Meningitis Vaccine Offered effective 5/25/05

• Covered for ages 11, 12, 17 (vaccines limited)

• High school and college students living in dorms

• Over the age of 19 vaccine is covered only by a participating provider.

• Members will have access to a discounted network of participating providers. If a provider does not participate with the Empire Plan, but does partipcate with the discount network the charge will be subject to deductible and coinsurance but will be capped at the discounted allowance. Balance billing will not be allowed.

Prescription Drug Benefits for 2006Effective January 1, 2006, there will be a new insurance carrier and pharmacy benefits manager Empire Blue Cross Blue Shield and Caremark Rx Inc.

This change will impact the mail order and the Preferred Drug List for 2006.

Scholarship Program

• Dependent children who are enrolled in the fund and are eligible for coverage.

• Earn at least 12 undergraduate credit hours toward degree requirements for the semester

• Grade-point average of 2.0 or higher for the semester (as shown on the official transcript)

• Credits earned at a state-operated SUNY school (e.g., does not include SUNY community colleges)

• Apply with in 60 days after the ending date of the semester

• $500 per semester up to a maximum of 8
