Subject: DEMAND NOTICE U/S 12(3) OF EOB ACT, 1976

Default Period: 01-AUG-2011 to 31-MAY-2015

A sum of Rs. 107,100.00 being employer's share of contribution under section 9 of the Act and a sum of Rs. 21,420.00 beingemployees' share of contribution under section 9B of the Act, totaling to Rs. 128,520.00 in respect of 6 insured persons hasbeen determined as payable under section 12 of the Act. The statutory increase of Rs. 53,316.00 @ 2% per month, tilldate ,on the determined amount is also payable as per regulation 4 of Employee's Old - Age Benefits (Contribution) Rules,1976.

2. You are, therefore, required to pay the said amount of Rs. 181,836.00 within fifteen days of receipt of this notice.

3. In case of non-payment of amount mentioned above, the said demand shall be recovered as arrears under the LandRevenue Act 1967 (W.P Act XVII of 1967).
