Bed Bug Treatment Contract Bed bugs have become an increasing problem.  The bed bug company has equipment to kill bed bugs.  This equipment uses heaters to bring the home up to 140 degrees, similar to a hot car in the summer sun.  Bed bugs start to die at 120 degrees and quickly die at higher temperatures.  The first treatment is _____________ per room.  If bed bugs return after treatment, there will be a charge for additional treatments, so please follow all recommendations to prevent re­infestation. Bed bugs have become resistant to pesticides.  The pesticides cause the bed bug to move deeper into furniture and walls making extermination more difficult.    The bed bug company will provide the equipment and labor to treat the home.  The resident will, by signing this contract, approve and allow access to their home by treatment personnel and will follow the guidelines on the Bed Bug Treatment Preparation Information Sheet.  The following guidelines will be expected by the resident: 1)  The resident will remove all pets, live plants, aquariums and other living things from the home during treatment. 2)  The resident will provide electricity. 3)  The resident takes responsibility for any damage to the contents of the home. 4)  Anything that could be damaged by heat should be placed in the refrigerator or removed from the home. 5)  The resident will remove all medicines, vitamins, fruits, vegetables, chocolates, meltable foods, wines, liquors, artificial sweeteners, vinyl products, record albums, CD’s, DVD’s, audio/video tapes, aerosol or pressurized cans like hairspray, spray­on deodorants, bug sprays, spray paints, asthma inhalers, aerosol cleaning products and soft drinks 6)  The resident will remove all wax based products, make­up, lipstick, candles, wax figurines, crayons, oil paintings, furniture with fragile finishes or glue points, wooden or stringed musical instruments including pianos, ammunition, CO2 cartridges, all flammable materials,  butane lighters, lamp fuel, food warmer fuel, solvents, etc. 7)  The resident will shut down and unplug electronics (sleep mode is not enough) and partially deflate air mattresses, plus store all loose papers and other articles since strong fans are a part of the heat treatment. 8)  This listed items above are a necessary and important list for your items to be protected and your home to be safe.  This list is not all inclusive and is not limited to.  9)  FYI:  Most things come to us by truck and are exposed to temperatures up to 180 degrees in the truck before we purchase them.  Although the above precautions are recommended, heat damage is not common except to meltable items. 10)  The resident agrees that The bed bug company is not responsible for any injury, disease or illness caused or allegedly caused by bed bugs or any other insect or contamination caused by insects.  I have read, understand and agreed to all of the provisions contained herein and acknowledge receipt of a copy of this contract.  I hold The bed bug company, the operators and any others involved in the completion of this treatment, harmless of any liabilities connected with this treatment.   Resident Signature ______________________________________________________         Date___________________    Resident Signature ______________________________________________________         Date___________________      Company Representative _________________________________________________         Date___________________    Checks Payable to:   Miller Enterprise, Southeast, LLC
