Camp  Sunflower  Enrollment  Form  Please  call  (916)  780-­‐GROW  (4769)  or  email  us  to  get  a  list  of  this  summer’s  chosen  camps,  dates  and  

cost.    Then  refer  to  “Camp  Descriptions”  to  make  your  selection.  There  is  No  registration  fee  and  all  snacks  are  provided  by  Camp  Sunflower.    In  the  event  that  we  do  not  get  enough  campers  to  sustain  a  camp  we  will  have  the  option  to  cancel  the  camp.    At  that  time,  your  enrollment  fee  will  be  refunded  to  

you.    Payment  is  due  in  full  to  hold  your  spot  and  a  30  day  notice  is  required  for  all  cancellations.
