The authors demonstrate how advances in neuroimaging and video electroencephalographic monitoring have greatly enhanced our understanding of the diagnostic yield of ictal semiology. Contents: The text consists of 11 sections, including a brief interesting historical introduction to epilepsy as represented in art and literature in which ictal semiology is used to interpret Raphael’s famous painting Transfiguration. Brief overviews of magnetic resonance imaging, positron emission tomography, and single-photon emission computed tomography are provided. In- vasive electrical stimulation of the cortex with mapping of corti- cal response as it applies to semiology and presurgical planning is presented in detail. The most important part of the text, and a subject that occupies over a third of the book, is the section devoted to the symptoms associated with focal ictal discharges in the brain. These chapters are used to introduce a classification system of epilepsy that is based on semiology. The remaining sections of the book are brief and provide overviews of factors that can precipitate and modify seizures, postictal signs and symp- toms, scalp and intracranial electroencephalographic evolution of seizures, status epilepticus, and seizures as a manifestation of various neurologic diseases. Strengths: This textbook provides the most comprehensive re- view of ictal semiology currently available. The importance of ictal behavior in the clinical evaluation of patients with a seizure disorder is clearly demonstrated. The neurologic history of the ictal behavior remains a critical diagnostic tool even in the era of sophisticated neuroimaging and video electroencephalographic monitoring. An excellent compact disk with video clips accompa- nies the book. Deficiencies: The book has no major deficiencies; however, se- lected chapters may be considered outside the theme of the text.
