Requirements of the Role Must be able to provide support and give advice to patients, relatives and carers by providing individualised education and exercise

programmes.

Will be a source of expertise within their field supporting the establishment of a comprehensive cardiac rehabilitation service.

Supporting clinical governance by contributing to audit and research.

Who will liaise with all members of the team and also with secondary and tertiary care providers.

Shaping the future delivery of rehabilitation (across Greenwich & Bexley)

Monitoring and refining quality of care in line with the BACPR guidelines

Provide expertise, clinical leadership, facilitation change management, by forms of

collaboration education, innovation, research and practice development.

► Ability to critique research and contribute to clinical practice to improve outcomes for cardiac patients.

► Ability to monitor standards through the process of clinical audit in adherence with the Trust’s objectives.

► Uphold safe practice taking into account risk management and clinical incidents.

► Ability to identify, assess, monitor risks associated with service delivery.

► Make clinical decisions regarding the suitability and eligibility of each patient’s exercise programme.

► Conduct appropriate submaximal tests, to provide a baselines assessment and inform exercise programming.

► Exercise prescription and risk stratification.

► To keep up to date with evidence based knowledge and recognised best practice. ► Fostering a client centred approach and education.

► Be able to teach and aid the development of a multidisciplinary team in the area of cardiac rehabilitation.
