Defining Capacity Building

• “interventions aiming to produce sustained changeat levels ranging between the individual and entire nations” (Sajiwandani, 1998)

• Capacity building “can be applied to interventions which have changed an [individual’s] organization’s or community’s ability to address health issues by creating new structures, approaches and/or values” (Crisp et al; 2000)

• There are four approaches mostly used for capacity building:

1. top-down organizational approach;

2. bottom-up organizational approach;

3. partnerships approach; and

4. community organizing approach

• Top-down organizational approach:– Changing Existing Policies or Practices

– Organizational Restructuring

– Policy Development

What can be done?– Change some of the policies and practices regarding in DH and PH?

– Modify the current structure of DH or PH or …. ?

– Design a shorter duration of surgical skills trainings to decrease maternal mortality in remote areas ? (India-18month training in basic and advance EmNOC)

– Public health practitioner certification ?

• Bottom-Up Organizational Approach: • The most common type know to us

• Provision of Skills to the Staff

• Developing a core of well-trained individuals

• What can be done?• Train other staff besides nurse, midwives, doctors?

• Specific Surgical Skills Training for MDs ?

• Skills training for specialists working in DHs and PHs?

• Promoting a culture of continuous learning and improvement?

• Partnership Approach:

• Strengthening the relationships between organizations

• Providing possibilities for the two-way flow of knowledge

• Collaborations and information sharing between organizations

hospital in Kabul becoming a mother of PHs or DHs OR a PH for DHs)

• Developing Sisterhood relationship between hospitals ? (PH with PH or DH with DH)

• Training is just one approach to capacity building, there are three other approaches which can be used to build the capacity of health care workers or health care organizations or build the capacity of health sector.
