EMPLOYEE REFERENCE BENEFITS ENROLLMENT GUIDE
BENEFITS SUPPORTED IN WORKDAY
▪ United States (including Puerto Rico): All benefits except
retirement plans
▪ Great Britain: Health, travel accident, retirement plans
▪ Ireland: Retirement plans
▪ Countries that track dependents for taxation purposes
(Switzerland, UK, Taiwan, Galway, Latin America, Korea, United
States, Great Britain, Belgium, France, Spain and Lebanon)
▪ Employees on international assignment must request benefit
changes through Global Mobility
ACCESSING WORKDAY
▪ From Medtronic Network: Click the Workday link on the myMedtronic home page.
▪ From Any Computer with Internet Connection: Go to www.myworkday.com/medtronic
IMPORTANT: If adding new dependents to your health plans, you must create the dependents in Workday before starting the Benefits Enrollment process. Dependents can then be enrolled during the Benefits Enrollment.
ADD NEW DEPENDENTS / EDIT DEPENDENTS
1. From Home page, click Benefits worklet and then click Dependents. 2. To create a new dependent, click Add.
Dependent Options ▪ If dependent is already a beneficiary or emergency contact in
Workday, click the Edit icon and then click the Prompt icon, and select the dependent.
▪ Enter Effective Date (defaults to today’s date). ▪ To use the dependent as a beneficiary, click that row (or Edit icon)
and check Use as Beneficiary checkbox. Dependent Personal Information ▪ Click a row (or Edit icon) and enter Legal Name, Gender, Date of
Birth and Relationship. Contact Information ▪ Employee’s address displays as the default. Enter address, phone and
email information as desired. Identifier Information (varies by location) ▪ For U.S. and Puerto Rico: A Social Security Number (National ID) is
required when enrolling a dependent in a benefit plan. ▪ To enter National ID, click Add. Click Country prompt and select
country from list. Choose Social Security Number (U.S.) for the National ID Type and then enter ID number.
▪ When finished, click Submit. 3. Next Steps: If additional steps are required, click the To Do button to
learn what actions need to be taken. Click Submit once you have read the message.

Note: Non-U.S. Employees: Inform your location’s
payroll department of any changes to dependents.

SPOUSE HEALTH CARE COVERAGE QUESTION (Employees in U.S. and Puerto Rico)
You must answer the Spouse Health Care Question when first enrolling a spouse and then annually during open enrollment.
1. From the Home page, click Benefits and then click Dependents. 2. Click Edit button on your Spouse’s profile. 3. Scroll down to the question: “This person is or could be enrolled in
Health Care coverage elsewhere?”
4. Click the Edit icon. Check the box if this person has or can have coverage through his/her own employer.
5. Enter the date your spouse will become eligible for health care coverage elsewhere. a. If your spouse is already eligible for health care coverage
elsewhere, enter today’s date. 6. Click Save. 7. When finished, click Submit.
START BENEFITS ENROLLMENT (Applies to new employees and Open Enrollment events; not all choices shown below will appear for every employee)
If you are unable to locate the Life Event/Open Enrollment action, contact your local HR Service Center.
1. Log in to Workday.
2. Open your Inbox.
3. Locate the Benefit Enrollment action and click it.
The action displays on the right.
4. Click for full screen viewing mode.
HEALTH CARE ELECTIONS
1. Benefit Plans ▪ To enroll in a plan, select Elect. ▪ To withdraw from a plan, select Waive.
2. Enroll Dependents ▪ To enroll a dependent, click prompt and select the name from the list
of dependents. ▪ To remove enrolled dependent, click X next to the name. ▪ The coverage tier will auto populate based on the dependents selected.
3. Click Continue. ▪ Or click Save for Later to save your elections and continue at a later
time. ▪ Or click Cancel to leave the Enrollment process without saving.
If you chose Save for Later or Cancel, you can return to the Enrollment Change action by clicking the Open button in your Inbox.

DEPENDENT IDS (Employees in U.S. and Puerto Rico only) This step only displays if a dependent that you enrolled in a health care plan does not have a SSN in Workday.
1. Select Identification # button and enter the Social Security Number (SSN)
for the dependent.
2. If the Identification # is not available, select the Reason ID is Not
Available button and enter a reason in the text box.
3. Click Continue.
Dependents: You must provide a Social Security Number for dependents covered under a medical plan. Refer to Dependent Verification Documentation Requirements.
HEALTH SAVING ELECTIONS (Employees in U.S. only)
1. If a health care plan other than the Consumer Health Plan is selected,
then the HSA page will be greyed out. If you see a gray page, click
Continue.
2. To receive the employer contribution, you must click Elect for the HSA-
Employer benefit plan. If you are not eligible or do not want to
participate in the HSA, select Waive.
• During Open Enrollment, if you had the HSA Employer election in
2017, it is reflected in this event and will carry forward for 2018 unless
you deselect it.
3. If you want to make additional pre-tax contributions to the HSA, click
Elect for the HSA-Employee benefit plan.
• For new elections or to change your annual election, enter an amount
in the Total Year or the Bi-weekly contribution field, not both.
• During Open Enrollment, if you had an HSA Employee election in
2017, the most current annual election is reflected in this event. This
election will carry forward for 2018 unless you make a change.
4. Click Continue.
SPENDING ACCOUNT ELECTIONS
(Includes Healthcare FSA and Dependent Care FSA)
(Employees in the U.S. only)
1. To enroll in a plan, select Elect.
▪ In a midyear qualifying event, enter your new annual amount. If you
plan to stop or reduce contributions for the year, the amount cannot
be less than contributions already taken for the year.
2. Contributions
▪ Enter amount for total year.
3. Supporting Information
▪ The maximum contribution amount displays for each benefit plan.
4. When done with this screen:
▪ Click Continue to move to the next screen.
▪ Click Go Back to return to previous screen.
Flexible Spending Accounts (FSA) are part of the Open Enrollment process. You must enroll in a plan annually if you want to participate. You will not be able to make mid-year changes to this amount unless you have a life event that allows this type of change. If you are enrolling in a CHP plan with HSA, you are not eligible to enroll in the HealthCare Spending Account.
Note: Your options may vary based on the plan that has been chosen.

INSURANCE ELECTIONS
1. Click the Insurance Plan Dependencies and Coverage Limitations
section arrow to expand/collapse this section. If available, this section
provides information that may impact your insurance elections.
2. Benefit Plan
▪ To enroll in a plan, select Elect.
▪ To withdraw from a plan, select Waive.
3. Coverage Level
▪ To select level, click prompt (click in field to display prompt) and select
new level.
▪ Click Continue to move to the next screen, or click Go Back to return to
previous screen.
BENEFICIARY DESIGNATIONS
1. Beneficiary
▪ To add a beneficiary to a plan, click the plus sign.
▪ In the new row, click prompt and select Beneficiary Persons, Trusts, or
Create (to add beneficiary or trust).
▪ To remove a beneficiary, click the next to the name.
▪ To create a beneficiary from an existing dependent, click Create and
select Add Beneficiary Using Existing Contact.
2. Primary Percentage/Contingent Percentage
▪ For each beneficiary, select “Primary Percentage” or “Contingent
Percentage.”
▪ Enter the percent allocation.
3. When done with this screen:
▪ Click Continue to move on or Go Back to return to previous screen.
Primary/Contingent Percentages: The total percentage for all Primary beneficiaries must equal 100%.
You can designate one or more Contingent beneficiaries. The total percentage for all Contingents must equal 100%. You cannot designate a beneficiary as both Primary and Contingent.
ELECTED COVERAGES AND ELECTRONIC SIGNATURE
1. Evidence of Insurability
▪ This section appears if you elected an insurance level that requires
proof of good health.
2. Elected Coverages
▪ Review summary of benefit elections.
3. Waived Coverages
▪ Click the arrow to expand/collapse this section.
4. Beneficiary Designations
▪ Click the arrow to expand/collapse this section.
5. Attachments
▪ Do NOT attach documents. Documents attached in Workday are not
reviewed by Benefits personnel. Submit your documents to AskHR.
6. Electronic Signature
▪ Read the authorization statements (if applicable).
▪ Select the I Agree checkbox.

7. Click Submit to save your elections or Go Back to return to previous
screen.
PRINT CONFIRMATION PAGE
1. To create a printable version (PDF) of your benefit elections, click the
Print button at the bottom of the page.
2. Click the PDF button that displays at the bottom of the page to view the
printable version of the confirmation page.
3. Use the task bar to print the PDF file or to save it.
4. When finished printing or saving, close the PDF window. 5. Click Done at the bottom of the confirmation page.
Confirmation page only displays after you SUBMIT your elections.

CHANGE OPEN ENROLLMENT ELECTIONS
After submitting your elections, you can return to the Open Enrollment
action to view your elections or make changes.
1. Click the Benefits worklet.
2. Click the Change Open Enrollment button.
3. Use the Continue button to move through the Open Enrollment screens.
4. If you make changes, click the Submit button when finished.
5. Print a new copy of the confirmation page.
CHANGE BENEFITS
Use Change Benefits to modify or add benefit elections, plans or coverage levels.
1. From the Home page, click the Benefits worklet.
2. Under the Change heading, click Benefits.
3. Review the following list of life event situations before selecting your
Benefit Event Type. If you don’t see your event type listed or have
questions on which to event to choose, contact AskHR at 800-987-3565.
4. Click Benefit Event Type arrow and select the applicable event type from
the list.

5. Enter Benefit Event Date.
▪ Enrollment Offering Types display (based on Benefit Event Type
selected) when all required fields on Change Benefits screen are
entered.
6. Click Submit.
7. If Up Next screen displays, select Open button to begin the Change
Benefit Election steps.
8. Use the Continue button to move through each step of the Benefit
Election process.
On Hold: If you have an open benefit enrollment event, you must complete this event before you can complete the new event.

9. On the Benefit Elections Review page: Review your elections before you
click submit.
10. Review the Electronic Signature information: Your electronic signature
is required before you can submit your benefit elections. Go to the
Electronic Signature section and select “I Agree.”
11. Click Submit to accept your changes
12. Click Print to print a confirmation statement for your records.
