PaymentFull payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/AMEX/DISC). Make checks payable to the Carol Stream Park District; There is a $20 fee for returned checks.
Program CapacityAll programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another. If the program is cancelled, full credits will be issued.
Waiting ListsOnce the maximum enrollment is met for a class, a waiting list will be formed. We will create additional classes when feasible and you will be contacted. Credits will be processed after the first scheduled class.
InsuranceThe Park District does not carry medical or accident insurance for program participants or park patrons. Please review your personal health insurance plan to be certain that you and your family have proper coverage.
Financial AssistanceWe offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications are available at the Simkus Recreation Center.
Satisfaction Guaranteed!Our goal is your satisfaction! If you are not completely satisfied with any Carol Stream Park District program, class or event, please tell us! We will work to earn your satisfaction. If you are still not completely satisfied, we will transfer your registration to another program or session, or issue you a refund - that’s our guarantee!

Green HalloweenOnce Halloween is over, give your compost a boost with your old pumpkins, the straw you used to build scarecrows and any other organic material for great, nutrient rich soil in the spring.
Give Your Herbs ShelterIn late fall bring your summer herbs indoors to enjoy fresh basil, mint, and rosemary throughout the winter months. It takes a few weeks to transition herbs to indoor living, so start before the weather turns.
Go Fruit PickingGet some fresh produce, some fresh air, and support a local farmer or orchard with a day of pumpkin, berry or squash picking.

Love dogs? Sign up as a volunteer for our third annual CSBarks dog festival on Sept 18!. Over 500 dogs are expected to attend, and we need volunteers to help run games, supervise the off-leash and Rover Rest areas, take photos, or help with set up and take down.
Got a group? Volunteer opportunities are also available before and after the event with set up and break down. If you are interested in gaining hands on experience in event planning and recreation or meeting new people and having fun call Kelly at 630-784-6107 or email [email protected].

Sports Volunteers
Without a solid volunteer base, hundreds of local children would miss out on the experience of playing on a league team for soccer, baseball, softball and basketball. Please consider giving some time to the children of the community.
Event Volunteers
Help is always needed at our community events. General duties and responsibilities include: Assisting staff with set up, running activity stations, serving refreshments and clean up. As a volunteer you can make your own schedule!

Meet Your Park Board of Commissioners

The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by the community to represent the community’s best interests. The charge of the Board is to set policies that will guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait -a commitment to serving the park and recreational needs of our community.
Board MeetingsRegularly scheduled meetings of the Board are held on the second and/or the fourth Monday of each month according to a schedule adopted by the Board yearly. Please check our website for our current meeting schedule. Regular Board Meetings open at 7:00 pm.

Howloween Dog Parade & Costume ContestFriday, Oct 21Dress your dog up and be a part of the Haunted Halloween Happening at Armstrong Park! The parade will take place outdoors on the inline skate courts. Check in at 7 PM and the parade starts at 7:15 PM. Categories include best, scariest, funniest & cutest couple (dog & owner). Judges will be looking for creativity and showmanship & prizes will be awarded. Each dog will receive a treat bag! Dogs must be on a six foot leash and bring a flashlight.

Fright Fest at Great AmericaMonday, Oct 10Fright Fest at Great America is full of ghoulish fun! Experience the thrill of a roller coaster at night, check out a frightful show or get freaked out in one of the haunted houses. Registration includes transportation, supervision and admission. Haunted Halloween HappeningFriday, Oct 21 Celebrate Halloween at this fun family event. Enjoy games, crafts, entertainment and a warm bonfire. Children will also have fun hunting for candy in the haystack and bouncing in the inflatable. Costumes are encouraged and children must be accompanied by an adult. A majority of the event is held outdoors so dress for the weather! Register before Oct 20 for a discounted rate. Fee is per child.

Polar ExpressSunday, Dec 4 All Aboard! Come along on our annual train ride for a reading of the children’s classic story ‘Polar Express’. Bring a copy of the book and read along with us if you wish. We will ride from the Roselle train station to the ‘North Pole’ (Elgin/Big Timber). Santa will then join us on the train for the ride back to Roselle and sing holiday songs with us. Once we are back at the station, Santa will have plenty of time to visit with each child and hear their holiday wishes. Refreshments will be served and each child will receive a small keepsake. A parent or guardian must accompany children on the train and adults will also need to be pre-registered for this event. Please arrive 20 minutes prior to departure and parking is available. The deadline for registration is Nov 1.

Santa VisitsWednesday & Thursday, Dec 14 & 15 Why wait in line at the shopping mall to sit on Santa’s lap when Santa can come see you? Surprise a child with a visit from Santa right to your house. Santa will arrive in his ‘custom’ RV and listen to your child’s holiday wish list inside the RV! Due to his heavy schedule, Santa will stay for five minutes in front of each house and will only be able to visit with immediate family members. You will need to be available 5:30 to 8:00PM so Santa finds you at home. Please register by November 25 as Santa has many children to visit and will need to know his schedule early! Santa visits are open only to residents of the Carol Stream Park District. Santa Visits are sponsored by Hometown RV, located right here in Carol Stream. Teeny Weenie New Year’s Eve BallFriday, Dec 30 Are you too young to stay up until midnight? Parents and their young children can get an early start on the party at CSPD’s countdown to Noon party! Dance to the music with our disc jockey! Games, crafts, dancing and a fun balloon drop are all part of the fun. Festive snacks and a fun dessert will also be served. Cardboard Car Drive-In MovieTuesday, Jan 3Spend the last day of winter break with us! Bring your creativity and imagination as you make your own cardboard car! All supplies will be provided to you including the box! Once your car is complete, have a seat in it and enjoy the original Disney classic movie ‘Cars’! Refreshments, drinks and supervision are included in the fee. Bring your fabulous car home afterwards. The event will be held indoors in the Simkus gym. The Park District is partnering with the Carol Stream Police Department in collecting new toys for our community’s children in need. Pick up a gift tag on the Christmas tree in the Simkus Recreation Center lobby corresponding to a boy or girl of a specific age range. Then drop off your unwrapped gift with its gift tag at Simkus or the Police Department no later than Dec 9.
Letters to SantaEmail Santa at [email protected] the excitement of opening a personalized letter from Santa Claus! Your child can drop off a letter to Santa that we will deliver to the North Pole. Look for the special mailbox inside Simkus Recreation Center from November 28 - December 9. Please make sure that your child’s full name and address are included in the letter so that Santa knows where to send his correspondence or you can email him at [email protected]. Don’t miss out on this special holiday tradition that is sure to put a smile on your child’s face!

AquaticsSwim LessonsThe Bartlett Park District’s swim lesson program is for swimmers of all ages. Before registering for a swim lesson, review your child’s previous lesson history and age limitations of class. Due to the limited amount of space on our pool deck, parents will be asked to sit either in the bleachers or viewing area. For those who have already been in our swim lesson program, please sign your child up for lessons according to a previous swim lesson report card.
Classes fill quickly and transferring from one level to another is not always possible. If you have questions, please ask at the Registration Desk. Registration will close 3 days prior to the start of the class. Please note the time and dates of your lesson. No refunds or make-ups will be given for classes missed or forgotten. The park district reserves the right to combine class levels if deemed appropriate. Class Length: M-Th - 40 minutes. There are no classes the week of Nov 21- 26.
Parent and Tot Lessons (1-3yrs) Swim instructors will facilitate the lesson while parents act as the main instructor for the class. This class helps to develop comfort in the water, swimming readiness, floating with assistance, putting face in water blowing bubbles.

On the first day…
Please arrive 15 minutes early for first day check-in. Guardians may either wait for their swimmers in the bleachers or the viewing room. It is important to keep the pool deck clear so the instructor can have your swimmers full attention during the class time. Please stay for the entire swimmer’s class, in case of emergency where consent is required.

Semi-private lessonsLooking for a smaller group lesson? These lessons will have a 3:1 ratio and are tailored to meet your personal needs. Students need to sign up for their skill level and will be split up into a class with children with their same ability on the first day of class. Registration Guidelines1. Registration is by GRADE. Players
are to be registered for the grade the player will be in for the 2011-12 school year.
2. Preschoolers must be entering kindergarten for the 2012/2013 school year.
3. Register on line at csparks.org or use the registration form in this guide.
Wait List 1. Players placed on a wait list are not
guaranteed placement on a team.
2. Before team formation: Player will be moved off the wait list when there are enough to form an additional team.
3. After team formation: Placement will only be done when a team or teams are below the established maximum number of team participants.
4. Players will not be added to any team one week prior to the first scheduled game.
Requests1. One mutual friendship request per
player will be honored for players in Preschool thru Grade 2.
2. One practice day conflict form per player will be accepted for players at all levels.
3. We cannot accept any other coach, player, or carpool requests

Basketball Travel League - NEWThis league is for the more advanced and competitive player. Potential players must attend one of two tryout dates (Sept 18 or 25) and make the team. Teams will participate in the DuPage Youth Travel Basketball League. League will be supervised by the Park District but run by a parent volunteer group. Twenty game season (ten home games in Carol Stream) concluding with a post season tournament. Practices start the week of Oct 10. There will initially be three practices per week. Starting the week of November 7 there will be one practice and one game per week. The fee to participate is $200. The $50 tryout fee will be applied to the participation fee for players making the team and refunded to those that do not.

Gymnastics General Information• Parent viewing opportunities are

• Assessments are required for Grasshopper, Beginner, Beginner II, Advanced Beginner, and Novice levels. Assessments are also required for any of these levels if more than one session has been skipped. Call (630) 784-6105 to schedule a time.
• Only the registered child will be allowed to attend and participate.
• During parent/child classes and parent observation week, please make alternate arrangements for siblings. Siblings under the age of six may not attend evening class observation. Kids Korner on-site play care is available.

Dance Registration InformationThe Carol Stream Park District offers short term seasonal classes as well as long term recital classes.
EnrollmentDancers should enroll in recital classes during the months of August and September. Dancers are asked to enroll according to their age/grade as of September 12, 2011 and continue at that level for the remainder of the school year.
Recital classes run September 12, 2011 through May 11, 2012 and will conclude with our year-end recital in May 2012. The costume fees are already included for all recital classes. Costumes will be ordered the first week in December and costume fees are non-refundable after the order is completed.
Holiday BreaksNovember 21 through November 26, 2011, December 21, 2011 through January 3, 2012 and March 26 through March 31, 2012.
PaymentPayment for recital classes can either be remitted in full at registration or can be remitted via our monthly payment plan. All recital classes have the option of using EZ pay.
Required Class AttireHair must be secured up and away from face. All dancers are required to wear leotards or unitards, tights and the appropriate shoes. No over-sized sweatshirts or t-shirts should be worn. Please do not bring toys, stuffed animals and jewelry to class.

WAIVERREGISTRATION/BROCHURE WAIVER & RELEASE IMPORTANT INFORMATION
The Carol Stream Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the above listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.
WARNING OF RISK
Recreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and injuries due to inclement weather, slip and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities exist. In this regard, it must be recognized that it is impossible for the Carol Stream Park District to guarantee absolute safety.
WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK
Please read this form carefully and be aware that in signing up and participating in the programs listed, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a Result of participating in any and all activities connected with and associated with these programs (including transportation services and vehicle operations, when provided). I recognize and acknowledge that there are certain risks of physical injury to participants in these programs, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in these programs against the Carol Stream Park District, including its officials, agents, volunteers and employees. I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering on-line or via fax, my on-line or facsimile signature shall substitute for and have the same legal effect as an original form signature. Participation will be denied if the signature of adult participant or parent/guardian and date are not on this waiver.

PaymentFull payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/AMEX/DISC). Make checks payable to the Carol Stream Park District; There is a $20 fee for returned checks.
Program CapacityAll programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another. If the program is cancelled, full credits will be issued.
Waiting ListsOnce the maximum enrollment is met for a class, a waiting list will be formed. We will create additional classes when feasible and you will be contacted. Credits will be processed after the first scheduled class.
InsuranceThe Park District does not carry medical or accident insurance for program participants or park patrons. Please review your personal health insurance plan to be certain that you and your family have proper coverage.
Financial AssistanceWe offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications are available at the Simkus Recreation Center.
Satisfaction Guaranteed!Our goal is your satisfaction! If you are not completely satisfied with any Carol Stream Park District program, class or event, please tell us! We will work to earn your satisfaction. If you are still not completely satisfied, we will transfer your registration to another program or session, or issue you a refund - that’s our guarantee!

Love dogs? Sign up as a volunteer for our third annual CSBarks dog festival on Sept 18!. Over 500 dogs are expected to attend, and we need volunteers to help run games, supervise the off-leash and Rover Rest areas, take photos, or help with set up and take down.
Got a group? Volunteer opportunities are also available before and after the event with set up and break down. If you are interested in gaining hands on experience in event planning and recreation or meeting new people and having fun call Kelly at 630-784-6107 or email [email protected].

Sports Volunteers
Without a solid volunteer base, hundreds of local children would miss out on the experience of playing on a league team for soccer, baseball, softball and basketball. Please consider giving some time to the children of the community.
Event Volunteers
Help is always needed at our community events. General duties and responsibilities include: Assisting staff with set up, running activity stations, serving refreshments and clean up. As a volunteer you can make your own schedule!
