SIMS, Metabolic Unit, Department o f University o f Burlington, Vermont 05405 A B S T R A C T Eight untrained, obese females (>30% body fat), ages 25-33 y r , were studied before, a t 1 w k , a n d after 6 w k while taking either o f t w o 830-kcal/d diets: carbohydrate-containing (CC) group ( n = 4 ) : 3 5 % protein, 2 9 % fat, 3 6 % carbohydrate; carbohydrate- restricted (CR) group ( n = 4 ) : 3 5 % protein, 6 4 % fat, 1 % carbohydrate. Endurance, at -75% of VO2max (maximum oxygen uptake) o n a cycle decreased from base line b y 5 0 % a t 1 a n d 6 wk i n the C R group, b u t there w a s n o change in t h e CC group. Preexercise muscle glycogen (vastus lateralis) d i d no t change significantly i n t he C C group, b u t w a s decreased b y 4 9 % in t h e CR group after 1 w k , and b y 5 1 % after 6 w k . There w a s a close correlation between percent decrease in resting muscle glycogen a n d percent decrease i n endurance ( r = 0.79,P < 0.01). T he mean fasting a nd exercise plasma glucose con- wa s lower i n t h e CR group in t h e C C group after 6 wk, but n o subject became hypoglycemic during exercise. Serum FFA, lactate, pyruvate, beta- hydroxybutyrate, acetoacetate, insulin, a n d glucagon changed similarly in t h e t w o groups during exercise at base line, 1 a nd 6 w k . Glycerol concentration w a s higher i n t h e C R group during exercise only after 6 w k . Increases i n serum lactate concentrations, a n d a mean exercise respiratory quotient o f 0.93 suggested that cycle exercise a t -75% VO2max used predomi- nantly glucose a s a fuel. I n contrast, significant decreases occurred in resting muscle glycogen a n d endurance, during similar exercise, after 6 w k o f a 1 % carbohydrate, 830-kcal/ d diet. INTRODUCTION Physical exercise has been recommended a s therapy adjunctive t o l o w calorie diets i n t he treatment of obesity (1-3). There is little data, however, o n t h e capacity forexercise during rapid weight loss. Christen- s e n a nd Hansen ( 4 ) a nd later Bergstrom e t a l . (5), showed that t h e capacity for exercise wa s reduced after a f ew days o f carbohydrate restriction, b u t these were acute experiments i n which little time w a s allowed f o r adaptation. McClellan e t a l . ( 6 ) i n 1930 studied t w o subjects w h o took a 1-2% carbohydrate, weight-maintaining diet f o r 1 y r . Although n o formal exercise testing w a s done, neither subject noted a n y change i n h i s ability t o perform a 2.25 mile "run" i n -20 min after 3 m o o f t h e diet. These early studies o f carbohydrate restriction have current relevance t o t h e renewed interest i n l o w carbohydrate diets, but they n o t delineate t he capacity f o r exercise after adaptation t o these diets. I n an earlier study i n this laboratory (7), obese sub- jects were studied before, a n d after 1 a n d 6 w k o f eating a hypocaloric, l o w carbohydrate diet. Endurance, whilewalking o n a treadmill at -60% maximum oxygen uptake (VO2max) w a s decreased after 1 wk, b u t b y 6 w k had increased t o 155% o f base line. D I S C U S S I O N

T h i s s t u d y w a s d e s i g n e d t o d e t e r m i n e w h e t h e r t h e r ea r e d i f f e r e n c e s i n m e t a b o l i c f u e l u s e a n d / o r t h e c a p a c i t yf o r s t r e n u o u s e x e r c i s e a f t e r a d a p t a t i o n t o a c a r b o h y d r a t e -c o n t a i n i n g o r a c a r b o h y d r a t e - r e s t r i c t e d , h y p o c a l o r i cd i e t . D u r i n g 6 wk o f a l o w c a r b o h y d r a t e d i e t ( 1 % c a r -

b o h y d r a t e , 8 3 0 - k c a l / d ) , m u s c l e g l y c o g e n s t o r e s w e r ed e p l e t e d t o -50% o f b a s e - l i n e l e v e l s a n d t h e r e w a s as i m i l a r r e d u c t i o n i n t h e c a p a c i t y f o r s t r e n u o u s e x e r c i s e( - 7 5 % V O 2 m a x o n a c y c l e ) . In our previous study (7), s ix untrained obese sub-

jects were evaluated before and a f t c e r 1 and 6 wk of a

low carbohydrate, hypocaloric d i e t . The base-line

endurance t e s t was performed on a treadmill at a mean

intensity of 76% VO2max, but despite carrying a back-

pack with weight equivalent to the weight l o s s , the 6-

wk t e s t was performed at only 60% VO2max. This

probably a c c o u n t e d for part of the 55% increase in

endurance d u r i n g treadmill e x e r c i s e a f t e r 6 wk. I t i s

also possible that after adaptation the elevated FFA

c o n c e n t r a t i o n s during t h e period of t h e d i e t d e c r e a s e d

t h e r a t e o f glycogen depletion, and thus improved

e n d u r a n c e . T h i s h as also been shown by t h e s t u d y

of C o s t i l l e t al. (14) during similar FFAelevations and

work l o a d s ( 6 8 % VO2max on a t r e a d m i l l ) . The " g l y c o -

l o a d s > 70% VO2max o n a c y c l e ( i n u n t r a i n e d s u b j e c t s )c o m p a r e d w i t h 60% V O 2 m a x o n a t r e a d m i l l , a s i n t h ep r e v i o u s s t u d y , m u s c l e g l y c o g e n i s n o t " s p a r e d " a f t e ra d a p t a t i o n t o t h e d i e t , a n d t h e d e p l e t i o n o f m u s c l e g l y -c o g e n o c c u r r i n g d u r i n g c a r b o h y d r a t e r e s t r i c t i o n l i m i t se n d u r a n c e . I f t h e g l y c o g e n s t o r e s a r e m a i n t a i n e d , a sd u r i n g t h e h y p o c a l o r i c c a r b o h y d r a t e - c o n t a i n i n g d i e t ,
