Wat wil het kleppen-team eigenlijk zien?
Identificeer je met hun ideeën en oplossingen
Soms is het niet helemaal duidelijk wat
iemand wil…
MCQ1 –Kleppen-team
• Inschatting ernst klepvitium
• Mechanisme helder krijgen
• Reparabiliteit beoordelen (+ succeskans)
• Uitgebreidheid (bij endocarditis)?

Endocarditis – classic diagnosis
• Pathological criteria
• Clinical (DUKE) criteria:
• Definite -
– 2 major criteria
– 1 major and 3 minor criteria
– 5 minor criteria
• Possible -
– 1 major and 1 minor criteria
– 3 minor criteria
Modified
Duke
criteria
(‘old’)
• Viridans streptococci, strept bovis
• Staphylococci (aureus, epidermidis)
• HACEK (Haemophylus, Actinobacillus, Cardiobacterium, Eikenella, Kingella)
• Community acquires enterococci (faecalis)
• Q fever (coxiella burnetti)
Typical pathogens
Duke’s Major Criteria (2)
• Evidence of endocardial involvement
– positive echocardiogram for endocarditis• oscillating intracardiac mass on valve or supporting structure,
or in the path of regurgitant jets, or on implanted material, in the absence of an alternate anatomic explanation
• abscess
• new partial dehiscence of prosthetic valve
– new valvular regurgitation (increase or change in pre-existing murmur not sufficient)
Echo:
Oscillating intracardiac mass on valve
Echo: Abscess
Echo: New partial dehiscence of prosthetic valve
• Rocking motion
Echocardiography is cornerstone for diagnosis
Challenges in diagnosis of endocarditis
• Peri annular extension: 30-40% negativeblood culture
• Imaging issues:
– Sensitivity of echo ….
– Acoustic shadowing with echo
– Role for metabolic imaging?
3D echocardiography – virtual reality
Echocardiography (TTE and TOE)
• Easy, non invasive bedside technique
• High spatial (up to 0.2 mm) and temporal (15-60 ms) resolution
• Assessment of regurgitation and obstruction
• No radiation
But, although high specificitiy:
only moderate sensitivity (TTE ~40%, TOE ~80%) for diagnosis of endocarditis
Acoustic shadowing
Issues:
A+B: ‘ncc/lcc’ region on TTE
C+D: ‘rcc’ region on TOE
importance of performing bothtechniques
E+F: 3D echo does NOT solve thisissue
Advanced imaging techniques
CHALLENGE:
To overcome shortcomings of currentstrategies to diagnose PHV endocarditis andits extension.
Role of other imaging modalities:
1. MDCT
2. FDG-PET scanning
Complementary value of CT
- AVR (Carbomedics)
- Severe AR
- Mycotic aneurysm
Habets et al. IJCVI. 2012
Complementary value of CT
• Vegetation
• Peri annular extension
• Anatomic relationships
More accurate diagnosis
Optimal treatment planning
Allograft: Detection of vegetation by MDCT
a) TOE short axis: no vegetation
b) TOE short axis: moderate AR
c) MDCT: vegetation at lcc.
AVR (Perimount): Mycotic aneurysm at left cusp
Relationship of aneurysm to coronary artery …
MDCT: Paravalvular leakage AVR
AVR (SJM): abscess formation
a) TOE short axis: echolucent cavities
b) TOE: flow in coronary, but not cavities
c) MDCT: aspecific signs of aortitis, but no abscesses visualized ….
Surgery confirmed the 2 abscesses.
Additional value of PET/CT(A)
Metabolic imaging (FDG-PET/CT):
• 18F-fluorodeoxyglucose positron emission tomography including low-dose CT (not gated, no contrast)
• low-carbohydrate diet
• Additional imaging with retrospectivelyelectrocardiogram-gated computedtomography angiography (CTA)
Case
• Patient, 61 yrs
• AVR (SJM) 6 months ago
• Admission: Fever, shiffers, complaints
• Blood culture: Staph. Aureus
• TTE + TOE: No abnormalities found
• Persistent complaints + fever
CTA dag 3
CTA - after close look: aortic wall thickening …
FDG PET fusion with CTA:

Day 8:
Cont…..
Day 3:
Day 8:
PET/CT as additional major Duke criterium …
• Prospective study, 72 patients with suspicion forPHV endocarditis
• Sensitivity of modified Duke (incl. TTE/T0E) raisedfrom 70% to 97%.
• But what about specificity? (no control group)
Saby et al. JACC, 2013;61:2374
PET/CT: Moderate sensitivity for vegetations
Extra cardiac finding: metastatic spleen infection
Focus of endocarditis …
Modified ESC criteria Endocarditis
