1. Introdução
A descrição das doenças ou lesões, assim
como dos seus determinantes, constitui um
passo vital para a tomada de decisão e
planeamento em saúde. Para a
implementação de políticas e estratégias que
procurem melhorar o estado de saúde da
população e a alocação de recursos, é
importante conhecer não apenas a carga das
doenças, mas também a carga de doença que
é atribuível aos vários fatores de risco que as
originam (como, por exemplo o sexo não
protegido, o tabagismo, e a dieta).
A avaliação de risco comparativo
(Comparative risk assessment - CRA) é uma
avaliação sistemática das mudanças na saúde
da população que resultam da alteração da
distribuição da exposição a um fator de risco
(ou a um grupo de fatores de risco) em
relação a outros fatores de risco. É diferente
da análise da intervenção, que é a avaliação
dos benefícios para a saúde de uma
intervenção bem definida, embora as duas
possam utilizar métodos semelhantes.
Em 1991 a OMS (Organização Mundial de
Saúde) deu início ao projecto “Global Burden
of Disease and risks factors”, que disponibiliza
estimativas da carga da doença, morbilidade e
mortalidade, que é atribuível a 24 fatores de
risco selecionados, para as diferentes regiões
e países do mundo inteiro. Estes estudos da
carga global da doença (CGD) têm como
objetivo quantificar a carga de mortalidade
prematura e de incapacidade para as
principais doenças ou grupos de doenças,
usando como medida resumo da saúde da
população o DALY (Disability Adjusted Life
Year). Esta medida combina as estimativas
dos anos de vida perdidos por morte
prematura (YLL, Years of Life Lost) e dos
anos de vida perdidos por doença e/ou
1. Introduction
A description of diseases and injuries and
the risk factors that cause them is vital for
health decision-making and planning.
In order to implement strategies and
policies which will improve population
health and resources allocation, it is
important to know not only the disease
burden, but also the burden attributable to
various underlying risk factors that cause
disease and injuries (such as smoking,
unsafe sex and diet).
Comparative risk assessment (CRA) is a
systematic evaluation of changes in
population health that result from changing
exposure distribution to a risk factor (or a
group of risk factors) in relation to other
risk factors. This is different from
intervention analysis, which is the
evaluation of the health benefits of a well
defined intervention, although the two may
use similar methods.
In 1991 the World Health Organization
(WHO) began de project “the Global
Burden of Disease and risks factors”,
which provides estimates for the burden of
disease, morbidity and mortality,
attributable to 24 risk factors selected
worldwide, for the different regions and
countries. Global burden of disease (GBD)
studies aim to quantify the burden of
premature death and disability for the main
illnesses or groups of illnesses, using as a
summary measure of population health the
DALY (Disability Adjusted Life Year). This
health measure combines the estimates of
years of life lost due to premature death
(YLL, Years of Life Lost) with the years of
life lost due to illness and/or incapacity
(YLD, Years Lived with Disability).

incapacidade (YLD, Years Lived with
Disability). Assim, um DALY corresponde a
um ano de vida saudável perdido. Para
quantificar a carga de doença atribuível a
fatores de risco, a OMS utilizou informações
atualizadas provenientes dos seus programas
e de vários estudos científicos. Aplica-se essa
análise de risco às últimas estimativas
regionais de mortalidade e morbilidade de um
conjunto abrangente de doenças e lesões
para o ano de 2004.
Este estudo, para a região Norte de Portugal,
pretende contribuir para o reforço da base de
evidência para a tomada de decisão e
planeamento em saúde. Utilizou-se uma
metodologia similar à que foi desenvolvida
para o estudo Global Burden of Disease 2004
(GBD) publicado pela OMS. No cálculo do
YLL usaram-se dados de mortalidade da
região Norte. Os valores do YLD foram
estimados a partir dos dados publicados pela
OMS para Portugal. Com este estudo
pretende-se estimar a carga de doença que é
atribuível a um determinado fator de risco, ou
seja, que é resultado da exposição a esse
risco.
O que é um factor de risco?
Para prevenir doenças e lesões, é necessário
identificar e lidar com suas causas - os riscos
de saúde que lhes estão subjacentes. Embora
haja muitas definições possíveis de "risco
para a saúde", este é definido como "um fator
que aumenta a probabilidade de efeitos
adversos à saúde"[1]
. O número desses
fatores é incontável e este estudo não
pretende abordar todos, mas antes centrar-se
em alguns fatores de risco selecionados, para
os quais existem dados disponíveis para
estimar as exposições ou distribuições da
população ou a sua distribuição, e para os
quais são conhecidos os meios de os reduzir.
Thus, one DALY represents one year of
healthy life lost. To estimate the burden of
disease attributable to risk factors, WHO
uses updated information from its
programs and scientific studies for both
exposure data and the causal associations
of risk exposure to disease and injury
outcomes. This risk analysis applies these
updated risk analyses to the latest regional
mortality and morbidity estimates, for a
comprehensive group of illness and
injuries for the year 2004.
This study, for the Northern Region of
Portugal, aimed to strengthen the evidence
base for health decision making and
planning.
The methodology used in this study is
similar to the one used on the GBD-2004
study published by the World Health
Organization (WHO). Northern Region
mortality data were used for the estimation
of YLL. Data published for Portugal by the
WHO were used to estimate the values of
YLD for the Northern Region. This study
aims to estimate the burden of disease and
injury attributable to a certain factor,
resulting from exposure to that risk
What is a risk factor?
To prevent disease and injury, it is
necessary to identify and deal with their
causes – the health risks that underlie
them. Although there are many possible
definitions of “health risk”, it is defined as
“a factor that raises the probability of
adverse health outcomes”[1]
. The number
of such factors is countless and this study
does not attempt to be comprehensive. It
focuses on selected risk factors which
have global spread, for which data are
available to estimate population exposures
or distributions, and for which the means to
reduce them are known.

À medida que um país se desenvolve, os tipos
de doença que afetam a população alteram-
se, das doenças transmissíveis (por exemplo
diarreia e pneumonia) para doenças não
transmissíveis (por exemplo doenças
cardiovasculares e neoplasias malignas). Esta
evolução deve-se a melhores condições dos
cuidados médicos, ao envelhecimento da
população e às intervenções de saúde pública
(como a vacinação, medidas sanitárias), que
reduz a incidência das doenças
transmissíveis.
Da mesma forma, os fatores de risco que
afetam as populações também variam à
medida que o país se desenvolve. Os países
com baixo rendimento são mais afetados por
fatores de risco “tradicionais” ou mais
associados à pobreza, como a má nutrição,
sexo não protegido, água não potável, más
condições sanitárias. Nos países mais
desenvolvidos, e à medida que a esperança
de vida aumenta e a morbilidade se altera no
sentido das doenças crónicas e não
transmissíveis, as populações vão
enfrentando outros riscos, como sejam a
inatividade física, o excesso de peso e a
obesidade, tabaco e álcool. Os países de
médio rendimento enfrentam um crescimento
dos fatores de risco modernos, enquanto que
ainda lutam contra os fatores de risco
tradicionais. [2]
The risk transition
As a country develops, the types of
diseases that affect its population shift
from communicable diseases (such as
diarrhea, pneumonia) to noncommunicable
diseases (such as cardiovascular disease
and cancer). This shift is caused by
improvements in medical care, to the
ageing of the population and public health
interventions (such as vaccination, access
to clean water and sanitation), which
reduce the incidence of communicable
diseases.
Similarly, the risks that affect a population
also shift as a country develops. Low-
income countries are most affected by
“traditional risks” associated with poverty,
such as undernutrition, unsafe sex, unsafe
water, poor sanitation and hygiene. In the
most developed countries, as life
expectancy increases increase and the
major causes of death and disability shift to
chronic and noncommunicable diseases,
populations face other risks such as
physical inactivity, overweight and obesity,
tobacco and alcohol. Middle-income
countries face a growing burden from the
modern risks to health, while still fighting
an unfinished battle with the traditional
risks to health. [2]

associados a várias doenças ou causas e,
atuando nestes fatores, podemos reduzir
múltiplas doenças (por exemplo, reduzir o

A particular disease or injury is often
caused by more than one risk factor, which
means that multiple interventions are
available to target each of these risks. On
the other hand, most risk factors are
associated with more than one disease,
and targeting those factors can reduce
multiple causes of disease (for example,

consumo de tabaco vai resultar em menos
óbitos e doença por tumor maligno do pulmão,
doença isquémica do coração, doenças
cerebrovasculares, doenças respiratórias
crónicas e outras).
Várias doenças são causadas por múltiplos
fatores de risco, que individualmente ou em
conjunto (combinados) podem ter impacto no
risco de doença. Como resultado, a carga de
doença atribuível a fatores individuais
sobrepõem-se e normalmente somam mais de
100%. Por exemplo, dois fatores de risco -
tabaco e poluição urbana- causam cancro do
pulmão. Algumas mortes por cancro do
pulmão são atribuídas a ambos os fatores de
risco, que atuam em conjunto- exposição
representada pela área onde os círculos se
sobrepõem (figura 1). Esta área de
sobreposição representa a percentagem de
mortes por cancro de pulmão em 2004 que
poderia ter sido evitada se ambas as
exposições, ao tabaco ou poluição
atmosférica urbana, fossem menores. As
mortes que seriam evitáveis pela remoção de
ambas as exposições, são representados pela
área onde os círculos se sobrepõem[1]
.

deaths and less disease from lung cancer,
heart disease, stroke, chronic respiratory
disease and other conditions).
Many diseases are caused by multiple risk
factors, and individual risk factors may
interact in their impact on the overall risk of
disease. As a result, attributable fractions
of deaths and burden for individual risk
factors usually overlap and often add up to
more than 100%. For example, two risk
factors – smoking and urban air pollution –
cause lung cancer. Some lung cancer
deaths are attributed to more than one
exposure – represented by the area where
the circles overlap. This overlapping area
represents the percentage of lung cancer
deaths in 2004 that could have been
averted if either tobacco exposure or urban
air pollution had been lower. Deaths that
would have been prevented by removing
either exposure are represented by the
area where the inner circles overlap.

Além disso, cada fator de risco tem as suas
causas, numa complexa cadeia de eventos ao
longo do tempo, consistindo em fatores
socioeconómicos, ambientais e individuais.
Como pode ser visto no caso da Doença
Isquémica do Coração (figura 2), alguns
fatores de risco, tais como a tensão arterial ou
colesterol elevados, ou colesterol, atuam
como causa direta da doença.
Alguns fatores de risco estão localizados a
montante desta cadeia, atuando indiretamente
através de fatores intermediários, como sejam
a inatividade física, álcool, tabaco e a ingestão
de gorduras. Existem fatores ainda mais
distantes como sejam a educação e o
rendimento, aos quais é mais difícil de atribuir
causalidade.[1]
Each risk has its own causes too, and many
have their roots in a complex chain of events
over time, consisting of socioeconomic factors,
environmental and individual.
As can be seen from the example of
ischaemic heart disease (figure 2), some
elements in the chain, such as high blood
pressure or cholesterol, act as a relatively
direct cause of the disease.
Some risks located further back in the causal
chain act indirectly through intermediary
factors - physical inactivity, alcohol, smoking
or fat intake. For the most distal risk factors,
such as education and income, less causal
certainty can be attributed to each risk.[1]

Medir o impacto do risco
A carga de doença atribuível a um fator de
risco é calculada estimando a fração
atribuível na população (PAF), ou seja a
proporção da redução na mortalidade ou
morbilidade que iria ocorrer se a exposição a
um fator de risco fosse reduzida para um
cenário ideal alternativo, que minimiza o risco
ou que levaria a incidência de doença para
níveis mais baixos[1],[3],[4]
. Para alguns fatores
de risco, a exposição ideal é clara (no caso do
tabaco, o consumo zero seria o ideal); noutros
casos não é tão simples estabelecer o nível
de exposição ideal.
A mortalidade (YLLs), morbilidade (YLDs) ou
carga de doença (DALYs) atribuível a um
factor de risco é quantificada aplicando a
fracção atribuível à carga total da doença:
2. Methodology
Measuring impact of risk
The burden of disease attributable to a risk
factor was calculated by estimating the
population attributable fraction (PAF);
or the proportional reduction in population
disease or mortality that would occur if
exposure to a risk factor were reduced to
an alternative ideal exposure scenario, that
minimizes risk or that would lead to the
lowest levels of disease burden[1],[3],[4]
.. For
some risk factors, the ideal exposure level
is clear (for example, zero tobacco use is
the ideal); in other cases, the ideal level of
exposure is less clear.
The attributable mortality(YLL), incidence
(YLD) or burden of disease (DALYs) due to
the risk factor is quantified applying the
attributable fraction to the total burden of
disease:

Uma vez estabelecida a fração atribuível de
uma doença (ou lesão) para um fator de risco,
a carga atribuível (AB) obtém-se multiplicando
as estimativas totais de DALY para a doença
pela fração atribuível.

estimates for the disease and the

Dados
Neste estudo os valores do YLL, YLD e DALY
foram obtidos a partir do primeiro estudo
“Carga Global da Doença na região Norte
de Portugal”, publicado em Abril de 2011.
Os valores de YLL foram calculados com base
nos dados de mortalidade da região Norte
produzidos pelo INE, nos anos de 2003 a
2005. Os dados de YLD foram estimados
indiretamente a partir das últimas estimativas
para Portugal produzidas pela OMS. Assumiu-
se, assim, que a distribuição da morbilidade e
dos anos de vida perdidos por incapacidade
(YLD) na região Norte é idêntica à de
Portugal, de acordo com as últimas
estimativas produzidas, em 2008, pela OMS.
Ao YLL e YLD é aplicada uma taxa de
desconto de 3% para que os benefícios de
saúde presentes sejam mais valorizados do
que os benefícios futuros, isto é, o valor de
um ano de vida é reduzido, anualmente, por
uma percentagem fixa. Esta prática tem sido
muito comum nos estudos efetuados na área
da economia da saúde. Também foi aplicada
uma função matemática que atribui um maior
peso aos anos de vida perdidos por jovens
adultos e menor peso às crianças e aos
idosos.
Neste estudo utilizaram-se as frações
atribuíveis estimadas pela OMS para Portugal
para cada causa, sexo e grupo etário. Essas
frações atribuíveis para cada fator de risco
foram estimadas com base em estudos
existentes[1]
.
A principal limitação do estudo é o acesso a
dados de morbidade fiáveis para a região
Norte de Portugal.
Data
In this study, the YLL, YLD e DALY values
were obtained from the study Global
Burden of Disease in the Northern Region
of Portugal, published in April 2011.
The YLL values are calculated using the
Northern region mortality data produced by
the Statistics National Institute (INE), for
the years 2003 to 2005. The YLD data was
indirectly estimated using data from
Portugal, according to the latest estimates
produced by WHO. We assume, then, that
the morbidity and years lived with disability
(YLD) in the Northern region has the same
distribution as in Portugal.
In calculating YLD and YLL, a discount
rate of 3 percent is applied based on the
concept that resent health benefits should
be more valued than future benefits (the
value of a year of life is reduced annually
by a fixed percentage). This practice has
been commonly used in studies conducted
in the area of health economics.
We also applied a mathematical function
which values more the years of life lost by
young adults than the years of life lost by
children and old people.
In this study, we use the attributable
fractions estimated for Portugal by WHO,
for each cause, sex and age groups.
Those attributable fractions for each risk
factor were estimated based in existent
studies[1]
.
Its main limitation is the access to reliable
morbidity data for the Northern Region of
Portugal

3. Resultados
A próxima tabela resume a carga total de
doença, em DALY, associada aos principais
fatores de risco estudados. Como já foi
referido na nota metodológica, várias doenças
são causadas por mais do que um fator de
risco, que, atuando individualmente ou em
conjunto, podem ter impacto no risco de
doença. Por esta razão, as cargas de doença
que são atribuíveis a cada fator individual
sobrepõem-se e usualmente somam mais de
100%.
3. Results
The next table summarizes the total burden
of disease, in DALYs, associated with the
risk factors analyzed. As mentioned in the
methodology, various diseases are caused
by more than one risk factor, which, acting
singly or together, can have an impact on
disease risk. For this reason, the burden of
disease attributable to each individual factor,
usually overlap and add up to 100%.

O tabagismo, com 11,4 % da carga total de
doença, é o fator de risco responsável pela
maior quantidade de anos de vida saudável
perdidos na região Norte, sendo a 1ª causa nos
homens (com 16,8% do total de DALY) e a 3ª
causa nas mulheres (com 5,1% do total de
DALY).
Em seguida, surge a tensão arterial elevada,
responsável por 8,1% do total de DALY (terceira
causa nos homens com 8% do total de DALY e

Next overall is high blood pressure,
responsible for 8.1% of total DALYs (3rd
cause in men with 8% of total DALYs and

primeira causa nas mulheres, com 8,2%).
O consumo de álcool surge em terceiro lugar,
sendo responsável por 7,6% do total de DALY.
Contudo, a carga de doença atribuível a este
fator de risco nos homens (10,8% do total de
DALY) constitui mais do dobro da observada
nas mulheres (3,8%).
Na lista dos principais fatores de risco
estudados seguem-se, em 4º e 5º lugares,
respetivamente, o IMC elevado (6,6% do total
de DALY, 5,9% em homens e 7,5% nas
mulheres) e o colesterol elevado (4,7% em
homens e 3,7% nas mulheres).
O consumo de drogas (2,8% do total de DALY),
a inatividade física (2,4% do total), o baixo
consumo de fruta e legumes (1,6% do total) e o
sexo desprotegido (21,4% do total) surgem
entre os 10 primeiros fatores de risco,
responsáveis por mais anos de vida saudável
perdidos.
A deficiência de ferro é responsável por 0,6%
do total de DALY.
A mortalidade é a principal componente da
carga de doença, em DALY, atribuível ao
tabaco. O mesmo se verifica para a tensão
arterial elevada, IMC elevado, colesterol
elevado, inatividade física, baixo consumo de
fruta e legumes e sexo desprotegido. As
doenças que esses fatores de risco causam são
caracterizadas por uma maior mortalidade. Ou
seja, para estes fatores de risco, o total de anos
de vida perdidos por mortalidade prematura
(YLL) é superior ao total de anos de vida
perdidos por doença ou incapacidade (YLD).
No caso do consumo de álcool e de drogas, a componente YLD, relacionada com doença ou incapacidade, é superior à componente YLL, ou seja, de anos de vida perdidos por morte prematura.

Iron deficiency accounts for 0,6 per cent of
the overall disease burden.
Mortality is the main component of the
burden of disease, in DALYs, attributable to
smoking.
The same applies to high blood pressure,
high BMI, high cholesterol, physical
inactivity, low fruit and vegetable intake and
unsafe sex. The diseases that these risk
factors cause are characterized by greater
mortality. For all these risk factors, the total
years of life lost due to premature mortality
(YLL) is greater than the total years of life
lost due to illness or disability (YLD).
In what concerns alcohol consumption and
illicit drugs, the YLD component, related to
illness or disability, is higher than the YLL
component, ie, the years of life lost due to
premature death.

3.1. Tabaco
Dos fatores de risco estudados, o tabaco é o
que está associado à maior carga de doença.
É responsável por 53.907 anos de vida
saudável perdidos (DALY) quer por morte
prematura, quer por doença ou incapacidade.
O grande grupo dos Tumores Malignos
representa 34% (18.103) do total de anos de
vida saudável perdidos devido ao tabaco,
seguindo-se as Doenças do Aparelho
Circulatório (23,3%) e as Doenças do
Aparelho Respiratório (21,3%).
Na análise por causa específica de morte ou
incapacidade, a Doença Pulmonar Obstrutiva
Crónica (DPOC) surge como a primeira causa
com mais anos de vida saudável perdidos
(DALY) devido ao consumo de tabaco,
sobretudo, devido à componente YLD. Em
seguida surgem as neoplasias malignas da
traqueia brônquios e pulmão, as doenças
cerebrovasculares e a doença isquémica
cardíaca, nas quais maior contributo para a
carga de doença vem da componente YLL.
3.1 Tobacco
Of the risk factors examined, tobacco is
associated with the greatest disease
burden. It is responsible for 53.907 years of
healthy life lost (DALYs).
Malignant neoplasms represent 34%
(18,103) of total years of healthy life lost due
to tobacco, followed by diseases of the
circulatory system (23,3%) and respiratory
diseases (21,3%).
In the analysis by specific cause of death or
disability, Chronic Obstructive Pulmonary
Disease (COPD) emerges as the leading
cause with more healthy life years lost
(DALYs) due to tobacco consumption,
mainly due to the YLD component. Next
come malignant neoplasms of the trachea,
bronchus and lung, cerebrovascular disease
and ischemic heart disease, in which the
largest contribution to the burden of disease
comes from the YLL component.

O grupo etário dos 45-59 anos é o que
apresenta o maior número de anos de vida
saudável perdidos, com 34,9% dos DALY
atribuíveis ao tabaco a ocorrer neste grupo
etário.
Cerca de 79% do total de DALY atribuíveis ao
tabaco, ocorrem no sexo masculino. A carga
de doença atribuível ao tabaco no sexo
masculino é quatro vezes superior ao sexo
feminino.
A maioria da carga de doença atribuível ao
tabaco é devida a mortalidade prematura.

3.2 Tensão arterial elevada
A tensão arterial elevada ou hipertensão
arterial é responsável por 38.150 anos de vida
saudáveis perdidos (DALY) na região Norte.
Cerca de 58% desses DALY correspondem às
doenças cerebrovasculares, e 25% à doença
isquémica do coração.

High blood pressure, or hypertension is
responsible for 38.150 years of healthy life
lost (DALYs) in the Northern Region.
Approximately 58% of DALYs correspond

A carga de doença atribuível à hipertensão é
ligeiramente mais elevada nos homens do que
nas mulheres (53% do total de DALY por este
fator de risco ocorrem no sexo masculino).

3.3 Álcool
O consumo de álcool surge como terceiro fator
de risco com maior número de anos de vida
saudável perdidos, com 35.878 DALY. É
responsável por conjunto de doenças e lesões,
entre as quais se salientam as perturbações
devidas ao uso de álcool (com 41,7% do total
de DALY), cirrose do fígado (18,9% do total de
DALY) e acidentes de trânsito (com 11,7%),
entre outras.
3.3 Alcohol
Alcohol consumption appears as the third
risk factor with the greatest number of
years of healthy life lost, with 35,878
DALYs. It is responsible for all different
diseases and injuries, among which stand
out alcohol disorders (41.7% of total
DALYs), liver cirrhosis (18.9% of total
DALYs) and traffic accidents (11.7%),
among others.

incluindo diabetes mellitus (com 41% do total
de DALY atribuível ao IMC elevado), doenças
cerebrovasculares (20%) e doença Isquémica

3.5 Colesterol elevado
O colesterol elevado surge em quinto lugar na
lista dos fatores de risco que mais contribuem
para a carga de doença na região Norte.
Cerca de 60% da carga de doença atribuível
ao colesterol elevado é por DIC e 40% por
doença cerebrovascular.

3.6 Consumo de drogas
O consumo de drogas surge como causa
direta de morbilidade, mas também como fator
de risco para doenças como HIV / SIDA,
envenenamento, suicídio e lesões
autoinfligidas. O maior peso vem da
dependência de drogas (desordens por uso
de drogas), que representa 62% do total de
DALY atribuível a este fator de risco.
3.6 Illicit drugs
Illicit drugs are a direct cause of morbidity,
as well as being risk factors for conditions
such as HIV/AIDS, poisoning, and suicide
and self-inflicted injuries.
The biggest burden comes from drug
dependence and harmful use, which
accounts for 62 per cent of DALY by this
risk factor.

O efeito adverso do consumo de drogas na
saúde ocorre principalmente nos jovens. A
proporção da carga de doença devida a
incapacidade é particularmente elevada nos
grupos etários compreendidos entre os 15 e
os 44 anos, e deve-se sobretudo à
componente YLD.

3.7 Inatividade física
A inatividade física é responsável por 2,4% do
total de DALY na região Norte, surgindo em
sétimo lugar na lista de causas.
Existe uma forte evidência de que a
inatividade física é fator de risco para a DIC,
doenças cerebrovascular e diabetes mellitus,
entre outras.
3.7 Physical inactivity
Physical inactivity accounts for 2.4% of
total DALYs in the North occupying the
seventh place in the list of causes.
There is strong evidence that physical
inactivity is a risk factor for CHD
cerebrovascular disease and diabetes,
amongst others.

3.8 Baixo consumo de fruta e
legumes
O baixo consumo de fruta e legumes é
responsável por 1,6% do total de DALY. Cerca
de 69% destes DALY, devem-se a anos de
vida saudável perdidos por doenças do
aparelho circulatório (doença isquémica do
coração e doenças cerebrovasculares) e os
restantes 31% a tumores malignos.

The low intake of fruits and vegetables is
responsible for 1.6% of total DALYs.

3.9 Sexo desprotegido
O VIH/Sida é responsável por quase 68% da
carga da doença atribuível ao sexo
desprotegido, que por sua vez é responsável
por 1,4% da carga total de doença. O tumor
maligno do colo do útero corresponde a 21%
da carga de doença atribuível ao sexo
desprotegido.
3.9 Unsafe sex
HIV/AIDS accounts for almost 68 per cent of the burden due to unsafe sex, which amounts to 0.4 per cent of the overall burden. Cervical cancer corresponds to 21 per cent of the disease burden attributable to unsafe sex.

3.10 Deficiência de ferro
A deficiência de ferro surge em 10º lugar na
lista dos fatores que mais contribuem para a
carga global de doença na região Norte, sendo
responsável por 0,6% do total de DALY.
A anemia é responsável por cerca de 98% da
carga atribuível a este fator de risco, quase
exclusivamente à custa da componente YLD.
3.10 Iron deficiency
Iron deficiency appears on th 10th place
amongst the factors that contribute most
to the burden of disease in the Northern
Region, accounting for 0.6% of total
DALYs. Anemia is responsible for about
98% of the burden attributable to this risk
factor, almost exclusively due to the YLD
component.
A deficiência de ferro afeta sobretudo as
mulheres e os grupos etários entre os 15 e 44
anos.
Iron deficiency affects mostly women and
the age groups between 15 and 44 years.

4. Conclusão e Discussão
Este trabalho dá continuidade ao estudo da
Carga Global de Doença na população da
região Norte, publicado em Abril de 2011, e
que pela primeira vez procurou identificar as
principais causas de doença e mortalidade
utilizando a metodologia DALY, que combina a
mortalidade (YLL) com a morbilidade (YLD),
permitindo ainda uma comparação com o país
e a região Europa A. Apesar de, neste estudo,
se usar dados de morbilidade estimados para
Portugal, foi possível utilizar dados de
mortalidade para a região Norte
O presente estudo dá um passo em frente,
uma vez que procura quantificar a carga de
doença associada a cada um dos fatores de
risco selecionados, utilizando a metodologia da
OMS[1]
. São, assim, destacados os fatores de
risco relacionados com os estilos de vida como
sejam o tabagismo, consumo de álcool,
inatividade física e sexo desprotegido, como
sendo responsáveis por uma proporção
assinalável do total da carga da doença na
região Norte. Confirma-se a importância dos
fatores de risco fisiológicos, como sejam a
obesidade, a hipertensão arterial e o colesterol
elevado.
O consumo de tabaco, com 11,4 % da carga
total de doença, é o fator de risco responsável
pela maior quantidade de anos de vida
saudável perdidos na região Norte, sendo a 1ª
causa nos homens (com 16,8% do total de
DALY) e a 3ª causa nas mulheres (com 5,1%
do total de DALY).
Em seguida, surgem a tensão arterial elevada (8,1% do total de DALY); o consumo de álcool (7,6%), o IMC elevado (6,6%) e o colesterol elevado (4,3%).
Estes resultados são consistentes com os
verificados nos países com elevado
rendimento per capita, sendo de destacar que
a carga de doença em DALY atribuível ao
consumo de tabaco foi de 10,7% em 2004.[1]
Ao quantificar a carga associada a uma série
de fatores de risco, este estudo fornece
4. Conclusion e discussion
This work continues the study of the Global Burden of Disease in the population of the North, published in April 2011 which first sought to identify the main causes of illness and mortality using the DALY methodology that combines mortality (YLL) with morbidity (YLD), while still allowing a comparison with the country and the European Region A. Though only Portugal-estimated data morbility was available, specific mortality data for the Northern Region was used in this study.
It takes a step forward, since it attempts to
quantify the burden of disease associated
with each of the selected risk factors using
the WHO methodology[1]
. This study
highlights the lifestyle risk factors, such as
tobacco smoking, alcohol consumption,
physical inactivity and unsafe sex, which
are responsible for a considerable
proportion of the total burden of disease in
the North Region. It also confirms the
importance of physiological risk factos,
such as obesity, high blood pressure and
high cholesterol.
Tobacco smoking, with 11.4% of the total
burden of disease, is the risk factor
responsible for the greatest amount of
healthy life years lost in the North, being
the 1st issue in men (16.8% of total DALY)
and the 3rd question in women (5.1% of
total DALYs).
Then come high blood pressure (8.1% of total DALYs), alcohol consumption (7.6%), increased BMI (6.6%) and high cholesterol (4.3%). These results are consistent with those
seen in countries with high income per
capita, where the burden of disease in
DALYs attributable to tobacco
consumption was 10.7% in 2004. [1]
By quantifying the burden associated with
a range of risk factors, this study provides

informações valiosas sobre as possibilidades
de ganhos em saúde nesta região de saúde, o
que ajudará a informar o planeamento em
saúde e a tomada de decisão.

atribuível a fatores de risco é o primeiro do seu
género realizado em Portugal.
