Tips for Successful Follow Up for 6

Month GPRA Interview

Follow up begins at intake!

Inform the client about the impor-

tance of the GPRA data collection

requirement. Remind them that

they will receive a $20 incentive for

their participation.

Calendar and track the GPRA fol-

low up window (5-8 Months follow-

ing In-take) for each of your cli-

ents… so that you don’t miss it!

Collect and enter good contact/

locator information, confirm contact/

locator information at each encounter

with your client and always update

the information in the VMS.

Utilize the Care Coordination service

to assist with staying in contact with

your clients– Follow up will be that

much easier.

We started Year 3 with an award of $3,227,840. Of this amount, $2,532,450 is allocated for assessment and treatment services.

To ensure we adequately manage Year 3 funds and meet SAMHSA’s 50% resi-dential & 50% outpatient/RSS service allocation, we implemented a Residential Treatment criteria.

Since we did not reach our Year 2 client

intake goal of 2801. The difference of 1656 is carried over into Year 3. As a result, we are expected to reach 4090 new clients in Year 3. Our original Year 3 client intake goal was 2434.

2434 + 1656 = 4090

I encourage you to work with your local community organizations to increase referrals to your program.

A: No. The Assessment

positive influence this funding has had on people’s lives.

Stories have a unique capacity to engage interest and
