Nationwide access to biometric screeningsA common barrier to screening participation is convenience. Quest Diagnostics Health & Wellness completes screenings for over 3.5 million participants from more than 4,400 companies every year, delivering nationwide access that helps increase participation while saving time and money. There are four convenient ways to complete screenings:

• 2,200 Quest Diagnostics Patient Service Centers nationwide, including locations in Walmart and Safeway stores • Onsite events• At-home with Qcard™ using the Dried Blood Spot method• At a physician’s office with a Physician Results Form and secure online upload

Heath improvement servicesAn employer’s Blueprint for Wellness starts with screening, but increases engagement by offering intervention services immediately following the screening experience. These services help accelerate health improvement by targeting those in your population at the greatest risk for chronic health conditions, and providing a convenient means for those individuals to adopt the behaviors necessary for change.

• Diabetes prevention program: An estimated 84.1 million Americans have prediabetes, a condition that puts them at high risk for developing type 2 diabetes, and nearly 90% don’t know they have it. The estimated economic burden of diabetes in the United States is over $245 billion in medical costs and lost productivity.2 The general lack of awareness of prediabetes and its future economic burden has driven us to create a powerful partnership to help reduce type 2 diabetes risk, by offering the Omada Health® Diabetes Prevention Program to qualified participants.3

• Physician Health Information Sessions (PHIS): Physicians are the most trusted source of health information. Quest Diagnostics Health & Wellness has built an innovative tool that enables those who participate in screenings to review their results with a board-certified physician. The PWN Health physician can refer the participant into in-network care, and an upgrade is available to allow PWNHealth, our national clinician network, to schedule appointments on behalf of the participant.

• Outcome-Based Rewards™ and Activity-Based Rewards™: Our Rewards Engine compares participants’ clinical and biometric outcomes to pre-established targets to determine their eligibility for the incentives. We also provide clients with an automated means to score and deliver pass/fail status based on completion of a wellness screening, attestation, or a wide variety of third-party-administered activities.

Personal reportingDeciding how employees engage with their screening results is just as important as determining how they can complete their screening. Quest Diagnostics Health & Wellness offers a variety of report types with varying levels of features and customization. Each report helps participants decipher their results and develop actionable plans for health improvement. All reports also include a Medical Summary Report to promote coordination of care with the employee’s personal physician.

Aggregate reportingAggregate reporting provides actionable population health insights for employers, to identify focus areas for planning future wellness initiatives. If 40 or more individuals complete an employer’s biometric screening, an OurCompany Profile™ aggregate report will be provided to the employer following the conclusion of the program.

Year-over-year cohort data (up to 6 years) is included each year a Blueprint for Wellness is offered for a population. An executive summary highlights the most significant risk factors affecting the population, along with a financial savings analysis based on cohort results and change in risk status (when available).

Key result averages for the population screened will also be compared to the Quest Diagnostics database to distinguish similarities and differences with potential implications for programming.
