Cardiac Care Program • Medication assessment, teaching, and management for prevention of complications related to new medication or medication changes. • Telemedicine for daily in-home monitoring of vital signs/weights and nursing contact as appropriate. • 24/7 Triage nursing which is instrumental in preventing ED visits and decreasing re-hospitalization rates. On call nurses respond to emergent triage calls. • Palliative Care and symptom management for patients with End Stage Heart Disease. Our medical director for Palliative Care is available for consultation and home visits in Monroe County. • Infusion Pharmacy for IV therapy in the home such as diuretics, Milrinone, and antibiotics. Our IV Lasix treatment plan is also available. ** • Clinically expert care of LVAD patients. • Collaboration and close communication with you, and the other health care team members for optimal outcomes and disease management. • Rehab services (P.T, O.T, S.T) to build endurance and strength while improving overall functioning and safety in the home. • Social Work to assist with linking to community resources and long term planning for your patients’ ongoing needs.
