We expect to holdour Kirkwood “open house” in January and willpublicize the date, once our staff members andpatients are “back in business” in Kirkwood’s six-bed patient care unit.

As we come to the conclusion of an eventful 2010,all of us at Caldwell Hospice and Palliative Care,including the 26 new employees we’ve added thisyear, thank this community, the patients andfamilies we have served, as well as the individuals,churches, and groups who continue to support usin providing quality end-of-life care.

Caldwell Hospice and Palliative Care Board of Directors

The Caldwell Hospice and Palliative Care “to do” listhas grown and multiplied since we began planningfor the Robbins Center facilities and broke ground inNovember 2008. On August 12, 2010, we realized along-anticipated dream when we included our firstresidential patient among the hospice patientsmoving in to our state-of-the-art Forlines PatientCare Unit.

It is safe to say that everyone felt the excitementthat day, as we put our moving plans into action.Caldwell County’s EMS team was both professionallyand personally attentive to our needs, and the onlydelays occurred when ambulances were called awayto emergencies. Once we had completed the movefrom Kirkwood to the Robbins Center for ourpatients, we could focus on “small” details: helpingpatients figure out how to use the remote controlsfor their TVs, purchasing clothes hangers for theirclosets, ordering pizzas for our exhausted staffmembers, and so forth.

The big details that followed the move-out day atKirkwood focused on renovation. We’re includingbefore-and-after photos in this issue of Carelines, soyou can see what we’re doing.

The hardwood floors have been refinished. We haverefurbished the family kitchen, including newflooring and a new paint color; installed ceiling fansin all the patient rooms, along with recessed lightingand wall-mounted televisions;re-laid the brick pavers in the

Medical technology has advanced so quickly that,sometimes, we are not sure when or if to put itinto practice. It is common to worry about themoral and ethical outcomes of end-of-life caredecisions, especially. For as long as individuals arecompetent, decisions regarding medical treatmentare theirs to make. What happens when they losethat ability? Who decides on the treatments theywill receive and under what circumstances orconditions changes will be made, etc.?

Physicians try to educate patients and familymembers regarding treatment goals and possibleburdens that result, but individuals have their ownspecific circumstances and beliefs regarding whatan acceptable quality of life is. Do YOU have awritten plan that states the kind(s) of health caretreatments you would want (or NOT want) if youhad a sudden accident or illness that left youunable to express your decisions? Would you wantto give up your ability to participate in makingserious decisions about your own treatment? Wouldyour loved ones be comfortable with makingdecisions for you? Would they know what yourpreferences were?

If we wait as long as possible before making end-of-life decisions, we risk waiting too long. Pleasereview the following information. The CaldwellPartnership for End-of-Life Care (754.0101) orCaring Connections (www.caringinfo.org) can helpto answer your questions.

The Partnership encourages people to completeadvance directives (living wills and health carepowers of attorney). The living will is a legaldocument that allows you to express your wishesabout life-sustaining treatments, if you becomeincapacitated. The health care power ofattorney allows you to appoint a person you trustto make your health care decisions, if you cannotmake them yourself.

The Living Will (Advance Directive for a NaturalDeath) allows you to express your wishes aboutlife-sustaining treatment, should you becometerminally or irreversibly ill. In a living will, youstate that you do not want your life to beprolonged by extraordinary means if you have aterminal, incurable condition or are in a persistentvegetative state.

A living will is simple to fill out and, in NorthCarolina, it requires the signature of two witnesses(not related to you) and a notary public. A livingwill becomes effective when you can no longerdirect your health care and are not expected torecover.

The Health Care Powerof Attorney (HCPOA,or Medical Power ofAttorney, Health CareProxy or Agent, HealthCare Surrogate,Durable Power ofAttorney for HealthCare): This legal form identifies who YOU want tomake the decisions about your medical care, ONLYif you are unable to make your own medicaldecisions. Your agent can be any adult you choose,EXCEPT your health care provider.

Your agent has the same authority you would haveto hire and fire health care providers; admit ordischarge you from a health care institution;consent to or refuse tests or treatments, and more.

After you make your decisions and fill out thesedocuments, do NOT keep them in a safe-depositbox at your bank because you may not be able toget them in a crisis. Give copies to your doctor,your hospital (if you have a record on file), yourhealth care agent(s), your family member(s), andyourself (keep your copies in an easy-to-findplace).

Be sure to talk about your decisions with yourfamily members—especially the person who will beyour HCPOA—clearly and honestly before youdecide. Be sure that your HCPOA will do what youwant if you are unable to choose for yourself. Thebottom line is for individuals to make their owndecisions concerning end-of-life issues, expressthese wishes to their family members, and—mostimportantly—put the decisions in writing.

Often, when we talk about advance directives,people ask how a Do Not Attempt ResuscitationOrder (DNR) fits into this planning. The Do NotAttempt Resuscitation Order is a physician’s orderthat states the patient does not wish to receiveCPR. The North Carolina General Assembly hasexpanded these orders to include other life-prolonging treatments. Called MOST (MedicalOrders for Scope of Treatments), the new ordersgive patients a choice of whether or not to receiveCPR, the medical intervention they do wish toreceive, such as the wish to receive antibiotics,medically administered fluids, and nutrition. MOSTis a doctor’s order and must be reviewed annually.

Despite all the medical documents, it is extremelyimportant that your HCPOA and family membersknow exactly what your desires are for your end-of-life care, regardless of their wants and wishes foryou.

A New Caldwell Hospice Volunteer Program and More

Caldwell Hospice Volunteer Coordinator DawnCannon and Caldwell County Veterans Honor Guard(CCVHG) commanding officer Bob Scherer, who alsoserves as a Caldwell Hospice volunteer, worked fornearly six months to develop a meaningful way torecognize our patients who are veterans of militaryservice.

The new program, Veterans Honoring Veterans, wasintroduced to Caldwell Hospice staff and volunteersat a Robbins Center breakfast in early October. Theprogram included a “presentation of colors,” theplaying of “Taps,” and an explanation of how therecognition service will be presented to patients.

When patients are admitted to Caldwell Hospice,their social workers will ask them or their primarycaregiver about military service, including branch ofservice. A military veteran volunteer will beassigned, if available. Service information will beincluded in those patients’ charts, etc.

Volunteer Coordinator Dawn Cannon will arrangewith the veterans group, the patients, and theirfamilies a time to meet—it can be informal or theycan invite their families to be present, if they wish.They will receive a certificate identifying theirservice branch, an American flag lapel pin, and asmall American flag.

Caldwell Hospice maintain a strong connection withthe area churches. At the Ambassador volunteers’quarterly lunch meeting, Tony Deal and MikeBenson provided the music. Caldwell HospiceChaplain Nancy Campbell, Volunteer CoordinatorDawn Cannon, and Volunteer Support SpecialistCrystal Burch provided assignments for the nextquarter, and everyone enjoyed a delicious meal.

Ambassador volunteers serve as liaisons betweentheir churches and Caldwell Hospice. They gothrough a special volunteer training program, meetwith us quarterly, and inform their congregationsabout upcoming programs (I Just Want to Help,etc.), special events (our annual memorial service,for example), and special needs (Wish List items,etc.). The Art Cart. Caldwell Hospice received a specialdonation from Shirley Story and the Brush &Palette Club recently. The art cart, given inmemory of Shirley's mother, contains a widevariety of arts and crafts supplies for the use ofour patients, their families, and their children.

Trained volunteers in the Forlines Patient Care Unitat the Robbins Center wheel the art cart intopatients’ rooms when they visit with patients andfamilies. Responses have been positive. During thefirst week, a patient mentioned her desire for asketch pad and pencil. When the volunteerreturned with a pad and pencil, the patientproclaimed it "the best day ever!"

“I just love taking the art cart to patients,” saysCaldwell Hospice volunteer Linda Watson. “Patientsenjoy it, whether they are creating crafts orinstructing me in making crafts for them.” Lindahas helped patients make bracelets, necklaces, andgift cards. They seem especially interested in the“letter beads,” she says. “Patients like having theirnames on the crafts they make” for themselves or

for their loved ones.

The Brush & Palette Club will keep the art cartstocked with arts and crafts supplies.

The Activities Cart. Patients who may notconsider themselves to be “crafty” will have thebenefit of another recent gift for Caldwell Hospice.The "activities cart" is stocked to overflowing withgames, puzzles, etc., including Dominoes, Monoply,Scrabble, checkers, playing cards, crosswordpuzzles, and Sudoko.

Volunteers are happy to take the activities cart topatients' rooms, the family living room, and thesun rooms in the Forlines PCU.

The cart was stocked and donated by volunteerBarbara Jones in memory of her husband, BillJones, who died on the PCU at Kirkwood.

"Bill was a loving, caring person," Barbara says,"and he would be tickled to know that families,during hard emotional times, can find relief andenjoyment” when a Caldwell Hospice volunteerwheels the activities cart their way.

Getting through theholiday season can bedifficult for manypeople, but for thosewho are living with theloss of a loved one, itcan be especially hard.

For many families, theholidays call attentionto the absence of theperson they have lostor someone who willnot be with them nextyear. Finding a copingstrategy before theholiday season canhelp. The followingsuggestions cannottake away all the grief, but during a time of theyear when everyone around you is full of holidaycheer, you may find some solace in them:

Be patient and realistic. Plan ahead so that youare not overwhelmed by responsibilities at the lastmoment. When you are grieving, it is difficult tomake decisions, so make lists. Set priorities.Decide what is important to you this holidayseason.

Acknowledge your limits. Become aware of yourneeds and express them to family and friends.

Have a family meeting. Decide together what'simportant to each of you, what you want to do thisyear, what you can let go of, and what you can dodifferently.

Remember: it is okay to say “no.” Don’t feelobligated to accept every invitation that comesyour way. Do what you can this holiday season,and let that be enough. Don’t try to tackle all thedecorations, entertaining, and cooking.

Incorporate your loved one into the holidays.Keep traditions alive in ways that bring comfort.Encourage the family members to share memoriesof how the person who died contributed to makingthe holidays special, while creating an atmospherethat welcomes discussion and supports thememories of what made that person so special.

Help others. Caring for and giving to others willnourish you, sustain you, and help you to feelbetter about yourself.

• Take a meal to a homebound couple. • Volunteer in a shelter or soup kitchen. • "Adopt" a family and buy presents or food for.

You may be facing the holidays with dread, but ifyou can plan and allow time for the grief andshared memories, you might find joy and laughterin the midst of sadness and tears.

Caldwell Hospice ishappy to introduce ournew counselor, JessicaHatton. She left herhometown of Winston-Salem, NC, to completeher undergraduatestudies at AppalachianState University,earning a Bachelor ofScience degree inSociology and Religion.

During Jessica’s sopho-more year at ASU, an introductory Sociologycourse and the professor who taught it inspired herinterest in counseling as a career choice. Throughcounseling, she says, she can help people duringtimes of crisis and the turning points in their lives.

After graduating from ASU, Jessica moved toDallas, TX. She lived there for four years andearned her Master’s degree in Counseling fromDallas Theological Seminary. The young man shehad begun dating at ASU moved to Texas to workon his Master’s degree, as well. They married whilein graduate school and returned to North Carolina.

Jessica’s professional experience includes workingin churches and in private practices, with specialattention to anxiety and depression, women'sissues, and marriage counseling. She has hadaddiction training, as well.

Free time for Jessica includes reading, learning newthings, gardening, cooking, watching football,working out, and spending time outdoors.

Please join us in welcoming Jessica and hercounseling skills to Caldwell Hospice.

Volunteer Spotlight: Marc Carpenter

The first things many people notice when theymeet Marc Carpenter are his kind smile and gentledemeanor. These are desirable traits in a personwho has spent 30 years in human resources atBroyhill Furniture Industries and who has served asa Caldwell Hospice and Palliative Care boardmember for more than 18 years.

Marc Carpenter was invited to join CaldwellHospice’s board when Tony Deal, who was on theboard at that time and who Marc knew fromCCC&TI, explained that the board would benefit,when making personnel decisions, from someonewith human resources experience. Marc knew verylittle about hospice programs, but what he hadheard about Caldwell Hospice was positive. He wasflattered to be considered, he says.

Over the past 18 years, Marc has enjoyed partici-pating on the personnel committee, seeing CaldwellHospice’s growth, and contributing to this specialorganization. He says it has never felt burdensomebecause what he gives is more than balanced bywhat he gets: “If I can contribute something, I’mglad to do it.”

Marc identifies the most exciting part of his boardexperience so far as the board’s decision topurchase land and expand Caldwell Hospice’s facili-ties to include the 12-bed patient care unit at theRobbins Center. He says the board looked closely atthe figures, as the census for Kirkwood’s six-bedpatient care unit increased from 5.5 to 6.1, whichmeant that as soon as a bed became available, itwas filled. “Are we not meeting the community’sneeds any more?” was the big question thatneeded to be answered.

With the additional facility in Hudson, Marc saysthe board is confident that “we serve the countybetter.”

Marc is a Caldwell County native. He grew up on asmall farm on Collettsville Road, attended LenoirHigh School, and CCC&TI, where he met his wife,the former Bennie Wilson from Hudson, whosefather Boyd ran the hardware store in Hudson.

Marc earned a bachelor’s degree in businessadministration and marketing from AppalachianState University, and he served in the NorthCarolina National Guard/US Army Reserve, havinggraduated as a commissioned second lieutenantfrom the NC Military Academy.

Another volunteer commitment close to Marc’sheart is the Community in Schools program. Hebelieves firmly that the genuine encouragementoffered by a caring adult can be the difference foran at-risk young person.

In his spare time, Marc enjoys yardwork, frommowing to working in flower beds to pruningduring winter months. He reads everything: HarryPotter books, a biography of Stonewall Jackson,books related to his job, classics such as TheInvisible Man and Moby Dick, John Grisham novels,and The Hobbit. It is not surprising that Marcenjoys woodworking, given his years in the furni-ture industry. He has made a few pieces of furni-ture and does projects around the house, includingtrays that slide out of cabinets.

Marc’s essential rule is that “I will not lie to anyemployee.” Believing in integrity, he has tried to behonest and sincere in dealing with people for threedecades, with employees at Broyhill, with theteenagers in Sunday school classes he has taught,and in conversations with his daughters, both ofwhom are now grown and successful in theirprofessional and personal lives.
