Behavioural Reporting• Historical perspective
- System first introduced in 2005 to identify hazards- Expanded in 2012 to include behavioural reporting
• Multi cultural system one system global application- Words used for descriptions- Words in listings incorporating widest choices- Simple in application and easily understood
• Works for physical hazards as well as behaviours- One form reports both, simultaneously if required
• Enables praise to be provided - Essential for a balanced use of the reporting process
Why do people behaviours fit in the system?
• Focus is on behaviour to confirm where hazards are generated
- Human behaviours causation to 90%+ of accident events
- Complacency, accepting circumstances without challenge
- Situational awareness limiting action
- Ignoring hazards, not seen as a threat
• Why positively consider behaviour?
- Give confidence to act and challenge a situation
- Positive immediate response to enable correction from the challenge
- Submission of reports even if no discussion takes place
- Corrective action is initiated immediately
- Reports submitted formulated for trending analysis

The Risk Assessment
Rate how you would consider the extent of the consequences of an accident that could have resulted had you not intervened, rate the potential you observed. Click the bullet to the right, only one may be selected.
Accepted that a resulting accident would have minor consequences resulting in first aid only without any lost time or loss of assets but is an area for continuous improvement.
The potential for injuries which may generate medical treatment or in extreme cases lost workdays. The behaviours should be monitored due to the risk and could be reduced further by appropriate controls.
The potential is serious with the expectation lost time injury would result and in extreme circumstances a single or multiple fatalities. Behaviours must be modified due to risk and further controls must be in place to reduce risk.

- 30% of trip hazards entered in FOCUS relate to poor housekeeping; unattended rubbish, unkempt rugs
& flooring & poor cable management are the top hazards identified
- 20% of hazards identified relate to water; rain water ingress from unmaintained roofs and doors, faulty
equipment such as fridges, air conditioning units and water dispensers
- 10% of hazards identified referred to hydraulic fluid, oil or other lubricant being spilled or leaked onto
the hangar / work area floor
- The remainder of hazards identified refer to poor lighting, condition of steps / stairs and poor drainage
coverings

Example of Hazards Identified during September:
• Hydraulic fluid from jacks was found on the hangar floor
• emergency exit point - the fire alarm went off. I noticed the back side down
the stairs was wet and flooding slowly, so it occurred to me in the event of a
real fire, people will have issues having to evacuate in the rain. The floor
was wet, with flood at the bottom meaning people will have to slow down,
watch steps during evacuation hence slowly others down.

was found in the flammable cabinet -Unmarked substance was found

Example of Hazards Identified during September:
• I noticed an engineer working at height on a step ladder seemingly without
head protection on Stand 19 on G-CERY. I noticed earlier that a colleague
was holding the ladder, but observed at 1550 there was no one holding the
ladder while engineer was leaning over the aircraft. I felt this had potential for
injury.
During August a total of 119 potential hazards were submitted through FOCUS card submission
