep head, gray snapper, and black drum. The flounder bite has decreased slightly as the fall run draws to a close. Still, anglers report several nice catches of flounder throughout St. Joe Bay. Along Gulf and Franklin County beaches, expect a very good whiting and croaker bite as well as some bull redfish late in afternoons. In the blue water, expect Amberjack over high relief wrecks and structure. This time of year, big live baits will get you a big very lively AJ. Find your baits around docks and pilings. Mullet pinfish croaker and grunts all make great baits for amberjack and remember big live baits equal big amberjack.

Chiluk-Ki stated he usually rides an electric bicycle everywhere

but this time he "screwed up"

PCPD 12-14876 12-14876 11/13/2012 10:18:11AM W 23RD ST & BECK AVE Traffic Stop On 11-13-2012 at approximately 10:18 am, I observed a red vehicle with the driver not wearing a seat belt. The driver was wearing a yellow Florida State University sweatshirt and the vehicle had red interior. I conducted a traffic stop on the 1991 Chevrolet, with Wisconsin tag - 911REH, near W 23rd St and Beck Ave. I made contact with Joseph Chiluk- Ki, the driver and sole occupant, who identified himself by name, address and date of birth. Chiluk-Ki stated he doesn't have his license on him because it is suspended. Chiluk-Ki further stated that he doesn't normally drive but he went to the liquor store for Bonnie Stith, the registered owner. A search of FCIC/NCIC and D.A.V.I.D revealed Chiluk- Ki has multiple suspensions (02-13-2012, 04-30-2012, and 06-11-2012) and it was revoked on 08-16-2012, which was confirmed through Teletype. I arrested Chiluk-Ki for driving while

license suspended/revoked (# A03R-WTE) and transported him to the Bay County Jail. I also issued Chiluk-Ki a civil traffic citation for not wearing his seat belt. Chiluk-Ki signed a tow waiver wishing to leave the vehicle on scene. While en route to the jail, Chiluk-Ki stated he was just trying to get some liquor for Stith because she asked him to go to the store for her because it's her birthday and she was on Facebook. Chiluk-Ki stated he has been living with Stith for since August and she knows that he doesn't have a valid drivers license. Chiluk-Ki stated he usually rides an electric bicycle everywhere but this time he "screwed up." Chiluk-Ki asked that I bring the keys to Stith because it's the only one she has and he doesn't know when he will get out of jail because he is on probation. PCPD Ofc. K. Doheny #610

I made contact with the complainant in reference to a damaged window. The complainant advised she noticed a window broken at approximately 0730hrs on this date. The window appeared to have been broken from the inside of the residence; a majority of the glass was laying on the ground outside. No other damage was noted and nothing found that may have broken the window. The complainant could not advised why the window may have broke. PCPD 12-14917 12-14917 11/14/2012 10:02:47AM W 23RD ST & FRANKFORD AVE Traffic Offense

On 11-14-2012 at approximately 10:02 am, I observed a tan vehicle traveling west on W 23rd St with the passenger not wearing a seat belt. The passenger was wearing a pink sweatshirt and the vehicle had tan interior. I conducted a traffic stop on the 2001 Honda, with Florida tag - BDUK06, and made contact with Aaron Pendola, the driver, and Summer Brooks, the passenger. Pendola stated he doesn't have a license but identified himself by name, address and date of birth. Brooks identified herself by her Florida drivers license. A search of FCIC/NCIC and D.A.V.I.D revealed Pendola has a suspended drivers license, which was confirmed through Teletype. Pendola stated his license was suspended because he failed to turn a tag in when he sold a truck. Pendola stated he usually doesn't drive but they just dropped off their daughter and were on the way to the methadone clinic. I issued Pendola a criminal traffic citation (# A03R-YIE)for driving while license suspended/cancelled and (A03R-YJE) for driving while license suspended for financial responsibility but released him on a notice to appear, which he signed and stated he understood. I issued Brooks a civil traffic citation for not wearing her seat belt. Pendola signed a tow waiver allowing Brooks to drive the vehicle. Ofc. K. Doheny #610

I made contact with the victim who stated back in August he lost his identification. The victim did not report it at that time as he was unsure of where he lost it. He did obtain a new identification card the next day from the DMV. On October 13, 2012 the victim learned that he had a warrant for

a failure to appear on a theft charge. The victim turned himself in and discovered that someone had used his identification on a theft charge at Walmart on the beach . That subject was released on a Notice to Appear and was not taken to jail or fingerprinted. This subject used his own social security number when he was arrested on October 5, 2012 by the Beach PD. This subject has been arrested numerous times on other shoplifting charges within our city. W 23RD ST & GRANT AVE Traffic Offense On 11-14-2012 at approximately 11:24 am, I observed a gold vehicle with the driver and passenger not wearing a seat belt. The driver was wearing a red shirt, the passenger was wearing a dark blue shirt and the vehicle had tan interior. I conducted a traffic stop on the 2000 Kia, with Florida tag - DV7640B, and made contact with Scott Williams, the driver, and Richard Ayers, the passenger. Williams identified himself by name, address and date of birth. Williams stated he didn't have his license on him because it was no good and he "hates" wearing seat belts. Ayers identified himself by name, address, and date of birth and he stated he forgot to put the seat belt on. A search of FCIC/NCIC and D.A.V.I.D. revealed Williams has a suspended and expired drivers license, which was confirmed through Teletype. I issued Williams a criminal traffic citation (# A03R-YTE) for driving while license suspended with knowledge and (# A03R-YUE) for driving while license expired more than six months but released him on a notice to appear, which he signed and stated he understood. I also issued Williams a civil traffic citation for not wearing his seat belt. I issued Ayers a civil traffic citation for not wearing his seat belt. PCPD 12-14923 12-14923 11/14/2012 11:19:25AM 1017 HARMON AVE BURGLARY

I made contact with the victim in reference to a burglary to his vehicle. He stated he parked his car in the driveway the night before and when he came out to go to work this morning he discovered his Garmin NUVU GPS and two seat covers were missing from the car. He stated he did not observe any signs of forced entry into the vehicle and does not know if the door were locked or not. The victim did not have the serial number for the GPS at the time of the report but stated he would attempt to locate it. I processed all four doors and side windows on the victims car for finger prints. There were no usable prints located only smudges on the passenger side door. At this time it is not known who entered the victims car and took the items. Officer C. Taylor #3305

2750 W 12TH ST; LAKE WARE APTS I responded to the above listed location in reference to illegal dumping. The caller advised on 11/13/12, an all white in color dump truck had dumped a load of concrete behind the address. Once on scene I observed large pieces of concrete laying on the side of the road, partially on the driveway access, to the apartment complex. There is no further descriptive information available for the person who left the concrete there. I also called real estate representatives of a vacant lot, next to where the concrete was dumped. The agent was not aware and would contact his associate, and property owner, to see if they knew of anything. The caller did not wish contact. Ofc. J. Turner #2110

Bicycle LARCENY On Wednesday 11-14-2012 around 9:09 p.m. I responded to a theft of a bicycle at 1819 Grant Ave. I arrived and spoke with the reporting person who advised me that his bicycle had been stolen within the past hour. I gathered all information from him to have it entered. He did not have the numbers on the bicycle but gave a description of the bicycle. Officer Justin Parrish #2010


Incident # 12-13201

On 11-8-12 at approximately 1654 hrs, I and Corporal Nusbaum responded to Walmart at 2101 SR 77 in reference to a retail theft. Upon arrival we met with loss prevention specialist, Alex Turner. He stated that on 11-8-12 at approximately 1600 hours inside the store he observed Ashley Nicole Rich conceal 16 mens clothing

items valued at $94.87 inside a baby stroller and pass all points of sale heading towards the front exit doors without paying for the items. Mr Turner then stopped Ms Rich and called Lynn Haven Police Department. I issued Ms Rich a Notice To Appear, with a court date of 11-28-12. She was given a Trespass warning. She signed it and stated she understood. Mr Turner gave a sworn written statement. I gave the original trespass warning to dispatch, the copy was included with the other paperwork attached to this report. Reporting Officer MICHAELS, GLEN Incident # 12-13308 On 11/11/12 at 20:44 I was dispatched to 1029 Bradford Circle in reference to a possible home break in. Upon arrival I made contact with the complainant Michael Adams who stated that he and his girlfriend Myra Spears left their home today at 1000 and stayed away until 2044. Mr. Adams went on to say that when they arrived back home they found the front door of the house standing open and both bedroom lights on. Mr. Adams advised that when they left the residence they made sure the lights were off and that the front door was shut and locked. As I was making a security check of the residence Mr. Adams advised that he felt like the person that broke into the home was Ms. Spears exboyfriend whom he knows as Curtis. Mr. Adams stated that Ms. Spears and her mother were on their way to his house to see if he was the person that broke in. After learning of the following information I asked Mr. Adams why he suspected Curtis. Mr. Adams advised that he and Ms. Spears got into an argument earlier and she called Curtis. Mr. Adams further advised that when they learned about the home being broke into he and Ms. Spears had a discussion about Curtis possibly trying to break in, to recover a ring that he gave her when they were dating. I noted that the front door did have some pry marks on it but they appeared to be old based on the fact they had been painted over. As I was talking with Mr. Adams, Ms. Spears arrived back on scene and requested Mr. Adams tell her what was going on. Upon learning of why I was there Ms. Spears advised I was wasting my time and she did not want to provide me with any information. Ms. Spears explained she thought I had better things to do and she did not want any other action taken. Pursuant to Ms. Spears request I completed my investigation and took no further action. In court documents filed on Friday, prosecutors called those claims "absurd" and "frivolous," and asked a judge to insist Bulger present proof of any such immunity deal before the trial becomes an "unduly elongated and unmanageable" morass, the Boston Herald reports. "His attempt to foist this absurd notion upon the court demonstrates his disdain for the criminal justice system and society itself," prosecutors write. Bulger's lawyers are demanding that the court turn over all of Bulger's correspondence with the FBI, Department of Justice, and the New England Organized Crime Strike Force concerning some of his fellow Winter Hill Gang members.

"Yogurt's been the problem," says one analyst, because many Americans now prefer that to, say, toast for breakfast. When it comes to other meals, things like energy bars and tortillas have been horning in on bread's market share. And as for Hostess' sugary snacks, well, Americans are becoming more interested in healthier food these days. "Hostess appears to have gotten stuck somewhere in the ‘60s and never really recovered," Sanburn writes. In the Wall Street Journal, Charles Passy strikes a similar tone, noting that while America collectively lost its mind over news of the Twinkie's possible demise (check out the snack's prices on eBay), no one seems to have actually eaten a Twinkie in years. "Affection for the brand [is] largely an exercise in nostalgia," Passy writes. Hi how are you? My name is jescilyn. I am contacting you because I really need help from someone or anyone. I'm not even sure if you are the right person to help. I found your site by searching for domestic violence help. I'm 25 years old and I have a 5 year old daughter and i live in alabama. I live with my boyfriend and his mom. My bf is controlling, abusive, and cheats on me leaving me with stds and feeling useless like its my fault. I do not leave the house unless I'm going somewhere and coming right back. I do not socialize with anyone other than family. And I avoid talking to family because of him and the things he says to me when I do. I just got a brand new car last November and he's drove it more than I have. I lack control of anything I do, but yet he has control of everything he does.

I have tried domestic violence shelters where I could not stay longer than a few hours due to the conditions of the shelters. I need a home to go to, with a bed that's all I need.... I can get a job working from home as that is what I've been doing and currently im in the process of getting a new job. I'm more of a prisoner if anything. I just want to get away from here before something terrible happens. I'm thinking about taking my income tax check in january and leaving behind everything I own to start a new life in Florida with my daughter where nobody will know where I am. He is abusive, it's not too often, few times a year maybe the amount of times doesn't matter what matters is I get the help I so desperately need. I need freedom. Help if you can..... Hi Jescilyn, I hope this correspondence finds you and your daughter safe. With the

limited information you provided in your letter, I would say the domestic violence shelter is the best place to start to get the help you are requesting, so my suggestion would be to go back to the shelter. This is for a few reasons (1). You and your daughter will be safe there (2). Since you have employment in Alabama; relocating to another state without a job and housing will only add to difficulties of your situation and (3). It is easier to obtain for services in the state in which you have established residency.

You state in your email that all you need is “a bed”. I'm confident that your local shelter can provide you and your daughter with the basic necessities you need to regroup and get back some of the control you feel you have lost. Also, once you are in a shelter, they will provide you with information about services such as: counseling, legal aide assistance, injunctions, and relocation funds which may be viable resources available to you.

I know it seems your life is spiraling out of control, but I want you to know that you have more control than what you give yourself credit. You are employed; which means you have financial freedom and do not have to dependent upon your boyfriend to give you money. You have a vehicle; which means you have mobility. You mentioned that he drives it more than you, but you may chose at any moment to revoke his use of your vehicle. You know where the domestic violence shelter is located, so you have a safe place in which to take refuge. Most importantly, you have taken the

initiative to seek help; which means you are becoming empowered to change your circumstances. The hardest part is taking a leap of faith that things will actually be better if (when) you leave. It’s easy to stay, because you know what to expect. You expect him to cheat, bring you disease, be controlling, abusive; both physically and mentally…..My question to you is, is that what you or your daughter deserve….???? I cannot promise everything will be perfect and without challenges once you leave. However, I will say that the hopelessness you feel will slowing dissipate, because there is something very liberating about regaining control over your own life.

I would also recommend you try reaching out to your family and bridging the gap the relationship with your boyfriend has caused. You may be surprised by the love and help you will find from your family.

Incident # 12-13296 On 11/11/12 at approximately 1410 hours, I was sitting at the intersection of Baldwin Road and Minnesota Avenue. I observed a gray Lincoln car bearing Florida tag AUTS50 travel ing southbound approaching the intersection. I recognized the car from a previous traffic stop conducted by Ofc. Seale on 11/9/12. During that stop, Ofc. Seale advised the registered owner, Joel P Byer, that his license was suspended. I also knew the tag was expired as of 6/10/12. I immediately got behind the vehicle and confirmed the tag expiration with the use of DAVID and the sticker displayed on the tag. I activated my emergency lights for the offense, and the vehicle stopped in the 3100 block of SR 77. I made contact with the driver Joel P Byer identified by his Florida drivers license. I advised him of the reason for the stop and also advised him his license was suspended. I advised Ofc. Seale to be en route to my location. Ofc. Seale arrived and confirmed that Byer had knowledge of the suspension due to the fact he informed him of the status on 11/9/12 while he was the passenger of the vehicle. I then asked Byer to step out of the vehicle, and I placed him into double locked handcuffs and escorted him to the rear of my patrol car. Byer adamantly denied any knowledge of the suspension during the entire encounter. I then issued Byer a criminal citation for the offense with a cou r t da t e o f 12 /5 /12 . B ye r acknowledged the court date with a signature and a thumb print. He was then released. Byer was given a warning for the expired tag. Incident # 12-13284 On 11-11-12 at 0127hrs, I was dispatched to the 500 block of Minnesota Ave in reference to a reckless driver. Upon arrival, I spoke with the complainant, Lorraine Gill, who was in the front yard of her residence of 501 Minnesota. Gill said she heard a loud commotion outside and went out to see what she thought was a white Ford truck dragging a city trash can northbound on Minnesota. She said the truck left the area and did not return. She went next door to 511 Minnesota and spoke with the tenants, later identified as Stephanie Block and Thomas Jason Cothron, who were less than helpful. I BOLO'ed the area and found a large quantity of trash strewn about on the road, a mangled trash can in the 400 block, and fresh spin marks leaving 511. I spoke with Block and Cothron, who were sitting on the steps of 511. Both denied knowing anything of the incident, but gave conflicting stories. Block would look at Cothron before she would answer questions relating to the incident. Cothron went inside to get his ID. A short time after he went inside, I heard a series of loud noises followed by the neighboring dog barking. I stepped around back and saw Cothron fleeing on foot after jumping a privacy fence. He saw me and immediately laid on the ground with his arms out beside him. He said he had a warrant, which was later confirmed through the Florida Parole Commission. (Violation of Conditional Release for burglary) He was later taken to the jail without further incident. The trash was removed/picked up by Block. Reporting Officer POINTER, S SHE WAS IN THE WRONG PLACE AT THE WROUNG TIME Incident # 12-13305 On 11/11/12 at 18:44 I was investigating a verbal argument at 2210 Hammocks Square Drive when I conducted a warrants check on one of the subjects involved, Mary Hicks. Upon conducting a warrants check on Ms. Hicks through FCIC/NCIC it was determined that she had an outstanding arrest warrant for failing to pay traffic fines. Ms. Hicks was taken into custody and transported to the Bay County Jail where she was booked in. Reporting Officer HOBBS, L Incident # 12-13334 On 11-12-12 at about 1549 hours, Ricky Burgfeld came into the Lynn Haven Police Department at 1412 Pennsylvania Avenue. Ricky told dispatch he had a warrant in Holmes County FL. I arrived and checked Ricky through NCIC and the warrant for grand theft was confirmed through Holmes County. Mr Cooper threatened to put her

head through the window Incident # 12-13333 On 11-12-12 at approximately 1540 hours, I responded to the 1600 block of Michigan Avenue in reference to a domestic call. Upon arrival I met with Whitney Hand. She was standing on the side of the road and appeared visibly shaken and upset. She stated that that her boyfriend, Jerome Terrell Cooper, picked her up from a hair appointment. She stated they started arguing because he was late picking her up. She stated that she was in the front passenger seat of his car. She stated they were driving on Michigan Ave when Mr Cooper threatened to put her head through the window. She stated that he then slammed the right side of her head against the passenger window. She stated she then punched his right shoulder to get him to stop. She stated that Mr. Cooper then stopped the vehicle, dragged her out, and threw her into the ditch. She stated that he then drove away. I observed that Ms Hand did not have any visible injuries. She was rubbing her elbow while I was talking to her. She stated she and Mr. Cooper had lived together for two years. She stated she was scared to go home, and asked to be taken to her mother's house. She stated she did want to file charges. Ms Hand gave a sworn witness statement. She was given domestic rights/ notice of legal rights. Ms Hand

signed the receipt of domestic rights/ notice of legal rights. She was then taken to her mother's house on Cocoa Ave. I completed a complaint affidavit to be sent to the State Attorney's Office for the issuance of a summons. Reporting Officer MICHAELS, GLEN Incident # 12-13346 On November 12, 2012 at approximately 1920 hours, I was dispatched to 2210 Hammocks Square Drive Apt 107 in Lynn Haven, Florida in reference to a battery. Upon my arrival, I made contact with Michelle Warren. Warren stated that she had allowed a friend, Mary Hicks, to stay with her for a few days. Warren said that on November 10, 2012 at approximately 2000 hours, Hicks had punched her in her left pinky finger during an argument, causing the finger to be broken. Warren went to the hospital for treatment on today's date, 2 days after the incident, and was advised by her social worker, Pam Silva, to report

the incident to police today. I asked Warren why she didn't report the incident on the date it happened, and she said she didn't think it was a big enough deal to call the police over. I asked Warren why she didn't report it on November 11, 2012 when officers from this agency were called to her apartmentconcerning a verbal argument between her and Hicks. Warren said she was not able to speak with officers without Hicks present, and she was scared of Hicks. Warren is physically handicapped, and has a caregiver. I spoke with her caregiver, Sarah Jackson, separately and away from Warren. Jackson said she was present in the apartment and unaware of an argument on November 10. She said she remembered Warren had spilled coffee and she was awakened by the noise. Jackson came out of the bedroom to check on Warren, at which time Warren told her everything was fine. Jackson cleaned up the mess and went back to bed. On November 11, Jackson noticed Warren's left pinky finger was purple. When she inquired about the injury, Warren told her she had accidentally hit her hand on the wall when she spilled the coffee the night before. On today's date, Jackson noticed the finger looked worse and suggested Warren have it looked at by a doctor. After going to the hospital, Warren advised the injury had been sustained when she was punched by Hicks. Jackson said there was some concern by her employer, ResCare, as to responsibility for the medical bill if a battery had occurred. Jackson called the police to report the incident after her and Warren returned home. Warren said she did not wish to pursue criminal charges against Hicks, who is currently in jail on a warrant. Warren signed a Declaration of Intent, and said she only wished to have the incident documented in a report. Reporting Officer ENFINGER, C


Incident # 12-13350 On 11/12/2012 at approximately 2216 hrs, I was dispatched to 2615 Country Club Dr. in reference to a domestic disturbance. Upon arrival I made contact with the complainant Robert Noe, the female had left before I arrived, Officer Adkins pulled her over leaving the neighborhood and transported her back to the residence. Robert stated that he and his fiance, Rebecca Theobald were arguing earlier and she tried to hit his cell phone out of his hand and accidently hit him in the face so he wanted her to leave but she refused. Robert said he then called the police station and Rebecca got scared and left the house. Robert had a small red area on his face from where he was hit, Robert said he did not want to press charges because he felt she hit him on accident. Robert said he just wanted Rebecca to leave his house tonight. Robert signed a declaration of intent stating that he did not want to press charges on Rebecca. At that time Officer Adkins transported Rebecca back to her vehicle and instructed her not to return to Robert's residence tonight. Reporting Officer ANTUNOVIC, M Incident # 12-13387 On 11/13/2012 at approximately 1957 hrs, I was dispatched to Walmart in reference to a retail theft. Upon arrival I made contact with Alex Turner, the Asset Protection Associate. Also present was Kimberly Dawn Wright, identified by Florida drivers license. Alex stated that approximately 1920 hrs he observed two females later identified as Kimberly Wright and Breanna Mazullo, select various items of the store to include jeans, electric shaving unit and whitening strips and conceal them in Kimberly's purse. Also Kimberly took a pair of shoes and put them on her feet. The other female, Breanna was observed "looking out" for Kimberly as she concealed the items. The females then split up to leave the store from 2 different exits. Kimberly walked through the garden center and passed all points of sale and was met by Alex Turner

at the front door. Breanna got into Kimberly's car and left the area. The total amount of the merchandise was $194.12. Alex said he observed Kimberly and Breanna working together to conceal the items and wanted to press charges and trespass both of them. Kimberly

provided us with Breannas number, Sgt Hamilton called Breanna and she answered. Sgt Hamilton told Breanna to come back to Walmart to talk to us about what happened but she hung up the phone and apparently turned it off as we were unable to call her back. Alex completed a sworn witness statement to the incident. Kimberly was trespassed from Walmart and I then transported her to PD-1 to complete the complaint affidavit and booking sheet. Once the forms were complete I transported Kimberly to the Bay County Jail without incident. Reporting Officer ANTUNOVIC, M

Incident # 12-13289 On 11/11/12 at approximately 0817 hours, I was dispatched to 1802 Alabama Avenue in reference to a verbal dispute. I made contact with Taryl York in front of the home. York pointed at Alissa leaving as I arrived. I immediately asked York if this was a physical altercation, and he stated no. He advised Alissa Miller-Tripp and himself got into a verbal altercation about him living in the home. He stated he called the police due to prior domestic issues in the past. He stated the altercation was not physical, but he did not want it to become physical. York advised Alissa packed all of his belongings earlier in the morning and placed them in the living room area. He stated he woke and found Alissa moving his things, so they began to argue. York stated he is willing to leave, but he has no where to go on this date. I identified York with the use of DAVID and delivered the information to dispatch. She advised of a outstanding warrant for his arrest out of Clay County Florida. Dispatch contacted Clay County, and they confirmed the warrant and extradition. I then placed York into double locked handcuffs and escorted him to the rear of my patrol vehicle. I transported him to the Bay County Jail without incident. A copy of the warrant, detainer form, NCIC hit confirmation, and booking sheet are attached to this report. Incident # 12-13211 On 11/8/12 at approximately 1143 hrs, I was on patrol driving southbound on S.R. 77 approaching the 2000 block. At that time I observed a black Saturn suv driving in front of me with no tail lights. I activated my over head lights initiating a traffic stop in the 2000 block of S.R. 77. I made contact with the driver, Emerria Forward identified by Fl drivers license. I explained to her the purpose of the stop, she then noticed she did not have her lights on. Dispatch advised that Emerria's drivers license was suspended on 10/29/12 indefinitely for failure to pay fine, notice of the suspension was sent. I issued Emerria a criminal citation for driving while license suspended or revoked with knowledge, Emerria signed the citation and submitted a finger print, I confiscated Emerria's drivers license due to it being suspended. Emerria called her mother to drive her car away, once she arrived they left the scene.

Reporting Officer ANTUNOVIC, M Incident # 12-13248 On 11/9/12 at 19:12 I was dispatched to the Dollar General Store located at 3820 W Hwy 390 in reference to a woman begging customers for money. Upon arrival I made contact with Dorothy Judd who was standing in the parking lot. Ms. Judd denied asking people for money and stated that she was only there to buy butter but it was too expensive. I next made contact with store clerk Shelby Jackson

who stated that the store wished to issue a trespass warning against Ms. Judd for the property. At Ms. Jackson's request I issued Ms. Judd a trespass warning for the Dollar General and then transported her to her home. Reporting Officer HOBBS, L

Incident # 12-13258 On 11-10-2012 at 0031hrs, I was dispatched to 208 E 8th St in reference to a physical altercation. Upon arrival, I spoke with a white male, later identified as David Anthony Yelk, who was sitting in a patio chair under the carport. Yelk stated that he was asleep in his two roommate’s bed, one of which is his brother Jason John Yelk and his girlfriend, Melyssa Meghann Sharp-Navarette, when they came home from a night on the town. He said they woke him up abruptly, dragged him out of the bedroom, and began attacking him by hitting him in the face. He said the struggle continued until he was thrown through the living room window. He had slight redness to his head and face, but declined medical treatment. He later provided a sworn audio statement, which will later be transcribed.

I spoke to both Jason and Melyssa. They both said they came home to find David asleep in their bed. They said they politely woke him up and he attacked them. They said David, at one point, directed his attack to Melyssa, tried to choke her, and Jason hit him several times in the face to get him off of her. They both said at some point, David forced Jason's hand through the window causing it to shatter. Jason had a laceration on his right hand, but also declined medical treatment. Melyssa said her neck hurt, but had no redness or swelling. They too, provided a sworn audio statement which will later be transcribed. None of the three wanted to pursue criminal charges on each other. They all signed a Declaration of Intent, which was approved by Sgt. Hernandez. Both Yelks were given the DV paperwork, of which they signed a receipt for. David agreed to leave for the evening and return when everyone is sober. Due to not being able to determine the primary aggressor, no arrest was made. This report should be forwarded to the domestic violence center.Reporting Officer POINTER, S

are changing colors. ‘Tis the season for getting together with friends and family and enjoying good food. The Bay County Health Department is reminding residents to use proper food safety practices to remain healthy through the holidays. With a little planning and attention to detail, unpleasant and even life-threatening gastro-intestinal illnesses can be avoided.

Food safety starts in the grocery store. Keep meat, poultry, seafood, and eggs separate from all other foods in the grocery bag or cart. Traveling from store-to-store and long car rides to be with friends and family can expose foods to unsafe temperatures for extended periods of time. Warm temperatures can make your holiday meal a veritable “petri-dish” for bacteria. Bacteria increase most quickly at temperatures between 40 0 and 140 0 F. Keeping frozen items frozen and refrigerated items refrigerated until preparation and cooking can minimize the time bacteria have to grow. If preparing a turkey, plan ahead. Turkeys should be thawed in the refrigerator and never on the countertop. A 16 – 20 pound turkey can take four to five days to thaw in the refrigerator and 8-10 hours to thaw submerged in cold running water. Removing the giblets facilitates further thawing. Be sure to not over pack your turkey with stuffing, contents need room for expansion to cook properly. Turkeys should be cooked in a preheated oven at 325 0 F or higher until all parts of the turkey and stuffing/dressing have reached 165 0 F. Internal temperatures should be verified with a meat thermometer at the thickest part of the breast, the innermost part

of the thigh, the wing, and the center of any stuffing/dressing.

Clean kitchen surfaces and wash your hands

before and after preparing each item to avoid cross contamination. Ready to eat foods should not be

prepared on surfaces that have not been sanitized with a bleach solution or the commercial equivalent (read the use instructions on the sanitizing product).

During holiday gatherings, it is easy to lose track

of time and how long things have been “sitting out.” Refrigerate or freeze perishables, prepared food, and leftovers within two hours. Use shallow containers to ensure rapid cooling. Make sure the refrigerator is set at 40 0 F or below and the freezer is set at Zero 0 F.

Here are a few more tips to keep you and your

family healthy throughout the holidays: — Wash hands and sanitize utensils and

surfaces before and after food preparation. — A person who has had nausea,

vomiting, or diarrhea in the weeks proceeding a holiday gathering should put an extra effort into hand washing before handling food that others will be eating. Bacteria and viruses that cause gastro-intestinal illnesses can be excreted in feces for weeks after an infected person no longer has symptoms.

— Bacteria and viruses can also be transferred from inanimate objects such as faucet handles, doorknobs, countertops, kitchen utensil handles, condiment bottles, etc. Sanitize these items frequently and wash your hands after touching them.

— Don’t cross contaminate. Surfaces should be cleaned with a bleach solution or other commercial disinfectant between preparation of different foods. Use separate cutting boards and plates for produce and for meat, poultry, seafood, and eggs. Discard grooved or pitted cutting boards as they can no longer be thoroughly cleaned.

— Dish sponges, brushes, rags, and hand towels are some of the most contaminated surfaces in the kitchen. Sponges and brushes should be disinfected with a bleach solution or sent through a wash and dry cycle in the dishwasher. Rags and hand towels should be replaced, laundered, and dried frequently.

— Don’t store raw meat over other items in the refrigerator or put the meat in a bowl or tray. This will prevent meat juices from dripping on ready to eat foods.

— Always wash fruits and vegetables, even if you plan to peel them.

Army Releases October

Suicide Data

The Army released suicide data today for the month of October. During October, among active-duty soldiers, there were 20 potential suicides: five have been confirmed as suicides, and 15 remain under investigation. For September, the Army reported 15 potential suicides among active-duty soldiers: four have been confirmed as suicides, and 11 remain under investigation. For 2012, there have been 166 potential active-duty suicides: 105 have been confirmed as suicides, and 61 remain under investigation. Active-duty suicide number for 2011: 165 confirmed as suicides, and no cases under investigation. During October, among reserve component soldiers who were not on active duty, there were 13 potential suicides (nine Army National Guard and four Army Reserve): three have been confirmed as suicides, and 10 remain under investigation. For September, among that same group, the Army reported 16 potential suicides. Since the release of that report one case was added for a total of 17 cases (13 Army National Guard and 4 Army Reserve); five have been confirmed as suicides, and 12 remain under investigation. For 2012, there have been 114 potential not on active-duty

suicides (75 Army National Guard and 39 Army Reserve): 83 have been confirmed as suicides, and 31 remain under investigation. Not on active-duty suicide numbers for 2011: 118 (82 Army National Guard and 36 Army Reserve) confirmed as suicides, and no cases under investigation. “Suicide is preventable, and its prevention is a shared responsibility among all members of the Army family,” said Gen. David M. Rodriguez, commanding general, U.S. Army Forces Command. Rodriguez said that everyone is empowered to intervene and save lives, “effective intervention requires leadership involvement and support, an environment that promotes help-seeking for hidden wounds like depression, anxiety and post-traumatic stress and prior knowledge of available local and national resources. We all must take the time to do a self-inventory to assess the presence and impact of stressors in our lives. Of equal importance is the awareness of the needs of others around us. Upon my arrival, I spoke to the victim who advised that his Girlfriend's Ex-Boyfriend kicked his front door in following a heated verbal dispute and also damaged a flat screen TV after pushing it off of the living room entertainment center . The victim stated that the suspect gave him to the count of three and then kicked the door in, causing approximately $500.00 damage. The victim advised that the suspect then entered his residence and continued to argue with him at which time the victim called police. The victim stated that the suspect then pushed the TV off of the stand . The victim stated that his girlfriend then left with the suspect to keep there from being any further

problems. I was able to make contact with the Girlfriend / Witness at her residence as well as the suspect. The Witness confirmed that the suspect did in fact kick the front door in and that he was very upset with the victim and they were arguing back and forth. The witness stated that there had been several times tonight when the victim and suspect spoke regarding their relationship with the witness and that it eventually escalated into the suspect becoming so angry that he was threatening the victim and eventually made forced entry into the victim's residence. I handcuffed the suspect and he was advised his Miranda

Warning . The suspect agreed to speak about the incident. The suspect refused to acknowledge the fact that he was ever at the victim's residence and denied committing any criminal acts. I notified Sergeant Hines of the facts and circumstances surrounding the case at that time. I obtained photographs of the damaged property at the incident location and have submitted the photographs into property evidence. Sergeant Hines then contacted Det. R. Perkins in reference to this case. I transported the suspect to the Panama City Police Department for further interview. Det. Perkins arrived at the police department and the investigation was turned over to him. The suspect was later transported to Bay County Jail. This concludes my involvement in this case. Ofc. I was dispatched to 2810 W. 15th Street in reference to a disturbance. I made contact with Suzette Patrick. She stated she was involved in a verbal altercation with her daughter Jennifer Patrick and her boyfriend, William Smith. Suzette stated her daughter has been accusing her of stealing her prescribed pain medication. William stepped in and began to argue with Suzette as well. To avoid any further altercation, Suzette stated she wished to have William trespassed from her property. William was issued a trespass warning and was advised not to return and he stated he understood. William left the property in my presence.

The Bay County Sheriff's Office was presented with over $7,000 in new toys and cash today by the American Muscle Car Club of Panama City. The toys and money were collected at the Project 25 Show and Shine car show, held today at the Holiday Inn near the Panama City Mall. Project 25 is the annual toy drive for needy children of Bay County. For the past 11 years, the kick-off event for Project 25 has been the Show and Shine car show. This year's show is the most successful event to date. The Bay County Sheriff's Office will accept donations of new, unwrapped toys and cash to provide toys for needy children at Christmas. Cash donations will be used to purchase toys at the Super Wal*Mart in Callaway at 6 pm on December 11, 2012. Volunteers are asked to attend this shopping event to help shop for the children. Donations for Project 25 can be made at the Bay County Sheriff's Office on 3421 North Highway 77 in Panama City.

The Panama City Police Department continues to investigate the burglary of a vehicle that occurred on

November 17, 2012, in the area of the 1300 block of Stephens Drive and the subsequent arrest of the suspect. The Panama City Police Department first received a call from the complainant at approximately 9:45 am in reference to a male suspect being near the scene of the burglary, and that he was armed with a handgun. As officers approached a subject who matched the description given by the caller near the scene, he resisted verbal commands and direction from the officers and physically resisted efforts to detain him. An officer observed a weapon as the male subject reached for it, and fled on foot towards the end of Stephens Drive. Other officers secured the surrounding area and assistance was requested from the Bay County Sheriff’s Office to help locate and apprehend the suspect. The suspect was located at approximately 11:30 am after a ground search in a wooded area near the railroad tracks. A Bay County Sheriff’s Office K-9 and its handler located the suspect. The suspect did resist the efforts and commands of officers during his apprehension and received injuries from the K-9 during the arrest. He was later treated and released from a local hospital for his injuries and transported to the Bay County Jail for the below listed charges. Officers did recover the handgun used during this incident from the area where the suspect was arrested along with items taken during the vehicle burglary.

The arrested person has been identified as Phillip B. Mathes, 31 years of age. Mathes is charged at this time

with, armed burglary of a vehicle, aggravated assault on a law enforcement officer, resisting an officer with violence, battery on a law enforcement officer, felon in possession of a firearm, possession of a firearm with an altered or

removed serial number and violation of probation. If you have any information in
