InsuranceinyourSuperThefollowingGuideprovidesfurtherinformationabouthowinsuranceinthePlanworksandshouldbereadinconjunctionwiththePDS.

Insuringyourmostvaluableasset,you,isanimportantpartofyouroverallfinancialplan.Itcanhelpprotectagainstunexpectedeventsthatcanaffectyouandyourfamily’sfuturefinancialsecurity.

Havinginsurancecoverwithinyoursuperannuationcanbeacostandtaxeffectivewaytocoveryourselfagainsttheunexpected.Itisalsogenerallycheaper*thanbuyinganindividualpolicyoutsideofsuper.HavingaccesstoDefaultCover(ifeligible)meansthatyouareautomaticallycovered(althoughconditionsandexclusionsmayapply)withouthavingtocompleteanypaperworkorprovideevidenceofyourhealth.

ThePlanoffersthreetypesofinsurancecover:

Death and Total and Permanent Disablement (TPD) cover

TPDcovercanhelpcoverfinancialcommitments,includingmedicalcosts,andcan be invested to ensure you receive an income to assist with everyday living.

Paysalumpsumbenefitifyouareunableto ever work again because you are permanentlydisabled.

Income Protection cover IncomeProtectioncanhelpyoutomeetcash flow commitments like bills and medicalexpenses.

Paysareplacementmonthlyincomeinthe event you are unable to work for an extendedperiodoftimebecauseofinjuryor illness.

ThePlanofferstwooptionswithyourinsurancecover:

1. Default Cover

TherearetwotypesofDefaultCoverinthePlan:

• Standard Default Cover You will generally receive a default amount of DeathandTPDinsurancecoverautomatically,subjecttoyoumeetingoureligibilitycriteriaandconditions.ThisisavailabletoalldivisionsofthePlan(includingthoseinvestedintheBendigoMySuperinvestmentoption),howevertheamountofcoverwillbedifferentdependingonyourage,memberdivision,genderandoccupation.

You can choose to decline some or all of your StandardDefaultCoveroryoucanapplytoincreaseyourStandardDefaultCoveruptoamaximumof6unitswithin90daysofjoiningthePlanbycompletingalimitedhealthquestionnaire.Youcanchoosetoadjustitdowntoaminimumof1unitofDeathOnlyorDeathandTPDcoveratanytime.

• Employer-sponsored Customised Default Cover EmployersintheEmployer-sponsoreddivisionofthePlancanelectacustomisedinsurancedesignfortheiremployeesthatisdifferenttotheStandard Default Cover described above.

2.TailoredCover

You can choose to tailor your insurance cover to suit yourpersonalcircumstances.YouwillneedtoapplyforTailoredCoverandbefullyunderwritten,providingevidenceofyourhealth.Subjecttocertainconditions,youmayalsobeabletotransfercoverintothePlanfromanotherinsurancepolicyyouhave(seethe‘TransferofinsurancecoverfromaPrevious Policy’ section later in this Guide).

What is underwriting?Underwritingistheprocessofassessingtheriskandthecosttoinsureyou.TailoredCoverrequiresyoutocompletethe‘InsuranceApplicationandPersonalHealthStatement’availablefromourwebsite.TALwillusethistodeterminetherisksofacceptingyourapplicationforinsurance.Medicaltests may be required and in some cases your application,ifaccepted,mayincludeapremiumloading or an exclusion,whichmayrestrictthecircumstances under which your claim would be paid.

Eligibility and conditions for coverTobeeligibleforcoverunderthePlan,youmustmeetthe following conditions:

• beamemberofthePlan;and

• be an Australian Resident (unless otherwise agreedinwriting);and

• be in Active Employment on the date cover commences;and

• beaged15to64forDeathandTPDcover,andaged15to69forDeathOnlycover;and

• forIncomeProtection,beaged15to64andbeworkingatleast15hoursperweek.

ThefollowingconditionsapplytoDefaultCover:

Personaldivisionmembers

WhenDefaultCovercommencesafiveyearPre-Existing Condition (PEC) willapply.ThismeansthataDeathorTPDbenefitwillnotbepayableforanyclaimwhichisdirectlyorindirectlyrelatedtoapre-existingconditionthatexistedinthefiveyearspriorto,oratthetime,covercommencedorrecommenced.

OntheexpirydateofthePEC,whichisfiveyearsfromthecommencementdate,ifyoumeettheActive Employmenttestfor10consecutivedays,standardcoverconditionswillapply.However,ifontheexpirydateofthefiveyearPEC,youdonotmeettheActive Employmenttest,thePEC will continue until such time as the Active Employment test is met.

YoumaywishtoconsiderTailoredCoverifyouwishtoapplytoremovethePEC condition.

Employer-sponsoreddivisionmembers

If you are not in Active Employment on the date cover commences,coverwillbeLimited Cover.

Default CoverDefaultCoverisunitisedwhichmeansthatthecostofcoverwillremainthesame,buttheamountofcoverperunitwillchange as you get older.

Standard Default Cover

StandardDefaultCoverconsistsoffourunitsofDeathandTPDcoveratacostof$4perweek($1perunitperweek).Thiscostispayablebyyouandisdeductedmonthlyfromyourmemberaccount.Yourcoverwillbeeffectivefromthedateyoumeettheeligibilitycriteria(seethe‘Eligibilityandconditionsforcover’sectionearlierinthisGuide),providedthatacontributionisreceivedwithin120daysofthedateyoujointhePlan.AsnotedpreviouslyyoucanchoosetodeclinethiscoveronyourBendigoSmartStartSuperapplicationform.Youcanalsocancelyourcoveratanytimebycompletingthe‘InsuranceVariationandCancellation’formavailablefromourwebsite.

AsanewmemberinthePlan,ifyoumeettheeligibilitycriteria(seethe‘Eligibilityandconditionsforcover’sectionearlierinthisGuide),youcanapplytoincreaseyourStandardDefaultCoveramountfromfourunitsuptosixunitswithin90daysofjoiningthePlanthroughasimplifiedapplicationprocess.Totakeadvantageofthisoffer,youwillneedtocompletethe‘IncreaseDefaultCover-NewMembers’formavailablefromourwebsite.Thisformincludesalimitedhealthquestionnaire.

IncreasedcoverwillcommenceuponthedatethatTALacceptstheapplicationforincreasedcover.

YoucanreduceyourStandardDefaultCoveramountfromfourunitstoaslittleasoneunitofDeathOnlyorDeathandTPDatanytimebycompletingthe‘InsuranceVariationorCancellation’formavailablefromourwebsite.However,youshouldbeawarethatifyouwishtoincreasecoveragainatalaterdate,youwillneedtoapplyforthiscoverandbefullyunderwritten.Theacceptanceofyourapplicationwillbesubjecttotheusualevidenceofhealthrequirements.

Reducedcoverwillcommenceonthedatethatyourapplicationisreceivedbyus.

IfyouareconsideringdecliningDefaultCoverorcancellingyourinsurancecover,itisstronglyrecommendedthatyouseekadvicefromalicensedfinancialadviserbeforeyoudoso.

Refertothe‘Thecostofyourinsuranceandbenefitspayable’sectionlaterinthisGuidetoworkouthowmuchDefaultCoveryou are entitled to (if any).

Employer-sponsoredCustomisedDefaultCover

EmployersintheEmployer-sponsoreddivisionofthePlanwhohavefiveormoreemployeescanelectaCustomisedDefaultCoverdesignfortheiremployeesthat is different to the Standard Default Cover describedabove.Theamountofcoveryouwillreceiveand the cost of this cover will be based on what your employerhasnegotiatedwithTAL.Detailsofthiscover will be included in the welcome kit that is sent toyouonceyoujointhePlan.

IfyouareamemberoftheEmployer-sponsoreddivisionandyouremployerhasnegotiatedaCustomisedDefaultCoverdesign,youmaynotbeeligibletoadjustyourDefaultCover.Contactourcustomer service centre for more information about whatyouroptionsarewithcustomisedinsurancecover.

TailoredCoverHowmuchinsurancecoveryouneeddependsonyourpersonalcircumstances.WhilstSandhursthasnegotiated a level of cover that it considers to be acceptableforthePlan’smemberbase,itmaynotbethemostappropriatelevelofcoverforyou.Werecommendyouconsideryourpersonalsituationwiththehelpofalicensedfinancialadviserandadjustyourcoverifnecessary.PleasecallusifyouwishtomakeanappointmenttoseeaBendigofinancialplanner.

Youhavethefollowingoptionstotailoryourcover(subjecttoeligibilityasstatedearlierinthisGuide):

• applyforFixedCover(DeathOnlyorDeathandTPDcover).Youcanapplyforthiscoverontopof your Default Cover or instead of your Default Cover;

• addIncomeProtectioncover;

• transferexistingcoverintothePlanfromanotherinsurancepolicy,subjecttoconditions(see‘TransferofinsurancecoverfromaPrevious Policy’ later in this Guide) (this can be added to yourDefaultCoverand/orFixedCover);and/or

• applyforadditionalcoverbasedonalifeevent,subjecttoconditions(seethe‘IncreasingyourinsurancecoverthroughaLifeEvent’sectionlaterin this Guide).

HowtoapplyforTailoredCover

ToapplyforTailoredCoveryouwillneedtobeaneligiblememberandsupplyevidenceofhealthbycompletingthe‘InsuranceApplicationandPersonalHealthStatement’formavailablefromourwebsite.

Alternativelyyoucan:

• completeyourapplicationonlinewithyouradviser;or

• completeaTele-InterviewwithTAL(whichmustbeorganised by your adviser).

Inaddition,youmaybeaskedtoprovideadditionalinformationasrequestedbyTAL.

ThemaximumamountofinsurancecoveravailableandagelimitationspermemberunderthePlanareshowninthetablebelow:

Subjecttoanyspecificconditionsthatapplytotheparticulartypeofcover,yourcoverwillcommencewhenyourapplicationforinsurancehasbeenacceptedbyTAL.ThetermsofacceptancewillbedecidedbyTALonacasebycasebasisafterreviewing your evidence of health or any other evidenceitmayrequire.ShouldTALwishtoimposeconditionsontheacceptanceofyourapplication(e.g.anexclusionorpremiumloading),aletterwillbesenttoyousettingoutthetermsofyouracceptance.

InsurancecoveracceptedbyTALwithadditionalconditionswillnotcommenceuntilTALhasreceivedyoursignedacceptanceoftheseterms.

Fixed Cover

YoucanapplyforFixedDeathOnlyorFixedDeathandTPDcover.

WithFixedCover,yourcoverwillstaythesamebutthecostofcoverwillchangeasyougetolder.Thisisdifferent to how Default Cover works where the cover isunitised,whichmeansthatyourcoverwillchangeas you get older but the cost of cover will stay the same.

FixedCovercanbeappliedforinmultiplesof$1,000andthepremiumwillbebasedonyourage,memberdivision,gender,smokingstatus(formembersinthePersonaldivision)andoccupationclassification.

Toworkoutthecostper$1,000ofcover,referto‘Thecostofyourinsuranceandbenefitspayable’section later in this Guide.

PleasenotethatyourinsuredamountforTPDcoverwillreduceby20%eachyearfromagenextbirthday62tobenilatagenextbirthday66.ThisreductionincoverwillonlyapplytoyourTPDcover.Thereductioninbenefitsisreferredtoastapering.

Example:JackhasafixedsuminsuredunderhisDeathandTPDcoverof$100,000.Jack’sDeathandTPDcoverwilltaperasfollows:

IncomeProtection

HowmuchIncomeProtectionyoucanapplyforwilldependonyourEarned Income.ThemaximumIncomeProtectioncoverthatyoucanapplyforis75%ofyourEarned Incomeplusuptoanadditional10%tocoversuperannuationcontributions.

IncomeProtectioncoverinthePlanisbasedonindemnityvalue.Thismeansthatthebenefityouwillbepaidwillbebasedonthelesserof75%of your Earned Incomeatthetimeofclaim,yourcurrentinsuredlevelofIncomeProtectioncover,or$30,000amonth.YoumaywanttoadjustyourIncomeProtectioncovertobetterreflectyour income changes.

IncomeProtectioncanbetailoredtosuityourrequirements.Youcanapplytobeunderwrittenfor:

• a30or90dayWaiting Period;and

• a Benefit Periodofeither2yearsoruptoage65.

ThecostofyourIncomeProtectioncoverisdeterminedby a number of factors including the Waiting Period and Benefit Periodselectedbyyou(andacceptedbyTAL),yourage,memberdivision,gender,smokerstatus(formembersinthePersonaldivision),andoccupationclassification.

ToworkoutthecostofIncomeProtectioncoverpleasereferto‘Thecostofyourinsuranceandbenefitspayable’sectionlaterinthisGuide.

InterimAccidentCover

IfyouhavemadeanapplicationandarebeingunderwrittenforFixedDeathOnlyorFixedDeathandTPDcover,youmaybecoveredbyInterimAccidentCover in the event of your death caused by an Accident foranamountequaltothelesseroftheinsuredbenefitappliedforor$750,000,whileyourapplicationisbeingassessedbyTAL.

IfyouhavemadeanapplicationandarebeingunderwrittenforIncomeProtectioncover,youmaybecoveredbyInterimAccidentCoverintheeventofanAccident,foranamountequaltothelesseroftheinsuredbenefitappliedforor$15,000permonth,whileyourapplicationisbeingassessedbyTAL.

InterimAccidentCoverappliesfromthetimeyourfullycompleted‘InsuranceApplicationandPersonalHealthStatement’isreceivedbyTALuntiltheearlierof:

• yourapplicationiswithdrawn;or

• younolongermeetthedefinitionofanInsured Person; or

• anassessmentdecisionismadebyTALtoaccepttheapplication(whetherunconditionallyorconditionally),ortodeclinetheapplication;or

• wereceiveyouracceptanceofacounterofferforcover;or

• 90dayspassfromthedateTALreceivesthefullycompleted‘InsuranceApplicationandPersonalHealthStatement’.Ifthe90dayperiodhaselapsedandTALhasnotyetmadeanassessmentdecision,youwillnotbeinsureduntilsuchtimeasTALacceptsyourapplication.Therefore,yourInterimAccidentCoverwillceaseafter90daysifTALhasnotyetacceptedyourapplicationforinsurance.

TransferofinsurancecoverfromaPrevious Policy

Youcanapplytotransferexistinginsurancecoverfroma Previous PolicyintothePlan.

Whenyouapply,youmustprovideusproof(astatementfromthepreviousinsuranceprovider)oftheinsurancecoveryouwishtotransferintothePlan.

TALmayagreetoacceptthetransferofinsuranceandincreaseyourDeathOnly,DeathandTPDcoverand/oryourIncomeProtectioncoverwithoutunderwriting,ifyoumeet all of the following conditions:

• youmustbeunderage60atthetimeyourapplicationisaccepted;and

• forIncomeProtection,youmustbeworkingatleast15hoursperweek.Waiting Periodsof30or90dayswillbeaccepted(otherWaiting Periods may beacceptedatTAL’sdiscretion).AllBenefit Periods otherthantwoyearsandtoage65willbeadjustedto2years;and

• theamountofDeathOnlyorDeathandTPDcovertransferred will be in addition to any existing Default Cover and/or Fixed Cover you currently have in the Plan(subjecttotransfertermsmaximums);and

• currentPlanpremiumrateswillapply;and

• you meet the Active Employment test on the date of thetransferofcoverandarenot,duetoaccident,illnessorinjury,offworkorunabletoperformyourfull and normal duties on a full-time basis (for at least30hoursperweek)eventhoughyouractualemploymentmaybefull-time,part-timeorcasual;and

• youhavenevermade,andarenotentitledtomake,aclaimforanyillnessorinjurythroughWorkers’Compensation,sicknessbenefit,invalidpensionoranyinsurancepolicyprovidingtotalandpermanentdisablementcover,accidentorsicknesscover;and

• you have not been diagnosed with an illness that reducesorislikelytoreduceyourlifeexpectancytolessthan12months;and

• youhavenothadanapplicationforDeathOnly,DeathandTPDorIncomeProtectioncoverdeclinedorofferedcoveronalternativetermswithaloading,limitationorexclusionadded;and

• you are not engaged in a Hazardous Occupation;and

• your Previous Policy will cease on the commencement of the transferred cover and you will notexerciseanycontinuationoptionorreinstateanycover under the Previous Policy.

Pleasenotethatthefollowinglimitsapplyinrespectofinsurance transfers:

• DeathOnlyorDeathandTPDcoveruptoamaximumof$2millionwithtotalcovernottoexceed$3million(includingcoveralreadyinplaceinthePlan).

• IncomeProtectionupto75%ofEarned Income plussuperannuationbenefitcover(optional)uptoamaximumof$20,000permonth(includingcoveralreadyinplaceinthePlan).

Pleasenotethefollowingwithregardstoyourtransferred cover:

• thetransferredcoverwillbeconvertedtoafixedcoveramountroundedtothenearestdollar.Thismeansthatyourcoverwillstaythesame,butyourpremiumswillchangeasyougetolder.

• anyindividualrestrictions,conditions,exclusionsorpremiumloadingsonyourPrevious Policy,providedthatTALissatisfiedwiththem,willalsoapplytothetransferred cover.

Increasing your insurance cover through a LifeEvent

YoumayapplyforLifeEventsCovertoincreaseyourTailoredCoverifoneofthefollowinglifeeventsoccurs:

• youmarry;or

• thebirthofyourchild;or

• theadoptionofachildbyyou;or

• yourdependantchildstartssecondaryschool;or

• youdivorce;or

• youtakeoutamortgageonthepurchaseofyourprimaryplaceofresidence(eitheraloneorjointlywithanotherperson);or

• you increase your existing mortgage for building or renovatingyourprimaryplaceofresidence;or

• a salary increase.

Youmayapplyforanincreaseofyourcoverduetoalifeeventonlyonceinany12monthperiod.

Anapplicationtoincreasecovermustbesubmittedwithin90daysofthelifeeventoccurringor30daysfrom the date your annual member statement is issued bythePlanfollowingthelifeevent;or60daysfromtheeffective date of your salary increase.

Maximumincreaseapplicable

ForFixedDeathOnly,orFixedDeathandTPDcover,themaximumincreaseforeachlifeeventapplicationshould be the lesser of the following:

• 25%increasefromtheexistingcover;or

• the total amount or amount of increase of the mortgageyouaretakingout(whereapplicable);or

CancellingorchangingTailoredCover

IfyouwishtoincreaseyourTailoredCover(outsideofLifeEventsCover)orreinstatecancelledTailoredCover you will need to be underwritten for the increasedamountandcompletethe‘InsuranceApplicationandPersonalHealthStatement’formavailable from our website.

Continuation of insurance coverforEmployer-sponsoredmembersOnSandhurstbecomingawarethatyouhaveceasedemploymentwithyouremployer,youraccountwillbetransferredintothePersonaldivisionofthePlan.Ifyourmemberaccountbalanceismorethan$2,000andyoucontinuetomeettheeligibilitycriteria,yourinsurance cover will also be transferred when you movetothePersonaldivision.

Theremay,however,bechangestoyourinsurancecalculationsandinsurancepremiums.YournewpremiumwillbebasedonthePersonaldivisionTailoredRatestable(see‘Thecostofinsuranceandbenefitspayable’sectionlaterinthisGuide).

Yournewpremiumwillbebasedonyoursmokerstatusandoccupation.Toensuretheratethatisusedtocalculateyournewpremiumisappropriatetoyourcurrentoccupationandsmokerstatuscompleteandreturnthe‘OccupationalDutiesQuestionnaireand Smoker Declaration’ form to us available from our website.

Yournewleveloftransitionedcoverwillbesubjecttoanytermsandconditionsthatappliedtoyourcover(e.g.loadings)priortotransferincludingmeetingtheActive Employment test at the time of transition with a 12 month Limited Coverprovision.IfLimited Cover provisionsareinforceatthetimeoftransition,theywillcontinuewhenyoutransition,althoughyouwillnothavetorecommencethe12monthtimeperiod.

Ifyourmemberaccountbalanceislessthan$2,000,your insurance cover will be cancelled. We will notify you30dayspriortocancellingyourcovertoallowyou time to contribute to your account and retain this cover if you wish to do so.

ThecostofyourinsuranceandbenefitspayableThecostofyourinsurancecover(TailoredCover)oramountofcoverperunit(DefaultCover)willvaryaccordingtoanumberoffactors,including:

• theinsuredamount;and

• thetypeofinsurancecover;and

• yourage;and

• yourgender;and

• whetheracommissionispayableornot;and

• whetheryouareasmokerornon-smoker(appliestoPersonaldivisionTailoredCover);and

• yourtypeofoccupation-premiumloadingswillapplytooccupationsotherthanWhiteCollar;and

• your member division.

Howandwhenpremiumsarepaid

Yourannualinsurancepremiuminrespectofaparticularfinancialyear(orpartthereofifyourcovercommencesduringafinancialyear)willbedeductedmonthly in arrears from your member account when your cover commences.

We will notify members in writing of any material changeinthepremiumratesapplicabletoPlanmembers.Anyotherchangeswillbemadeavailableon our website.

We may choose to change insurers at any time if we believe the change would be in the best interests of themembersofthePlanasawhole.Inthiscase,wewill notify members of such a change.

Paymentstoadvisers

Sandhurstorarelatedentitymaypaycommissionstoadviserswithwhomithascomplyingarrangementsasprovidedforbylaw.AnysuchcommissionswillbepayablebySandhurstoritsrelatedentityfromitsownresources,andwillnotaffectyouraccountbalance.NocommissionswillbepaidtoadvisersinrespectofthoseinvestedintheBendigoMySuperinvestmentoptionorEmployer-sponsoredmembers.TheamountofanycommissionthatyouradviserreceivesinrespectofyourinterestinthePlanshouldbe disclosed to you by your adviser.

Thesepaymentsareseparatetoanyadviserservicefee that you negotiate with your adviser and direct SandhursttopayfromyourinterestinthePlan.

Sandhurst or its related entity will cease making thesepaymentsifrequiredtodosobylaw.

DeathandTPD

Death Cover

Deathcoverprovidesalumpsumpaymentintheeventthat you die or are diagnosed with a Terminal Illness. Uponyourdeath(orTerminal Illness),theinsuredamount,ifpayable,willbepaidinadditiontothebalance in your member account.

When will your Death Cover cease?

Death cover will cease on the earliest of:

• 30daysafterSandhursthasissuedanoticetoyouadvisingofalapseincoverwherethereareinsufficientmoniesinyourmemberaccounttomeetthepremiumdue;

• ifaDeathbenefitispaidbythePlaninrelationtoyou;

• if a Terminal IllnessbenefitispaidbythePlanandthereisnoremainingDeathcover;

• whenyounotifythePlanthatDeathcoverisnolongerrequired;

• whenyouceasetobeamemberoftheEmployer-sponsoreddivisionanddonotmeettheminimumbalancerequirementof$2,000totransfercovertothePersonaldivision;

• whenyouceasetobeamemberofthePlan;

• the date you reach the maximum insurance age for Deathcoverof70years;

• thedatethepolicyisterminated.

We may reinstate your cover if we receive a contribution within120daysofyourcoverceasingprovidedyousatisfy the eligibility conditions.

TPDCover

TPDcoverprovidesyoualumpsumpayment(ifyousatisfy the criteria) in the event that you can’t work because you’re Totally and Permanently Disabled (see ‘Making a claim’ section later in this Guide for more details).

WhendoesTPDcovercease?

YourTPDcoverwillceaseimmediatelyontheearliestof the following:

• 30daysafterSandhursthasissuedanoticetoyouadvisingofalapseincoverwherethereareinsufficientmoniesinyourmemberaccounttomeetthepremiumdue;

• ifaTPDbenefitispaidbythePlantoyou;

• suicideoccurringduringthefirst13monthsaftercommencementorreinstatementofcoverbyTAL;

• anyintentionalself-inflictedactorinjurywhichoccursatanytime(incasesofaTPDclaimonly);or

• anyotherexclusionthatTALmayapplytoyouasaconditionofacceptanceofcover.

WhendoesIncomeProtectionCovercease?

IncomeProtectionCoverwillceaseontheearliestof:

ExclusionsforIncomeProtectioncover

Nobenefitwillbepayableasaresultofanyofthefollowing:

• anyintentionalself-inflictedactorinjury;

• uncomplicatedpregnancyorchildbirth;

• waroractsofwarwhetherdeclaredornot;

• service in the armed forces of any national or international organisation including active service and training exercises within national or internationalarmedreserveunits;or

• anyotherexclusionthatTALmayapplytoyouasaconditionofacceptanceofcover.

Coverifemployedortravellingoverseas

Your cover continues automatically if you are overseas,subjecttopaymentofpremiumsandmeetingtheeligibilityconditionsofthepolicy.

TALmayrequireyoutoreturntoAustraliaatyourown cost for assessment of any claim. If you have IncomeProtectionCoverandsufferTotal Disability or Partial Disabilitywhileresidingortravellingoverseas,paymentofanybenefitwillceaseaftersixmonthsfromthedatebenefitpaymentscommenced,unlessotherwiseagreedbyTALandadvisedbythePlaninwriting.

Coveronleavewithoutpay

Ifyouareonemployer-approvedleavewithoutpayandcontinuetopaypremiums,covercancontinueifyouareanAustralianresidentasfollows:

• Deathcovercontinues;

• TPDcovercontinuesforthefirst24monthsofyourleavewithoutpay*;and

• IncomeProtection-Total Disability cover continues forupto24months*.

TAL’spriorwrittenconsentisrequiredforTPDorIncomeProtectioncoverbeyond24months.

*OnthebasisthatyouremploymentstatuspriortotheperiodofleaveisusedtodetermineTPD/Total Disability status.

TPDbenefitandthetaxandotherimplicationsthatmayapplytothisbenefit.

AnyTPDclaimcanonlybepaidtoyouifyoumeetaconditionofreleaseassetoutundersuperannuationlawandthepaymentispermittedbytheTrustDeed.Formore information about the conditions of release under superannuationlawpleaseseethe‘HowSuperworks’sectionoftheBendigoSmartStartSuperReferenceGuide.

IncomeProtection

IncomeProtectionprovidesyouwithamonthlyreplacementbenefitifyouareTotally Disabled or

Partially Disabled.

BeforeyoucanreceivebenefitsunderIncomeProtectioncover,youmusthavebeenTotally Disabled asdefinedaboveforaminimumof14consecutivedays,followedbyaperiodofTotal Disability or Partial Disability extending to the end of the Waiting Period. TheWaiting Periodasselectedbyyou(either30or90days)willcommenceonthefirstdayyouaredeemedtobe Totally Disabled.

IncomeProtectionbenefits

TheIncomeProtectionbenefitpayableisbasedontheinsuredamountagreedbetweenyouandTALinyourapplication.However,themaximumIncomeProtectionbenefitthatispayableisanamountequalto75%ofyour Earned Income before you ceased work due to theinjuryorillness,lessanyotherbenefitsyoumayreceive.Thisbenefitmaybepaidforupto2yearsoruntilyoureachage65oryouceasetobeTotally Disabled or Partially Disabled,whicheveroccursfirst.Thebenefitwillbereducedbythe‘BenefitOffsets’referred to later in this Guide.

IfyouareentitledtoanIncomeProtectionbenefit,itwillbepaidbyTALmonthlyinarrears(eachmonthTALwillrequestthatyouprovidethecurrentstatusofyourillnessorinjurybycompletingaprogressclaimform).ThisbenefitispaidbyTALtoSandhurst,andSandhurstwillthendeductthePAYGincometaxandforwardthenetproceedstoyouasifitwerenormalsalary.

SuperannuationContributionbenefit

AnIncomeProtectionbenefitequivalenttoasuperannuationcontributionispayablewhenaTotal or Partial Disabilitybenefitispayable,andcanbeupto10%ofEarned Income.Thisissubjecttoany‘BenefitOffsets’asdefinedbelowthatapply.

Benefitoffsets

AnyIncomeProtectionbenefitwillbereducedbyallbenefitoffsetspaidorpayable,including(butnotlimitedto)workerscompensation,sickleavepayments,socialsecurityandotherincomeinsurancebenefitsorsimilarpayments.

Making a claimIfyouwishtomakeaclaim,pleasecontactyouradviserortheCustomerServiceCentreon1800033426,andtheywillarrangeallthenecessaryformstobesenttoyou.AllinsuranceclaimsmustbeapprovedbyTALbeforeaninsuredbenefitwillbepayable.

PleasenotethatpaymentofanyclaimisconditionalonyouprovidingusorTALwithallrelevantinformation,inatimelymanner,andyourongoingcompliancewithmedicaladviceandtreatment.Youwillpayallcostsrelatingtomedicalorotherevidencesupportingyourclaim,butTALwillpayexpensesforobtainingevidenceitrequests.Otherconditionsthatapplytolodgingaclaimaresetoutinthepolicy.

PleasenotethatanyinsuredbenefityoumayreceivewillbeprovidedbyTALunderthetermsofthepolicy.IntheeventofTALdenyingaclaim,inaccordancewiththetermsofthepolicy,Sandhurstwillbeunabletopaytheinsuredcomponentofthebenefit.

Death Cover

AnybenefitpayableunderDeathcover(includingaTerminalIllnessbenefit)willbepaidasalumpsum into your member account and distributed toyournominatedbeneficiary/s,alongwithyoursuperannuationbenefits.IfyouhavenotmadeaBindingDeathBenefitNomination,yourbenefitwillbepaidtoyourLegalPersonalRepresentative.IfnoLegalPersonalRepresentativehasbeenappointed,SandhurstmaypaythedeathbenefittoyourDependants(asdefinedinthePDS)or,ifSandhurstcannotlocateanyofyourDependants,toathirdpartyindividual,asprovidedforbysuperannuationlaw.Tofindoutmoreaboutnominatingbeneficiariespleaserefertothe‘BindingNominationForm’availablefromour website.

TerminalIllnessbenefit

If you are diagnosed as Terminally Ill and you have DeathcoverthroughthePlan,thenTALwillpayabenefittoyourmemberaccountthatisthelesserofyourtotalDeathcoverand$5million.

ProvidingmembershipofthePlancontinuesandpremiumsarepaid,anyremainingDeathcoverwillbepaiduponDeath(subjecttothepolicytermsandconditions).

TPDCover

IfyoubecomeentitledtoabenefitunderyourTPDcover,TALwillpayyourbenefittoSandhurstasalumpsum.Sandhurstwillthentransferthelumpsumbenefitintoyourmemberaccount,whereitwillbepaidtoyouasalumpsumormayberetainedinthePlan.Pleaseobtainfinancialadviceregardingany

Recurringclaims

If you have made a claim and received an Income Protectionbenefitandsubsequentlyreturntoyourpre-disabilityemployment,buthavearelapseofthesameorarelatedinjuryorillnesswithinsixmonthsofreceivingthebenefit,thepaymentswill recommence without the Waiting Period being reapplied.Inthesecircumstances,theIncomeProtectionpaymentsaretreatedasacontinuationofthe same claim.

AdditionalDeathBenefit

If you die while receiving a Total Disability or Partial Disabilitybenefit,TALwillpayanadditionalsumequalto one quarter or the annual Total Disabilitybenefit.

Assistancewithrehabilitation

If your return to work is likely to be accelerated byaprogramofrehabilitation(thatisapprovedbyyourdoctorandTALpriortotheprogrambeingundertaken),thecostoftheprogrammaybemetbyTAL.

Claimsescalationbenefit

IfanIncomeProtectionbenefitispayableforacontinuousperiodof12months,TALwill,fromeachanniversaryofthecommencementofthatbenefit,increasethebenefitbythelesseroftheincreaseintheCPIforthatperiodor5%.

Your duty of disclosure

UndertheInsuranceContractsAct1984(Cth),when you enter into a contract of life insurance withaninsurer,youhaveadutytodisclosetoTALeverymatterthatyouknow,orcouldreasonablybeexpectedtoknow,isrelevanttoTAL’sdecisionwhethertoaccepttheriskoftheinsuranceand,ifso,on what terms.

You have the same duty to disclose those matters toTALbeforeyourenew,extend,varyorreinstateacontract of life insurance. Your duty however does not require disclosure of a matter:

• thatdiminishestherisktobeundertakenbyTAL;

• thatisofcommonknowledge;

• thatTALknowsor,intheordinarycourseofhisbusiness,oughttoknow;

• ofwhichTALhaswaived.

Iftheinformationyouhaveprovidedtousisinaccurateorincomplete,thismaybetreatedasamisrepresentation.

IfyoufailtocomplywithyourdutyofdisclosureorhavemadeamisrepresentationandTALwouldnothaveenteredintoallorpartofthecontractonanytermsifthefailureormisrepresentationhadnotoccurred,TALmayavoidallorpartofthecontractwithin three years of entering into it. If your non-disclosureormisrepresentationisfraudulent,TALmayavoidallorpartofthecontractatanytime.

TAL,whoisentitledtoavoidallorpartofthecontractoflifeinsurancemay,withinthreeyearsofenteringintoit,electnottoavoiditbuttoreducethesumthat you have been insured for in accordance with aformulathattakesintoaccountthepremiumthatwouldhavebeenpayableifyouhad:

• disclosedallrelevantmatterstoTAL;and/or

• notmadeamisrepresentation.

TALLifeLimited,theInsurer,hasgivenitswrittenconsent in each case to the form and context in which it is included or named:

• tobenamedinthePDSandInsuranceGuideastheInsurerofthePlan;

• tothestatementsregardingTALmadeinthePDSandInsuranceGuide;and

• totheissueofthePDSandInsuranceGuidewiththe insurance information set out in the ‘Insurance’ sections.

Exceptasnotedabove,TALhasnotbeeninvolvedinthepreparationofanyotherpartofthisInsuranceGuide,has not authorised or caused the issue of any other partofthePDSandspecificallydisclaimsliabilitytoanypersoninrespectofstatementsincludedelsewhereinthePDS.TALhasnotwithdrawnitsconsentasatthedateofthePDSandInsuranceGuide.

Glossary of termsAccident Meansanunforeseenviolent,externalandvisibleeventthatoccursaccidentlyduringtheperiodyouareinsured.

Active Employment Means:

• ifemployedwithanemployerorengagedasacontractorbyanemployer,thepersonisactivelyperformingorcapableofactivelyperformingallof the duties and work hours of his or her usual occupationwithhisorheremployerfreefromanylimitationduetoillnessorinjuryforatleast30hoursperweek.Apersonwhoisonemployerapprovedleaveforreasonsotherthanillnessorinjury,whowouldotherwisebecapableofperformingtheirusualoccupation,foratleast30hoursperweek(freefromanylimitationsduetoillnessorinjury),willbeconsideredashavingmettherequirementsofthisdefinition;or

• ifunemployedorself-employed,thepersonisactivelyperformingorcapableofactivelyperformingallofthedutiesandworkhours(subjecttoaminimumof30hoursperweek)ofhisorherusualoccupationfreefromanylimitationduetoillnessorinjury;or

• ifengagedexclusivelyinunpaidDomestic Duties,thepersonisactivelyperformingorcapableofperformingalloftheirfulltimeunpaiddomesticduties free from any limitation due to illness or injuryforatleast30hoursperweek;and

thepersonisnotentitledtoorreceivingincomesupportbenefitsrelatingtoillnessorinjury,fromanysourceincluding,butnotlimitedto,workerscompensationbenefits,statutorytransportaccidentbenefitsanddisabilityincomebenefits.

Australian Resident MeansapersonwhopermanentlyresidesinAustralia,oratemporaryresidentonatemporaryworkingvisaincluding457visaholdersorothersasagreedbyTAL.

Benefit Period Meansthemaximumperiodforwhichthebenefitmaybepayable.

Domestic Duties Meansunpaiddomesticdutiesperformedbyaninsuredpersonwhoperformsonafull-timebasistheunpaiddomesticdutiesathome,includingthefollowing:

a)purchasingcleaningitemsandcleaningthefamilyhome;

b) laundering and ironing the clothing items for the family;

c) purchasingfooditemsandpreparingmealsforthehousehold;and

d) undertaking child rearing at home.

Butexcludesanytasksperformedforsalary,rewardorprofit.

Insured Persons who are actively seeking Gainful EmploymentorareperforminglessthanfulltimeunpaidDomestic Duties will not be deemed to be performingDomestic Duties.

Earned Income MeansforEmployedPersons:

TheannualwagesorsalarylastagreedbetweentheParticipatingEmployerorEmployerandtheInsured Person immediately before commencement of Total Disability,plus

a)anycommissionspaidbytheParticipatingEmployerorEmployertotheInsured Person in the twelve monthperiodimmediatelybeforecommencementofTotal Disability;and

b)allotherregularpaymentsorbenefitsprovidedtothe Insured PersonbytheParticipatingEmployerorEmployerinthetwelvemonthperiodimmediatelybefore commencement of Total Disability,whichwhencombinedwithsub-paragraph(a)TALreasonably considers as the Insured Person’s remunerationpackage;or

forSelf-EmployedPersons:

TheannualincomegeneratedbytheInsured Person fromhisorherpersonalexertion,calculatedbyaveraging the Insured Person’snetincomeperyearforthetwoyearsimmediatelyprecedingcommencementof Total Disability.Forthepurposesofthisdefinitionnet income means the Insured Person’s gross income frompersonalexertionlessallexpensesincurredbythe Insured Personinearningthatincome;butdoesnotincludeinvestmentincome,profitdistributionsorsimilarpayments.

Gainful Employment MeansapersonengagedunderacontractofemploymentorSelf-employedforgainorreward,orintheexpectationofeconomicbenefit.

Hazardous Occupation MeansanoccupationthatTALdeterminesasanoccupationinvolvinghazardousorveryheavymanualworkorpresentingparticularunderwritingdifficulties.

Insured Person Meansapersonwhosatisfiestheeligibilitycriteria.

Limited Cover Means cover for claims arising from:

a)anillnesswhichfirstbecomesapparent;or

b)ainjurywhichfirstoccurred;

onorafterthedatetheinsuredcoverlastcommenced,recommencedorincreasedforyouasapplicable.

Limitedcoverwillcontinueforaminimumof12months,andwillreverttofullcoveronceamemberisin Active Employment for 10 consecutive days.

Medical Practitioner MeansamedicalpractitionerlegallyqualifiedandregisteredtopractiseinAustralia;whoisnottheInsured Person,theownerofthepolicy,theirrelatives,businesspartners,shareholdersoremployees.IfthemedicalpractitionerisoutsideAustralia,heorshemusthavequalificationsequivalenttoAustralianStandards.

Partial Disability benefits MeansanIncomeProtectionbenefitthatispayableifyou have been Totally Disabled for 14 continuous days and,asadirectresultofthesameinjuryorillnessthat caused the Total Disability,you:

a)cannotworkyourpre-disabilityworkinghours,orareunabletoperformatleastoneimportantincomeproducingdutyofyourregularoccupation,ordonothavethecapacitytoworkatthesamelevelasyouwerepriortothecommencementofTotal Disability;

b)sufferapartiallossofEarned Income;and

c) are under the regular care and following the advice of a Medical Practitioner;and

d)inTAL’sopinion,arecomplyingwiththeadviceandtreatment given by that Medical Practitioner.

AnIncomeProtectionbenefitduetoPartial Disability is calculated according to the following formula:

Permanent Incapacity

Meansundersuperannuationlaw,amemberofasuperannuationfundistakentobesufferingfrompermanentincapacityifatrusteeofthefundisreasonablysatisfiedthatthemember’sill-health(whetherphysicalormental)makesitunlikelythatthemember will engage in Gainful Employment for which thememberisreasonablyqualifiedbyeducation,trainingorexperience.

Pre-existing Injury and Illness Exclusion (PEC) Meansanyinjury,illnessorsymptomthatyou:

• wereawareof,orareasonablepersoninyourpositionshouldhavebeenawareof;or

• should have sought advice or treatment (conventional or alternative) from a Medical Practitionerorotherhealthprofessional(incircumstanceswhereareasonablepersoninyourpositionwouldhavesoughtadviceortreatment);or

• hadamedicalconsultationfororwereprescribedmedicationortherapyfor.

Previous Policy MeanstheinsurancepolicyinrespectofanInsured Person,whichisa“lifepolicy”asdefinedundertheLifeInsuranceAct1995(Cth)whichprovideddeathcover,deathandtotalandpermanentdisablementcoverorIncomeProtectioncoverfortheInsured Personinanothersuperannuationfundorwithaninsurer.

Return to Employment Income (RTEI) Means the gross income received by the Insured Person duringthemonthinrespectofwhichaPartial Disability benefitmaybepayable,andwhichisearnedasaconsequenceofhisorherpersonalexertion(includingcommissions,bonusandotherpaymentsthatTALreasonablyconsiderformpartoftheInsured Person’s remunerationpackage),lessallexpensesincurredby the Insured Person in connection with earning that income during that month.

ForthepurposeofcalculatingtheInsured Person’s Return to Employment Income:

a)SuperannuationGuaranteeContributionsarenotincluded;and

b) ifyouareSelf-employed,hisorhershareofbusinessexpensesisnotincluded.

TPD - Definition of Total and Permanent Disablement MeansabenefitispayableunderyourTPDcoveronceyousatisfyTAL’sdefinitionofTPDthatappliestoyou.TheapplicabledefinitionofTPDwillbedeterminedbyTALatthetimeoftheclaim.Moreinformationisbelow.

CoverforTPDwillbeprovidedasTALdeterminesforeach Insured PersoninthePlanonthefollowingbasis:

TPDmeansthatinTAL’sopinion,anInsured Person,whileinsuredbyTALis:

1. under the care of and following the advice of a Medical Practitioner;and

2. For new members from 1 July 2014 (including any TPDcoverreinstatedorappliedforpost1July2014) who meet the Permanent IncapacitydefinitionundertheSuperannuationIndustry(Supervision)Regulations1994(Cth),asamendedfromtimetotime;and

3.meetsoneofthefollowing‘TPDDefinitions’parts(A),(B),(C)or(D)asapplicable(seebelow):

ThePart(s)ofthefollowingdefinitionofTotalandPermanentDisablementapplicabletoanInsured Person withTotalandPermanentDisablementcoverwillbedeterminedbyTALatthetimeofclaiminaccordancewith the following criteria:

a)Personsworkingatleast15hoursperweekarecoveredunderdefinitionsA,B,CandD(i).

b)PersonsnotworkingandinunpaidDomestic Duties arecoveredunderdefinitionsA,B,CandD(ii).

c) Personswhoareunemployedforacontinuousperiodofmorethansixmonths,TPDdefinitionsA,BandCabovewillapply.IfyoureturntoGainful Employment formorethan15hoursperweek(averagedover13weekspriortoyoubecomingTPD,orsuchshorterperiodsifemployedforlessthan13weeksimmediatelybeforedisablement)TPDdefinitionD(i)willalsoapply.

PartB–WholePersonFunction

Yousuffer,solelyduetoinjuryorillness,atleast25%impairmentoftheWholePersonFunction^,arenotengagedinanyoccupationandaredisabledto such an extent as to render you unlikely to everbeengagedinanyoccupationforwhichyouarereasonablysuitedbyeducation,trainingorexperience.

^WholePersonFunctionmeansyousufferWholePersonImpairmentbasedonthelatestadditionoftheAmericanMedicalAssociationpublication‘GuidestotheEvaluationofPermanentImpairment’,oranequivalentguideapprovedbyTAL.

PartC–ActivitiesofDailyLiving

Youare,solelyduetoinjuryorillness,unlikelytoeverbeabletoperformatleasttwoofthefollowingActivitiesofDailyLivingwithoutassistance:

• Dressing:theabilitytoputonandtakeoffclothing

• Bathing:theabilitytowashorshower

• Toileting:theabilitytousethetoiletincludinggetting on and off

• Mobility: the ability to get in and out of bed and a chair

• Feeding:theabilitytogetfoodfromaplateinto the mouth where ‘assistance’ means the assistanceofanotherperson.

PartD(i)–AnyOccupation

WhereattheDateofDisablement,theinsuredpersonwas

(i) Gainfully Employed15ormorehoursperweekor:

(ii)NotGainfully Employed however had been working 15ormorehoursperweekpriortobecomingnotGainfully Employed and had not been Gainfully Employedforacontinuousperiodof6monthsbefore the date of disablement and:

a)havebeenabsent,asaresultofinjuryorillness,fromemploymentforsixconsecutivemonthsormore;and

b)afterconsiderationofallrelevantevidence,youare disabled to such an extent as to render you unlikely to ever again be engaged in any occupationforwhichyouarereasonablysuitedbyeducation,trainingorexperience.

PartD(ii)Domestic Duties

WhereattheDateofDisablement,theinsuredpersonwas,notemployedandengagedinunpaidDomestic Dutiesathomeand,

a) asaresultofinjuryorillness,areunderthecareof a Medical Practitioner,

b) unabletoperformthoseDomestic Duties,

c) unabletoleavehomeunaided,

d) have not been engaged in any Gainful Employment foraperiodofsixconsecutivemonthsormoreaftertheoccurrenceoftheinjuryorillnessand,attheendofthesixmonthperiod,inTAL’sopinion,afterconsiderationofallrelevantevidence,youare disabled to such an extent as to render you unlikelytoperformthoseDomestic Duties or engageinanygainfuloccupationforwhichtheyarereasonablysuitedbyeducation,trainingandexperience.

Terminally Ill or Terminal Illness

Means:

a) two Medical Practitionershavecertifiedinwriting,that an Insured PersonsuffersfromanIllness,orhasincurredanInjury,thatislikelytoresultinthe death of the Insured Personwithinaperiod(‘thecertificationperiod’)thatendsnotmorethan12monthsafterthedateofthecertification;

b) at least one of the registered Medical Practitioners isaspecialistMedical PractitionerpracticinginanarearelatedtotheIllnessorInjurysufferedbytheInsured Person;

c) theIllnessandcertificationreferredtoinparagraph(a) occur while the member has cover under the policy;

d) foreachofthecertificates,thecertificationperiodhasnotended;and

e)TALaresatisfied,onmedicalorotherevidence,thatdespitereasonablemedicaltreatment,theillnessorinjurywillleadtotheInsured Person’s death within12monthsofthedateofthecertifications.

Total Disability or Totally Disabled Meansthat,asadirectresultofanillnessorinjury,you are:

• unabletoperformatleastoneimportantincomeproducingdutyofyourregularoccupation;

• notworkinginanycapacity,gainfulorotherwise;and

• under the regular care and following the advice of a Medical Practitioner,and,inTAL’sopinion,complyingwiththeadviceandtreatmentgivenbythat Medical Practitioner.

Waiting Period Meanstheperiodoftimethatmustexpirebeforeyouare eligible to claim.
