1- Q- Who is Companion Life Insurance Company (CL)? A- CL is a wholly owned subsidiary of Blue Cross & Blue Shield of South Carolina, and is licensed in 46 states and the District of Columbia. 2- Q- What products are offered through CL? A- CL offers a portfolio of group and voluntary products including Term Life, Accidental Death & Dismemberment (AD&D), Short Term Disability (STD) and Long Term Disability (LTD). 3- Q- What is the A.M. Best rating of CL? A - CL was assigned a Best’s Financial Strength Rating of A+ (Superior) for the 10th consecutive year. The company continues to generate favorable sales results. A.M. Best believes that CL can continue to recognize favorable, profitable growth in the future. 4- Q - Describe the CL distribution system? A - CL contracts with General Agents and several Health Partners throughout the United States. 5- Q - Where is the Home Office located? A - Columbia, South Carolina. Regional offices are located in Virginia, Maryland, Tennessee, Ohio, Wisconsin, Rhode Island, Kansas and Arizona. 6- Q - Generally describe your Group and Voluntary Life programs? A - CL group Life/AD&D Trust program provides plans for 2-9 employer groups. The plan has guarantee issue, competitive age band rates and flexible plan designs. CL 10+ True group Life/AD&D program provides custom matched benefits, composite rating, custom guarantee issue amounts and two year rate guarantees. CL Voluntary Life is a freestanding program for groups with as few as five enrollees. High Guarantee Issue amounts depending on the level of employee participation and group size. The program provides a spouse and child(ren) benefit. It also has an AD&D option. All programs have an ineligible list of industries. Brochures and forms are available on our website.

7- Q - Generally describe your Group and Voluntary STD program? A - CL provides disability income to employees who become sick or injured off the job to replace part of their lost income on a short term basis. CL group STD Trust program provides plans for 2-9 employer groups. The plan has guarantee issue, competitive age band rates and flexible benefit designs. CL 10+ True group STD provides custom matched plans, composite rates, guarantee issue, two year rate guarantees and can be freestanding. CL Voluntary STD is a freestanding program for groups as few as 3 employees. Employee participation levels are 3 enrolled for 3-9 groups and 5 enrolled for 10+ groups. Employee may elect a weekly benefit up to $750. Several elimination periods are available. Program includes a partial benefit and an age band entry freeze. All programs have a list of ineligible industries. Brochures and forms are available on our website. 8- Q- Generally describe your Group and Voluntary LTD program? A- CL provides disability income to employees who become sick or injured to replace part of their monthly income on a long term basis to age 65. CL group LTD Trust program provides plans for 2-9 employer groups. The plan has several choices of elimination periods, benefit durations and competitive age band rates. Guarantee issue is standard for 5+ employer groups. CL 10+ True group LTD provides custom matched plans, composite rates, guarantee issue, three year rate guarantees and can be freestanding. CL Voluntary LTD is a freestanding program for groups of 10 or more employees. Participation is the greater of 10 or 25% of the employees. Several benefit designs and elimination periods are available. Benefit maximum up to $10,000. EAP program included at no charge. All programs have a list of ineligible industries. Brochures and forms are available on our website. 9- Q- Does CL have any special programs available? A- CL has developed a flat rate package plan called “BROKER PLUS”. This plan is available to 2-49 employer groups. The plan includes a flat $15,000 basic Life and AD&D benefit and a $100 per week STD benefit. The employer cost is $12.00 per employee per month for the total package. Employees may purchase additional STD benefit amounts through payroll deductions. Age band rates apply for the additional coverage. All coverage is guaranteed issued. This program has a list of ineligible industries.

10 -Q- Describe your billing process? A- CL provides direct billing from our home office or your client may elect to to be billed by one of our broker administrators in Maryland. If CL provides billing services, additional features are available. “My Benefits Companion” provides group administrators the option to enroll new employees, view employee’s coverage, terminate employee’s coverage and request certificate of coverage. Employers can view, print current and prior bills, pay online, and view payment history.

11- Q - Describe your enrollment process? A - A master employer application is required with all products. Employee enrollment cards or a census is required . Beneficiary cards may be required if employer doesn’t maintain copies. A binder check is required on all employer paid products. CL has approved the broker administrators election forms and electronic enrollment process. All CL forms may be downloaded from our website. 11- Q - How are brokers compensated? A - CL offers competitive commission schedules. First year and renewals below. Trust 2-9 Life/AD&D and STD - 20% level Trust LTD - 15% level Voluntary Life/AD&D, STD, and LTD - 15% level True Group Life/AD&D, STD - 15% graded True Group LTD – 15% graded 12- Q – Can the small group product billing fee be waived? A – Yes, if the billing is being done by one of our approved Broker Administrators. 13 - Q - How do we request a proposal? Companion), but such payment shall be subject to and contingent upon (a) continuance of the Agent as the Agent of Record(as accepted by Companion); (b) continued reasonable servicing of the policyholders and cooperation with Companion bythe Agent; (c) applicable laws or rulings of lnsurance Departments; (d) compliance by the Agent with the reasonable rules

and regulations of Companion; and (e) commission schedules remaining in effect with Companion Life lnsurance Company.

It is the agent's responsibility to review each commission statement for accuracy. ln no event will Companion Life pay

commissions retroactively more than six months. Agent of record changes will not be effective until acknowledged and

approved in writing by Companion Life.

lf, because of cancellation of a policy, or for any other reason, any premium or premiums paid upon a policy are returned, the
